Employee was standing on a mezzanine wearing their harness to cut off a handrail. While cutting the handrail, the abrasive cutting wheel got caught in a bind which caused it to break and throw pieces of the disk. One of the pieces hit the employee in the knee causing a laceration.

Trucks hauling wet mica material onto stockpile. Trucks had to be pushed out by the dozer. Dozer was pushing truck 2362 Truck caught traction causing a gap.
