EE was cutting piece of steel with 21" torch out of the stilling well in 4 Tails Hydro. EE cut from the bottom of seam leaving ~6" gap in middle, then cut seam from top down. The piece of steel shifted, broke off, and fell backwards over top of the rake making contact with EE left arm and face shield resulting in stitches to the nose.

Employee was holding a bolt with the left hand and started to use an impact wrench to loosen the bolt. The bolt head began to spin in the hand resulting in a laceration of the left middle finger requiring sutures.

Employee got a piece of slag in their right eye while a nearby employee was using a grinder.

EE was chipping concrete. While chipping, EE felt material enter the right eye.

A Maviro employee was performing HPWB operations at the United Taconite mine, when a 10K PSI dump valve failed to properly dump water and cause the injured employee to lose their balance and hit the back right side of their head on the hand rail behind them.

Employee was cutting a rubber hose with a battery-powered, handheld cutoff tool. The wheel caught and kicked the tool back causing the cutoff wheel to cut through the employee's leather work glove and into their left wrist. Resulting injury required 7 sutures.

Crew was hydroblasting sludge off the walls at Utac Pellet Plant Line 2 Scrubber Rod Manifolds. A large piece of sludge came loose and landed on the employee's leg.

While building a plywood form, a worker was drilling a hole from the bottom and the bit came in contact with the workers hand above.

Setting up a hoist to lift gear cover off an engine. Injured employee was giving commands to mine mechanic who was running the hoist remote. Injured employee had said "hold on" but the mine mechanic continued to lift, causing tension on the chain and pinning EE's finger against a water pipe. The injured employee suffered a pinching laceration that required 12 stitches.

Injured EE was using a 6" powered hand grinder to remove the old bolt flange on the West #3 scrubber inlet. While cutting off flange the blade pinched in the cut resulting in the grinder kicking back and striking the EE in the left neck/collarbone area causing a laceration.

Employee was drilling into concrete. The drill bound up and spun and hit them in the cheek.

The employee was operating the Brokk machine into the kiln. The ramp was pushed off the discharge lip of the kiln causing the employee to fall into the cooler area below. The employee sustained fractures to the pelvis, vertebra, and ribs. As well as internal damage to the rectal area.

The injured employee was arc airing a crusher mantle liner when a hot piece of slag contacted the employee's coveralls, burned through and caused a small on left ankle. It was treated as a first aid injury. Weeks later, it became infected and became a lost time on 8/31.

Employee was using a 6" die grinder with a burr bit to clean welds as the employee progressed with welding. Employee got a small piece of metal in right eye.

While attempting to remove a bolt, the EE was operating 18 volt cordless drill and pressed the trigger with the intent of loosening the bolt but it was set for tightening instead. This movement jerked the wrist causing pain. On 2/11/17 EE sought medical attention and was diagnosed with a wrist fracture.

The EE was jack hammering concaves when the bit became lodged and became stuck. The EE retrieved a new tooth and was working near area of the stuck tooth when a section of the concave broke off and shot out due to the stored energy in the liner. The chunk of concave struck the employee in the jaw and neck.

Employee was stepping on the push arm of the 9233 dozer when his foot slipped off causing pain in left shoulder. It was dark and muddy. The slip caused a shoulder strain

Employee was using a 4-1/2 inch electric grinder while fabricating handrails, he set the grinder on the table before the wheel had stopped spinning, and the sanding flap wheel caught his glove and sucked it into the grinder between the wheel and the guard. The incident became reportable on 11/6/13 when the employee did not report for his scheduled shift.

Employee was using 6" grinder to clean up sharp edge when a small piece of debris got behind his safety glasses and face shield.

The employee states he was at work using machinery to dig through rocks and hand jarring that day. The jarring increased his neck and upper back pain. He stated that he has treated for many years regarding his neck and back pain. Discussions with the clinic indicate this is an exacerbation of a degenerative disease/illness not an injury.

The employee was operating a dozer pushing in a road when he reported that his back became sore. This is a reaggravation of a preexisting condition.

While grinding during welding classroom training, employee felt something in his eye, he was sent to the clinic where they removed the particle and prescribed antibiotic drops.

Working off a ladder platform Left elbow started aching after a couple of hours using the steamer on haul truck. Employee sought medical treatment on evening of 3/18/10 diagnosed with repeat trauma occ illness.

In the process of grouting new base for a replacement pump. He was mixing grout in small 5 gal portable mixer. Finished mixing up a batch, shut off mixer tried to remove pail from mixer but pail was stuck. While trying to free pail mixer started up: his hand spun round and round causing a sprain and possible fracture. Fracture ruled out at a later date. The sprain required a splint

Employee was in the process of removing right rear wheel on 9029 Loader using impact gun. Gun popped off the bolt, employee reacted to catch it, hitting the back side of left elbow on the rim of the wheel. States elbow hurts cannot fully straighten arm. Sought medical treatment.

Using air impact wrench to tighten bolts on case of 3b tailings pump. Impact wrench slipped out of employee's hand, due to "never sieze" on his gloves. Smashed his right hand pinky finger between handle of impact wrench and pump pedestal. Employee reported on 9/8/09 that he sought treatment at clinic. Received diagnosis on 9/9/09 statinig he had fractured the pinky finger.

Mechanic was pressure washing cooler carrying wheels in steam room. The wheel had been cleaned, standing upright. The employee turned around to clean the next wheel. The wheel behind him began to roll towards him. His partner yelled a warning but the employee couldn't hear due to noise. The wheel tipped over, fell, pinned employee's right leg/ankle to the pallet and floor.

After grinding on crusher parts employee lifted up grinding shield and loose material fell into his right eye

Running sawzall hand slipped and contacted blade. Employee required stitches to close the laceration.

While replacing wear plate on 57 loader employee was holding plow bolt in place by hand while his partner was removing the nut with an impact wrench. Plow bolt turned in place and cut gloved hand of employee. 7 stitches were required.

Individual was turning the self propelled power floor sweeper as she was walking behind it. She stated the sweeper can be difficult to turn in some instances when making sharp turns. She did not report the problem until 8/10/07 but the physician indicates this caused a flare up of plantar fasciitis (repeat trauma).

Attempting to break free bridge of material caught in #1 feeder of Line 1, jack hammers and jack hammer bits used. A large quantity of chunks gave way hit end of jack hammer and jack hammer bit. Both tools pivoted upwards and struck employee in right shoulder, neck and head causing a cut on head that required two stitches

Employee was assisting to unplug #3 secondary crusher, noted that he was sore from manually handling rocks, pry bars and jack hammer. Employee has had numerous work related and non work related injuries involving his lower back. This is a reoccurrence/reaggravation of a old injury.

Worker on top of eng. module on Hyd. shovel installing air cleaner assembly, switching safety harness lanyard from one eye to another. Crane swinging away from area, glove became entangled with hook as he was pushing hook of crane out of the way. Worker was suspended until glove tore. He fell to ground resulting in fracture of left wrist, and acute L2 compression fracture.

PROCESS OF REMOVING PIE LINERS FROM FEED AND #3 BALL MILL. ATTEMPTING TO REMOVE LINER WITH HYDRAULIC SCISSOR JACK LINER FELL UNEXPECTEDLY STRIKING EMPLOYEE ON RIGHT ARM RESULTING IN LACERATION REQUIRING STITCHES

Straightening bent steps on 9191 truck using overhead crane a long 5' pry bar; while doing so employee hit the down button on crane control instead of up button causing bar to strike him in the ribs. Received results from physician office on 6/3/05 indicating no fracture but restrictions.

PREPPING IRON WITH A 4" GRINDER. WORKING IN AWKWARD POSITION. AGGRAVATED LOWER BACK PAIN

EE WAS USING AIR HAMMER TO PUSH LINER BOLTS THROUGH THE SHELL OF THE BALL MILL. THE BIT BOUNCED OFF A BOLT AND PINCHED RIGHT THUMB BETWEEN THE BIT AND THE MILL SHELL. EE WAS GUIDING THE BIT WITH GLOVED HAND

INJURED WAS USING A 4" ELECTRIC HAND GRINDER TO CUT 2" ANGLE IRON. THE GRINDER KICKED BACK, HITTING EE'S LEFT INDEX AND MIDDLE FINGERS, RESULTING IN 6-STITCH CUT.

Cutting hole in classifier flight to get bolt to fit, while cutting piece of slag rolled down pant leg and burnt hole through coveralls, burns to right ankle. Report from doctor stated whirpool treatment required.

EMPLOYEE OPERATING DOZER, CUTTING DOWN RAMP WITH DOZER. WHILE BACKING UP EE RAN OVER BOULDER AND THE EMPLOYEE CAME DOWN HARD JARRING HIS BACK. ON RESTRICTED ACTIVITY & RECEIVING PHYSICAL THERAPY.
