HE WAS TRYING TO FIND A LEAK IN A HYDRAULIC HOSE ON THE DRILL RIG. SEVERAL HOSES; 30 TO 40 ARE ALL BUNCHED IN ONE AREA AND MAKE IT HARD TO FIND. HE PUT HIS HAND ON A HOSE AND TOLD THE OPERATOR TO GIVE IT SOME DOWN PRESSURE. THE FLUID FROM THE HOSE RIPPED OPEN THE SKIN BETWEEN HIS THUMB AND FIRST FINGER OF HIS RIGHT HAND.

HE WAS UNDERNEATH R-35 EUCLID TRUCK WITH A GRINDER. HE HAD HIS PRESCRIPTION SAFETY GLASSES ON AND A FACE SHIELD. AS HE WAS GRINDING, A FOREIGN OBJECT ENTERED HIS LEFT EYE. HE WAS ALMOST LYING DOWN WHILE HE WAS GRINDING AND IT IS FELT THAT THIS POSITION ENHANCED THE POSSIBILITY OF THE FOREIGN ENTERING HIS EYE.
NOT REALLY SURE. HE WAS WELDING IN MILL DEPT. ORGINALLY WHEN HE REPORTED HIS EYE WAS BOTHERING HIM, IT WAS TOUGHT HE GOT WELDING FLASH OR POSSIBLE DIRT IN THE EYE. IT WAS CLEANSED AND EYE D ROPS APPLIED. IT CONTINUED TO BOTHER HIM AND ON 8-4-91, HE WENT TO EMERGENCY ROOM AND THE DR. DID FIND A SMALL PIECE OF METAL IN THE EYE. MY CONCERN WAS THAT HE DID NOT LET ME KNOW SOONER.
I,NAME WAS ATTE4PTING TO DISLOADGE A ROCK WHICH WAS STUCK IN THE CRUSHER WITH THE OVERHEAD CRANE WHICH AD A HOOK ON THE END OF THE CABLE. THE HOOK SLIPPED OFF THE ROCK AND THE PULLEY CAME OVER & HIT HIS HAND3WHICH WAS ON THE RAILING AROUND THE CRUSHER.
CONE ON #5 ROCK TANK LET GO AND I.NAME WAS WORKING BELOW.GOT HIT WITH FALLING ROCK ON RIGHT FOREARM AND BACK.
