Crew was completing a stopping in the intake with a large amount of air. Crew was using mine mortar in between block joints, pieces of mine mortar and small debris went up under employee's safety glasses and into eye. Crew leader flushed the eye out 3 times after the incident.

A rock jam occurred at 5 shield and employee was holding the rock breaker as another employee functioned the shield. They attempted to lower the shield 3 times and it did not drop.
