Miner was operating an overhead crane when the chain hook got caught on the banker rail and the chain hook slid off causing the chain and hook to fly up striking the miner in the hand.

While attempting to move a piece of curb of a conveyor with a 3/8" grout chain the hook caught on the conveyor as the piece was being hoisted and the chain became tight and started to lift the conveyor. The operator stopped and placed a hand on the chain as it was being lowered and while the chain was still under tension, it dislodged from the conveyor and hit forearm

Drilling a vertical line with a plug Drill and the bit broke and caught EE's finger between the broken bit and stone EE was drilling

Setting up a Marini drill and EE's back started to hurt towards the end of the shift. Possible back strain. Miner had drilled anchor point holes with a plug drill.

Grinding a chisel and sneezed and finger hit the spinning grinding wheel

Curb shed employee had a piece of steel sliver enter into abdominal wall.

Employee was working on the double head diamond saw. A piece of curb tipped off the banker onto the back of his leg.

Employee was reaching for something and his hand was caught in part of the machinery.

Employee was drilling granite. The bit broke and the drill moved and the remaining piece of drillbit went into his foot, just behind the steel toe cap.

A block was being lifted from the quarry. The chain tightened and a small piece of stone (corner) broke off and flew over and hit his arm.

Between 8/13/07 and 8/20/07, employee was doing a task (burning slabs) where he had to be a little "bent over" all day. Back symptoms got progressively worse. (NOTE: worked until 8/31/07 - lost time started 9/4/07).

Employee was using the steam cleaner to melt ice from the drill holes. The pipe became stuck and employee tried to free it without shutting it off.

Employee was burning curb and felt his back getting tighter the longer he worked. Felt a tightness in his back.

Employee was cutting curb when a small piece of granite flew behind his glasses into his eye.

Employee was burning the curbing and came to the end of the row. He "threw" the hose over the end and felt a pull in his back.

EE WAS USING THE BURNER ON A PIECE OF CURBING. THE REGULATOR FOR THE FUEL OIL CAME LOOSE, EE ATTEMPTED TO FIT IT, AND THE TORCH SLIPPED ACROSS HIS HAND.

EE WAS BREAKING LINES IN THE QUARRY WITH A JACKHAMMER AND WEDGES. A PIECE OF STEEL BROKE OFF OF THE WEGE AND LODGED IN HIS LEFT KNEE.

WHILE MOVING THE CRANE TO ANOTHER LOCATION, THERE WAS A MECHANICAL FAILURE. EE ELECTED TO GET OUT OF THE CRANE WHILE STILL MOVING.

EMPLOYEE WAS DRILLING, PUSHING AGAINST THE DRILL WITH HIS LEG (REPETITIVE WORK).

EE WAS PUSHING CURB ON THE CONVEYOR ROLLERS. TO STOP THE PIECE OF CURB HE PUT A SMALL PIECE OF GRANITE INTO THE ROLLER AND THE CURB THEN HIT HIS FINGER.

EE WAS RUNNING PLATFORM DRILL & USING SLEDGE HAMMER-REPETITIVE MOTION ACTIVITIES.

A PIECE OF GRANITE BROKE OFF THE BANKERS AND SCRAPED DOWN HIS LEG.

CUTTING CURB BY HAND, PIECE OF GRANITE BROKE OFFAND HIT HIM IN THE KNUCKLE.

EE SAID HIS BACK HAD BEEN "BOTHERING" HIM FOR A COUPLE OF DAYS. HE NOTICED A "POP" IN HIS BACK WHILE HE WAS DRILLING HOLES WITH THE PLUG DRILL.

EMPLOYEE WAS SETTING UP THE QUARRY DRILL. AS ONE END OF THE DRILL WAS LIFTED WITH A CRANE, THE EMPLOYEE HAD HIS FINGER BETWEEN THE DRILL AND THE CHAIN.

WHILE HE WAS HELPING TO MOVE A CRANE, HE STRAINED HIS BACK.

EE WAS USING AN OVERHEAD CRANE TO LIFT A VEHICLE. THE CHAIN SLIPPED & "PINNED" HIS ARM AGAINST THE VEHICLE.

EMP WAS PREPARING TO MOVE BURNER CABINET. DERRICK HAD LOWERED THE CABLE AND HOOK ONTO THE TOP OFTHE CABINET. WIND BLEW HOOK OFF THE TOP OF CABINET AND IT HIT EMP.

EMPLOYEE WAS BENDING OVER DRILLING WITH A PLUG DRILL FOR A LONG PERIOD OF TIME DRILLING RIFT HOLES.

EMPLOYEE WAS HOISTING HIS DRILL TO ANOTHER LOCATION WHEN THE LEG CAME LOOSE, FELL & HIT HIS FOOT.

EMPLOYEE WAS DOEN CHAINIG A BLOCK AND WALKING AWAY, THE DERRICK OPERATOR STARTED TO LFIT THE BLOCK WHENA PIECE BROKE OFF AND HIT EMPLOYEE IN THE LEG.

EMPLOYEE WAS RUNNING THE JACKHAMMER IN THE QUARRY

EMPLOYEE WAS GRINDING STEEL WITH A HAND-HELD GRINDER WHEN A SMALL PIECE BLEW-BACK INTO HIS FACE BEHIND HIS GLASSES

EMPLOYEE WAS RUNNING THE CHANNEL BAR WHEN IT "BLEW-BACK" AND A PIECE OF GRANITE (OR DUST) LODGED IN HIS EYE, OR JUST SCRATCHED.

EE WAS SHARPENING CARBIDE BITS WHEN A SMALL PIECE OF METAL BROKE OFF THE GRINDING WHEEL WENT BEHIND HIS GLASSES AND INTO HIS EYE.

EMPL PUSHING CURB ON EXIT BANKER WHEN PIECE BEHIND IT TIPPED OVER A CORNER OF IT BROKE OFF AND HIT EMPL ON FOOT.

EMPL DRILLING SMALL PIECE OF GRANITE WHEN CORNER BROKE OFF AND EMPL FELL.

EMPLOYEE WAS CUTITNG A PIECE OF METAL WITH TORCHES WHEN A PIECE OF HOT METAL BLEW-BACK INTO HIS FACE AND LODGED IN HIS EYE.

EMPLOYEE WAS SHARPENING BITS WHEN A PIECE OF STEEL BLEW-BACK INTO HIS EYE.

EMPLOYEE WAS DRILLING AND THE DRILL STEEL BROKE AND WENT INTO HIS FOOT.

EMPLOYEE HAD HIS HAND IN THE WRONG PLACE. WHILE WAITING FOR THE MAN-BOX TO REACH THE GROUND, HIS FINGER GOT BETWEEN THE MAN-BOX AND A WOODEN SPOOL IT IS SET UPON.

EMPLOYEE WAS ON3A LOWER LEDGE CUTTING A PIECE OF STONE. THE MEN ON THE UPPER LEVEL WERE DRILLING A STONE AND DRILLED INTO AN UNSEEN CRACK AND THAT STONE FELL.

EMPLOYEE WAS MAKING SIDELINES IN THE QUARRY

EMP WAS PULLING HIS BURNEWR OUT OF THE CHANNEL WHEN HE TURNED AROUND TO SEE IF THE HOSE WAS CAUGHT ON SOMETHING
