EE was grinding and when they raised their face shield to use the hoist after completing grinding some debris fell from the rim of their hard hat and into their eye. Full release, no restrictions. Received an ointment in the eye at the Dr.

EE was pushing bolts when EE began to feel soreness in right shoulder.

EE had to chip out frozen material in conv. 11's tail pulley. Due to the fact the cover does not come off EE was on the ground working overhead while pushing the electric chisel into material.

EE was jarred in the seat when EE's piece of equipment was bumped by another piece of equipment.

Using an impact gun to install transmission mounts, the gun pulled EE's arm into the transmission causing EE to strike EE's elbow against the transmission.

EE was attempting to secure a second clamp for a wire mount when the drill bit gripped the strap causing it to strike EE in the face and upper lip. EE received stitches on the lip and was released to work with no restrictions

EE was running a jackleg performing bolting when the pressure of the jackleg pushed on the basket of the JLG and tipped it towards the ground. When the basket impacted with the ground the jackleg struck EE in the right shin causing a laceration. First aid at the scene and EE was treated at clinic with staples and released. No restrictions or missed days.

When the crane was lowered to the ground when it was done being used, it came to rest in what (at the time) was thawed snow/mud. Overnight the thawed snow/mud had become frozen. When the crane operator began to use the crane, it was observed that partial ice had formed and was not cleaned as well as thought. Falling ice/mud chunk hit and broke hard hat.

EE was using a drill, the drill bit broke causing a laceration on right 5th digit.

EE was welding grinding when dust got into his eye. He flushed his eye out but was sent to the Dr. Dr's administered a prescription in his eye making it reportable.

EE went to impact a nut and had his hand under the impact, another employee went to move the basket catching his hand between impact and the basket rail.

Unknown- EE's wrist is sore while jack legging. He has a small bump on his wrist that the Dr. is evaluating by having him wear a splint for a week.

EE was jacklegging and a piece of material fell into his eye. It was removed with no restriction, just an antibiotic prescription drop for his eye.

One employee was moving the strap from one pin of the hydraulic quick coupler to the one on the other side; he was having troubles lining the strap loop with the pin. EE came over and grabbed the strap. EE got his finger stuck in a pin hole. He smashed the tip of his finger and the skin and meat were removed down to the bone.

EE was grinding in the shop and he was wearing safety glasses and a welding helmet and he felt something go into his eye. He used Visine and thought it came out. A couple days later he told his supervisor he thought it was still in there and reported it to the safety manager. He then went to the eye doctor and medical doctor and had the material removed.

He was using a cutting torch to cut a water tank and it shot a hot piece of steel off the side and it went into his eye. He was wearing a side shield and a face shield.

EE was grinding counterweight for the loader when a piece of metal deflected off it and flew under ee face shield and safety glasses. Had to go to Elkader Hospital to have it dug out.

EE was spitting sand samples and got hand caught in sand spitter causing bruising to all 4 fingers on her right hand. Possible Fracture on 3rd finger.

EE was jacklegging bolts into overhead ceiling, he dropped drill on left forearm and wrist causing pain and sprain to left wrist.

While using an impact wrench to drive a rock bolt, a piece of debris from the exhaust entered employees right eye.

While drilling 4' hole drill steel broke when the hole was almost finished, causing drill steel to fall out of hole striking employee in the hand.

Did not wear PPE - face shield and/or eye shields while cutting steel with a torch, causing flashburn to eyes.

Employee was running a jack leg when he realized that the ventilation bag would be in his way, he went to move the vent out of his way. The bag caught on the jack leg causing him to have a strain in his back.
