While drilling out a broken bolt with a magnet drill on a D7 roller frame, EE was running the drill while magnetized to the frame. As the bit finished drilling through the broken bolt it bound up causing the drill to spin around pinching EE's fingers between the drill and the frame.

Employee was welding on a scoop with a plasma welder. Later he noticed his eyes were burning and irritated and went to the doctor to get checked. He was treated and released for minor UV exposure to his eyes and referred to an eye doctor. This is to report medical treatment only NO days were missed.
Employee was cutting a tree. When the tree began to fall a limb fell and hit employee knocking him to the ground breaking his leg.
While tramming the backhoe across the railroad crossing the employee inadvertently hit the bucket lever. The caused the bucket to drop and throw the employee into the windshield. This caused him to hit his head and nose on the windshield. He was later diagnosed with a small fracture to his nose. This is to report medical treatment only NO days were missed.
On 6-14-12 the employee was helping remove a bearing off a miner planetary when a piece of metal broke off striking him in his left forearm. The employee continued to work until 7-10-12 when he had the piece of metal removed. The employee received sutures after the metal was removed and remains off.
While the operator was attempting to line the bolt head up with the drill chuck, the joystick rotation control was accidentally hit causing his right hand and arm to be jerked. This resulted in a strain and sprain injury to his right hand and arm. This is to report medical treatment, employee missed NO day but remains on restricted duty.
At approx. 11:45 employee informed his section boss that he had been involved in an accident. Apparently the employee was walking past the #1CM in the #2 entry inby spad #2326 when the miner pinched the employee against the coal rib.
Employee was tramming bolt machine from #2-#1 entry from the deck. The T-bar hit the rib he placed his right hand on the canopy to raise himself to see. Just as he did the bolter shifted due to a downhill grade causing the fingers on his right hand to be pinched against the roof. This resulted in lacerations that required sutures. No days were missed however he is restricted duty.
Bolt machine was tramming up entry 3 in rehab area F. Conditions were wet and slick and at an incline. As operator was tramming from deck employee seen cable in a pinch point and reacted by trying to move cable. As he moved cable bolt machine slid sideways and his knee was caught between rib and ATRS pad. He fell and twisted knee. He continued to work for one Hour
Employee hit the lever on the bolter causing boom to drop on his right leg.
Employee was cutting old track rail with O&A torches, when a piece of hot slag entered his left eye under his eye protection.
Employee was installing resin bolts in browe of water ring. He was on level muckpile drilling. Strain/hernia.
Employee was roof bolting in the face of #4 entry. The drill steel fell out of the top and struck his right thumb, lacerating it. He received 3 sutures. This is to report medical treatment only, there was no lost time due to this injury.
Employee was pulling drill out of hole, strained back.
Employee was cutting a piece of conduit with a torch and the conduit swung around and struck him in the face resulting in a laceration to the lip and possible tooth damage. The laceration was treated with stitches.
While painting I-beams in the shop EE attempted to clean the tip of a high pressure spray gun with his index finger. He inadvertently pushed the trigger which caused a small incision that allowed paint thinner and aluminum paint to be injected under his skin. Surgery was required for treatment.
CMO was place changing the miner from #1 to #2 entry while positioned on the inby side of the intersection in #2 as he tramed the miner backwards towards #3 entry. As the machine went by he placed his right foot between the cable and the head to protect the cable and his foot was pulled under the head. He fell backward and fractured his r. tiba and fibula near his ankle.
While backing up the continuous miner after cleaning up some rock in the No. 1 entry the miner operator caught his right leg between the cable standoff and the rib. The operator sustained a fracture to his right femur.
While drilling a roof bolt from the operator side of the fletcher roof bolter the utility man laid his left hand on the drill head. This resulted in his left hand being caught between the drill head and the mine roof. His middle finger was amputated from the tip to the distal joint and his index finger from the tip to the base of his finger nail.
While installing a 6' resin bolt the resin begin to set up before the bolter operator got the bolt fully installed. When he begin to pull the bolt from the hole he caught his rt. thumb between the roof bolt plated the drill area. this resulted in a laceration requiring 5 sutures to case. this is the report medical treatment only.
