Employee was changing a piece of steel on the DTH. To keep balance, employee laid left hand on the track. Employee's glove got caught in the chain and pulled hand into the gear. The pinky finger and ring finger were completely severed.

Attempting to straighten a 12" anchor pin with drill press. Pin popped out of hydraulic press and hit his right hand - 5 stitches.
Transferring diesel fuel from transportable tank to generator tank, fueling hose was inhibiting operation of crank-style pump. Employee moved hose to allow cranking, hose slipped off nozzel, spraying fuel at other employee in face, eyes, front, causing ingestion of fuel. Immediate vomiting. Employee was sick 2 hours, finished shift to 10:00 p.m.
Drilling with hand drill when the fitting on the drill came off and hit the back of his hand. Bad bruise on right thumb and back of hand. Employee had performed all safety precautions.
