Employee left a wrench on a drill and forgot about it. Employee then turned the machine on causing the wrench to spin and come off and hit the employee in the ankle.

Mostrando los 7IE and another EE were removing the steels from a Fast 65 hole. The bottom steel didn't come loose and when it turned the wrench hit IE in the left ankle.
EE was moving wire when another employee attempted to start the wire saw twisting EE's finger. EE said the accident happened on 9-16-2013 but did not report the accident to anyone until 12-16-2013, 3 months later. He was at work everyday since 9-16-2013 and continues to work up to the time of this report. EE went to the doctor on and they said he might need surgery.

Pulling a block out from wall with Kamatsu PC400 track hoe and cable. The shackle broke and 1 piece came through windshield, hitting operator in the head and went on to hit the back window.
Employee was holding a wrench to loosen the drill steel from hammer head, when rotation started, pinning his thumb between the wrench and frame of machine. His left thumb had a crushing type injury.
