Employee was loosening bolt with an impact gun. The bolt came loose, striking the employee above the left eye on the forehead. Seen at ER where stitches were required. Released to full duty.
Employee removing bolts from discharge pipe on barren pump. Nut came out of socket that was still in motion and struck him in top front 2 teeth and broke them. Saw dentist and will be having a root canal and caps placed on teeth. Employee released to full duty.
Employee was performing a retorque on the left rear of a Komatsu 930 with a RAD 30 gun and he smashed his finger. He was seen at the ER where xrays showed a fracture to the left 4th finger. 5 sutures required to close the laceration.
While changing hoist ropes on a shovel, ee right foot was sucked in between the equalizer and ropes. The tension on the rope got tighter, clamping the ee foot. Xrays were negative for fracture, diagnosed with a strain. Released to restricted duty, on crutches and boot. Placed off work.
While using a pneumatic impact wrench, employee felt something enter his right eye. The eye was flushed but debris remained. The next day the employee was taken to Dr's office and a piece of metal was removed and the employee was given an eye patch to wear for 24 hrs. He was given some eye drops and released to full duty.
Emp strained upper back while twisting to adjust radio in equipment. No lost time, no restricted duty. Company was unaware employee was seeing chiropractor for treatment of strain. Investigation into treatment by work comp carrier confirmed on 01/04/2011 that employee was treating for work related injury resulting in this report for medical treatment.
Employee was disconnecting the portable pump discharge hose when the solution sprayed on his hands and burned him. He was treated at Battle Mountain Hospital and was given a full duty release with physical therapy to follow.

The employee was building some shelves and plugged the router in. As he plugged the router in, it fell off of the shelf and the employee tried to grab it causing a laceration to the middle finger on the right hand.
Getting loaded and bucket came in and a boulder fell off the teeth onto the tailboard, shaking the truck. Didn't feel anything until the next day. She was taken to NNRH and given a full duty release. Turned into RDI on a follow up visit on 08/08/08. Her shoulder was strained.
