Employee was spinning anvils in a Vertical Shaft Impact Crusher. The lid of the VSI came down upon his left hand. Employee's left hand was caught between the lid and frame of the VSI. The ring finger of his left hand was seriously injured.

Mostrando los 5Employee was using an impact wrench to loosen the bolts on the cutting edge of a loader bucket. He reached under the edge of the loader bucket to locate the bolt head. He then engaged the impact wrench. The bolt head on the underside of the loader bucket spun, caught the employee's glove, and lacerated his left middle finger.
EE WAS INSTALLING A SCREEN. HE WAS LYING ON HIS BACK AND USING AN IMPACT WRENCH TO TIGHTEN BOLTS. A METAL PARTICLE BECAME LODGE IN THE EE S EYE. EE WAS WEARING SAFETY GLASSES.
EE WAS OPERTING A CUTTING TORCH & HOLDING IT IN HIS RIGHT HAND. THE CUTTING TORCH FELL FROM HIS HAND AND THE FLAME FROM THE CUTTING TORCH CAME IN CONTACT WITH THE RIGHT SIDE OF HIS LEFT INDEX FINGER.
EMPLOYEE WAS WELDING. HE ACCIDENTLY TOUCHED THE HOLT WELDING ROD TO INSIDE OF LEFT UPPER FOREARM, CAUSING A BURN. 2ND DEGREE BURN.
