Employee initially stated that they began welding and the auto darkening feature on their helmet was delayed, which they believed caused a burn to the eye. They sought evaluation today, 1/7, and was diagnosed with a foreign body of the right eye.

While using a drill press the sheet of metal was clamped down but started to move. EE tried to secure it with EE's left gloved hand. As EE was securing it EE's glove caught the drill and entangled EE's left hand and thumb into the drill.

Using an angle grinder to prep the scalper lip for welding. During the process of grinding, the employee's safety glasses slid down EE's nose which resulted in debris going in EE's left eye.

Employee was using a skill saw to cut rubber board. Was at end of cut and blade jammed and kicked back. Employee let go of trigger but blade had not stopped, resulting in cut to right thigh.

While cutting a piece of angle iron with a chop saw, the blade jammed which pulled the angle iron. The iron cut the employees gloved finger. He recieved 4 stitches and returned to work with modifications.

Piece of metal lodged in right eye.

PUTTING PINS IN EXCELL CONE - RUNNING CYLINDER TO LINE PIN WITH HOLE. RESTED HAND ON BOTTOM OF CYLINDER TO RETRIEVE TOOL WITH RIGHT HAND, CYLINDER RETRACTED, PINCHING LEFT HAND/THUMB.

While welding on the crusher, hot sparks burned through his pants and went down his leg into his boot, burning his left heel. Employee failed to use provided PPE as directed.

Employee was in a manlift vertically fitting a 2" pipe that was 18" long. Employee welded a tack and as he adjusted the pipe the tack broke and the pipe struck his upper lip. Employee recieved stitches and was released back to regular duty. All appropriate PPE was worn.

During a cone liner change the employee was cleaning the crusher component with a wire wheel grinder and air nozzel. Debris (assumed metal) went in his eye. He then rinsed out his eye and later went to the doctor. He was wearing safety glasses at the time of the incident.

WHILE CHANGING CUTTING EDGES ON A 988F LOADER EMPLOYEES WERE USING A ROUGH TERRAIN FORKLIFT TO MOVE THE EDGES. INJURED EMPLOYEE WAS ADJUSTING THE CUTTING EDGE ON TO THE FORKS AND PLACED HIS LE FT HAND ON THE TOP OF THE FORKLIFT BACK REST AS THE FFORKLIFT OPERATOR TILTED THE FORKS BACK, PINCHING THE EMPLOYEES FINGER BETWEEN THE BACK REST AND THE BOOM HEAD. FAILURE TO COMMUNICATE BET

EMPLOYEE WAS USING A 2-TON JACK AS A SPREADER TOOL UNDER A FORKLIFT WHEN THE PUSHING BLOCKS SLIPPED ALLOWING THE JACK TO FALL ON THE EMPLOYEE'S FINGER WHILE HE WAS TRYING TO CATCH IT. THE JACK WAS TOO LARGE FOR THE JOB AT HAND. CAUSATION: WRONG TOOL FOR THE JOB. EMPLOYEE SUFFERED A FRACTURED FINGER. NO PROPERTY DAMAGED.

WHILE REMOVING A BOLT FROM THE SCREEN WITH AN AIR IMPACT WRENCH, THE EXHAUST PICKED UP A PIECE OF METAL AND BLEW IT INTO THE EE'S EYE.

WHILE TIGHTENING SCREEN BOLTS WITH AN AIR GUN, THE HEAD OF A BOLT SPUN AND LACERATED THE EE S LEFT HAND.

EMPLOYEE WAS STRUCK BY SLAB OF CONCRETE WHICH WAS LODGED IN FEEDER TO THE CRUSHER.

EMPLOYEE WAS CUTTING J-BOLT WHEN TORCH SLIPPED AND FLAME BURNED HIS LEFT HAND FINGERS.

WHILE SHOVELING UNDER CRUSHER JAW, TWISTED BODY AWAY FROM ROCK ABOUT 2 INCH IN DIAMETER THAT WAS FALLING FROM OVERHEAD.

ROCK FELL OFF OF PLATFORM ABOVE ROCK CRUSHER HITTING EMPL ON BACK OF HEAD. IMPACT CRACKED HARDHAT.

WHILE CHNAGING SCREENS ON CRUSHER WAS USING IMPACT GUN TO REMOVE BOLTS.WHILE HOLDING BOLT IN SLOT,SLICED FINGER.
