Welding all day and air arcing with auto darkening hood. Burned his right eye.

Employee was using a disc grinder and something got in his left eye. He was wearing safety glasses at the time of the accident. His eye continued to get worse and finally went to the Dr on the 7th.

Two oversized rocks were lodged in the jaws of the portable crusher. I managed a chain around one rock and had someone else pull it out. He then put his right toes in a pinch point for a step to reach the remote control. On grabbing the remote control, he absentmindedly switched the jaws on and caught his foot. His 4th and 5th toes on his right foot were fractured.

Rock chipped and piece of something went into left eye. Went to doctor and he drilled out imbedded peice of metal.

EMPLOYEE WAS USING AN AIR GUN TO LOOSEN SCREEN BOLTS. THE AIR HOSE BROKE AND HIT EMPLOYEE IN THEFACE ON THE RIGHT SIDE, CUTTING LIP WHICH REQUIRED SUTURES AND LOOSENED HIS FRONT TOOTH AND KNOC KED THE CROWN OFF HIS EYE TOOTH.

EMPLOYEE WAS DRILLING HOLES FOR TAKE UP BRACKET HE LOST HIS HOLD ON THE DRILL & IT HIT HIM ABOVE THE RT EYEBROW CUTTING HIM (REQUIRED 9 STITCHES)

HANGING WIRE V BELT PULLED HAND IN CUTTING OF F TIP OF MIDDLE FINGER

EMPLOYEE WAS UNPLUGING JAW ON TEH CRUSHER. A ROCK SLID SIDEWAYS AND HIT HIS FOOT. THE LEFT FOOT WAS CRUSHED AND BRUISED.
