Employee was holding an idling chainsaw when the chainsaw caught the outer seam of EE's PPE chaps. The saw kicked up and hit EE's left knee causing a 2.5 inch laceration on the left knee. Immediate first aid was given, then employee went to an urgent care and received stitches.

D-6 Dozer made contact with 740 Water truck during washing activity. Water truck was parked and as dozer moved forward the blade made contact with the front right tire. The contact caused the tire to pop and drop the water truck. The event led to EE seeking medical attention for back pain.

Team Member reported eye irritation resulting in conjunctivitis from exposure to airborne foreign particulates following the use of a grinder while wearing safety glasses and face shield. Examination by ER physician did not find a foreign object or corneal abrasion in the affected eye.

Team Member felt pain in right elbow while attempting to start weed eater. This was initially reported as first aid. On November 13, 2017, ee was given prescription medication for the continuing pain in elbow making this injury reportable.

While operating D-8 dozer, moving in reverse, dozer dropped unexpectedly on uneven terrain, jolting injured worker. Injured worker felt pain in lower back.

Cummulative trauma to lower back from operating loader and running the #2 sand plant for many years.

While cutting a hole in metal plate, metal "popped", causing slag to enter clothing at open neck. Hot slag ran down chest and abdomen, causing burns.

PULLED HANDLE DOWN AND CAUGHT END OF LEFT THUMB AGAINST MATERIAL AND BRACE USED TO HOLD THE MATERIAL IN PLACE WHILE CUTTING.

CLAIMED RAN OVER LARGE ROCK BURIED IN THE DIRT PILES AND GROOVES ON 10-21-03. DID NOT REPORT INJURY UNTIL CAME ON SHIFT ON 22ND.

EMPLOYEE REPORTED TO HIS SUPERVISOR ON 10/25/02 CUMULATIVE INJURY STARTING ON 10/17. CLAIM WAS UNDER INVESTIGATION BY INSURANCE COMPANY. 6/5/03 ADVISED CLAIM WAS WORK RELATED.

EE WAS HOLDING IMPACT GUN THAT WAS STILL ATTACHED TO FRAMEWORK WHEN THE FRAMEWORK "JERKED", WRENCHING EE'S SHOULDER.

SCRAPER BOTTOMED OUT.

CUT/ PUNCTURE. SOCKET SLIPPED OFF NUT AND STRUCK HAND.

CUT/PUNCTURE/SCRA-A PIECE OF WELDING ROD IS USEDTO SECURE THE SOCKET OF THE WRENCH ONTO THE SHANK EE HAD HIS HAND ON THE SOCKET, WHEN HE STARTEDTHE WRENCH, THE ROD CUT INTO EE'S HAND

INJURED BY MOVING EE BUMPED THE AIR RAM SWITCH WHILE POSITIONING A PIECE OF METAL. THE SWITCH ACTIVATED THE RAM WHICH CLOSED A CLAMP ON THE TIP OF HIS THUMB.
