EE was tramming a highwall drill from the south side of Grapevine to the north when the left side track locked up. EE was seated an the sudden stop threw them out of the seat against the door. Head hit door frame causing a headache and shoulder strain as well. Became a lost time accident on 8/17/2020

EE was removing a canopy from a shuttle car. He was sitting under the canopy & when he removed a bolt, the canopy dislodged & struck him on the right shoulder. EE was taken to the hospital & was diagnosed with a compression fracture of the No. 12 vertebrae. He had a follow up visit with his family physician on 6/8/2010 and returned to work on 6/9/2010.
While installing an o-ring in a 777 rock truck tire, the hydraulic jack employee was using flew out striking employee in hand.

EMPLOYEE INFORMED HIS SUPERVISOR AT QUIT TIME THAT HE HAD PAIN IN HIS LOWER BACK & LEFT LEG. THE PAIN STARTED SOMETIME AT THE BEGINNING OF THE SHIFT & AS THE NIGHT WENT ON IT GOT WORSE. HE STARTED MISSING WORK BECAUSE OF THIS ON 9/2/04 & IS STILL OFF WORK.
EE, DRILL OPERATOR, SET THE JACK PAD OF THE DRILL ON A ROCK. THE PAD SLIPPED OFF THE ROCK, JARRING EE. (ON 11-17-03 EE CALLED THE MINE CLERK, & STATED THAT THE OCCURRENCE AGGRAVATED A PREVIOUS INJURY.)
