A Mine Production employee was operating bolter drilling a 14 foot drill hole when the steel became stuck. EE placed EE's left hand on the drill steel to apply outward force to free the steel and reached for what EE thought was the lowering lever and inadvertently activated the rotation lever. In doing so EEs hand rotated with the drill steel causing a dislocated left pinky finger.

Employee was grinding on some metal and claimed a foreign body from material, flew past employee's face shield and safety glasses entering the left eye.
Using a battery-powered chainsaw to cut out foam stopping. Cutting upward so EE placed hand underneath and when EE did this, left ring finger entered the clean-out area of the chainsaw, resulting in EE's finger contacting the chain causing a laceration. Had on a cut-resistant glove which got pulled off, wadded up in the chain causing the brake to be applied to stop.
