I.E. was hooking up a tube strap when EE's finger became hung in the hook and when the strap was released EE's finger was pulled in a non-functioning direction.
I.E. was injured when EE put EE's hand on EE's drill pot to check suction and the drill steel that was hung in the roof fell out smashing EE's finger
I.E. was putting up a rib bolt when it became hung while EE was pushing it in. When EE tried to push it in EE felt a pop in EE's left shoulder
I.E. stuck a hand over the drill chuck to check the suction on the drill pot. The drill steel then slid out of the hole and pinched a finger. Possible ligament damage.
I.E. was helping shive a shear drum off the face of the longwall when the winch rope hook slipped off the drum striking EE in the face.
The IE was in the process of replacing a wear liner in the rock chute at the prep plant. When EE was removing a piece of the liner, EE felt a sharp pain in lower back.
I.E. was using a grinder to cut a power cable. While cutting the cable the cutting wheel broke apart striking I.E. in the left index finger. I.E. received 4 sutures.
IE had just put two roof bolting steels together and was putting up pressure to drill the hole. The two steels were not completely together, and a portion of the nipple was sticking out. This caused the nipple to break and the steel to swing and hit IE in the right arm. IE was diagnosed with a hairline fracture.
IE was roof bolting in HG-17 putting up cable bolts. IE went to push the cable bolt up in the hole and contacted the metal with the top side of their wrist. IE received 4 sutures.
IE was putting up carrier cable bolts in entry 2 right on HG-17. IE tilted back the platform and some bolts fell off of the tray. When IE tilted back forward. The bolts flipped upwards striking IE in the wrist. IE was given a wrist brace the next day at the doctor.
IE was trying to winch the longwall face mesh up out of the way of the roof bolter. IE flagged the roof bolter to stop tramming but the atrs contacted the mesh and spun the boat winch handle. The boat winch's handle contacted IE's right hand which resulted in a fracture.
IE just loaded roof bolter tray and was lowering the canopy. IE was talking to partner while lowering the canopy and had right hand resting on the roof bolt tray, resting against the bolts. When the canopy got low enough, it pushed down against the roof bolts and IE pinched palm region between two roof bolts that were in the tray. IE received 7 sutures.
IE was bolting entry 2 right installing 16' straps. The top was uneven and when the operator pushed the ATRS in the up position. The 16' strap hit the top on the operators side causing it to slide down the ATRS. When the strap slid down, it hit IE's middle finger on the right hand. Resulting in a broken middle finger below the first knuckle.
IE was changing parts out on B-East screen bowl. Taking chute out of building with a 5 ton hoist. When the chain slack went out of the building it whipped hitting the chain bucket causing it to fall 2' hitting IE in top of the head. IE was taken off work until July 7th. Injury/illness is still unknown.
IE was pinning and got the top steel stuck in the roof after removing the bottom steel. IE couldn't get the top steel unstuck so IE ran the chuck up to the stuck steel trying to align it. IE accidently hit the wrong lever while having hand on the chuck. IE smashed left hand's (index) finger between chuck and steel. Index finger was amputated at first knuckle.
IE was trying to get a drill steel unstuck from the drill chuck. IE used a four foot bolt to push the drill steel out from the underneath side of the drill pot. IE put down pressure on the four footer but also hit rotation. It rotated the four foot bolt which struck the IE's left hand. IE received a fracture right above the pinky knuckle.
Management was informed of an accident on 3/30/2022 that occurred on 03/24/2022. IE stated they were operating a tractor and bumped an elbow on the frame of the tractor in the cab area. The resulting injury was chipped bone spur to the left elbow.
IE was installing a 6' roof bolt into the bolt hole when EE felt a "pop" in the left shoulder and neck area. This resulted in some numbness and tingling in EE's arm and fingers.
IE was operating a roof bolter. A roof bolt was hanging out of the pin tray farther than the other bolts. When EE raised the boom the bolt was flipped up striking EE's right eyebrow causing a laceration. The IE also suffered a minor concussion and was removed from work the following day.
IE was installing the last bottom roller for a belt move. A boat winch is used to raise the lower belt. The IE released the boat winch and winch handle struck EE's hand resulting in a fracture small finger.
IE was operating a roof bolter installing a rib angle pin. When pushing the bolt up and into the drill hole, IE felt a "pop" in their left shoulder.
IE was operating a bolter. IE rested drill steel on foot and proceeded to drop the drill pod. The drill pod struck the steel and smashed EE's foot causing a contusion. (X-rays Negative)
EE stated they operated a hilti hammer drill on 09/03/2020. EE reported to the safety dept on 09/09/2020 that they had been having a sharp pain in left shoulder since operating the hilti drill on 09/03/2020. EE sought medical treatment and the file report has been modified to a LTA on 02/17/2021.
IE was cleaning the "chuck" on the drill pot. IE left the drill steel hanging from the roof bolt hole the IE was drilling. The drill steel fell down striking IE's left hand between the drill steel and the drill pot. This resulted in a fracture of the left hand.
The IE was attempting to pull a LW shield on to the "duck bill" of a shield hauler for transport. A nylon winch rope was hooked to the shield in preparation for pulling the shield onto the bill. As the winch rope was tightened, the tip of the foundry hook hung on the edge of the bill. The IE reached in to free it when it suddenly slipped from the bill striking EE in the chin.
IE was installing bottom belt rollers during a section belt move. Boat winches are used to lift the bottom belt for installation of the rollers. With the bottom belt lifted and the winches under load, the locking switch was inadvertently released allowing the winch handle to rapidly release causing the handle to strike the right hand of the IE.
Employee was attempting to pull the monorail trolley down with the diesel tugger. The monorail kinked causing the trolley to lock up. The tractor pin connecting the tugger's nylon winch rope to the trolley broke. A clevis struck the injured employee in the right hand causing a laceration requiring stitches.
Employee was using a 4 1/2" angle grinder with a wire wheel to clean of the top of a 20' section of pipe prior to welding. The wire wheel came into contact with the draw string of the employee's sweatshirt which caused the wire wheel to come into contact with lower lip resulting in a laceration/avulsion.
Employee was cleaning the cable tray for the shear cable when a piece of coal was thrown from the shear contacting the employee's left index finger. The left index finger was fractured as a result.
Employee was hanging mesh near the tip of a longwall shield during the preparation of finishing up a panel. Employee hung a piece of mesh on a hook told another employee to set the shield tight. He then stepped back a few steps and laid his hand on the side of the adjacent shield. As the first shield set three fingers on his right hand were caught between the two shields.
Hydraulic jack was being used to lift the screen in forder to change the spring. The screen was raised and the jack was under load. The jack suddenly kicked out. The base of the jack struck his shoulder/collar bone.
While cutting top, a miner bit broke off, striking the injured person in the head, cutting him above the eye.
Pulling on drill to unchuck 6th steel, felt pain in stomach, hernia.
A rock flipped from the face conveyor striking the operator.
Employee was assisting in cleaning concrete out of a pipe. When the air line was turned on, the shut-off valve broke and the hose was whipping around. The end of the hose struck employee on the inside of left knee.
EE WAS ASSISTING WITH THE MOVING OF THE CONTINOUS MINER. HE PLACE HIS RIGHT HAND ON THE PAN AND BENT OVER TO PICK UP THE MINER CABLE. HE ASKED THE MINER OPERATOR TO RAISE THE PAN. WHEN IT WAS RAISED, HIS HAND WAS PINCHED BETWEEN THE PAN & THE STOP ON THE CUTTER HEAD. HIS HAND WAS BROKEN IN SEVERAL PLACES.
