Mostrando los 18Employee reported a sore back and the injury was initially treated as a muscle strain - first aid. After the back did not improve, he went to a chiropractor for an MRI and subsequent treatments which began on 8/13/07. At this time the injury became reportable.
Single part down haul ball on crane was 2 blocked which caused the cable to be severed. Employee's right hand was struck by falling down haul ball.
After completing a water pressure test, crew was resuming drilling operations. When making a connection, some drilling foam (drill detergent) bubbled up the center tube and splashed into the helper's face. The foam sprayed under his safety glasses and irritated his right eye. Eye was washed out and prescription for eye drops received. Helper returned to work.
Loosening casing clamp & the driller turned away from the drill to explain to the new helper what was going on. He pulled the lever for the top head & ran it all the way down to clamp shell. That pulled against cable, snapped the cable that was holding clamp shell & came down. That smashed injured helper's finger between clamp shell and casing clamp.
Employee was thawing out a drain valve with a propane torch. The liquid farther up the pipe thawed out first. When the ice at the bottom of the pipe thawed and released, the fluid and ice came out of the pipe under pressure. striking employee on the right side of the face and under safety glasses. Abrasion to eye. Dr. prescribed antibiotic as a precautionary measure.
Employee was adjusting the stock while the saw was running. The saw blade grabbed his glove and caught the end of his finger (the tip of the middle finger).
EMPLOYEE WAS WELDING AND THERE WAS THREE OTHER PEOPLE WORKING BESIDE HIM AND THEY WERE GRINDING & USING AN AIR ARC CLOSE BY. SOME OF THE SPARKS FROM THE GRINDER WENT INTO HIS EYE. THIS WAS A ONE DAY ASSIGNMENT.
EE WAS USING A JACKHAMMER CLEARING A PLUGGED CHUTE IN #2 RECLAIM FEEDER AND LATER THAT DAY HIS BACK STARTED TO HURT.
ACCIDENTALLY BUMPED THE AIR ARC TRIGGER.
EMPLOYEE WAS REMOVING STARTER FROM CAT 994 LOADER. EMPLOYEE HAD SLING ATTACHED TO OVERHEAD CRANE TO SUPPORT STARTER. EMPLOYEE INADVERTENTLY ACTIVATED HOIST CAUSING THE STARTER TO BREAK FREE. STARTER STRUCK EMPLOYEE ON LEFT FOREARM CAUSING A FRACTURED ULNA BONE. EMPLOYEE PLACED ON LIGHT DUTY.
EE STATED THAT WHILE WELDING A CRACK ON A DUMP BODY, HE RECEIVED FLASH BURNS TO BOTH EYES.HE IS NOT SURE THE EXACTTIME OF DAY THIS HAPPENED.
EE WAS GETTING CABLE OFF FROM BACK OF BOOM TRUCKWHILE ANOTHER WORKER WAS LOWERING OUTRIGGERS. EELEFT FOOT WAS CAUGHT UNDER OUTRIGGER FRACTURING TWO TOES. EE PLACED ON LIGHT DUTY.
EE WAS GRINDING ON LOADER BUCKET AND TWO DAYS LATER NOTICED THAT HE HAD SOMETHING IN HIS EYE. EE WENT TO DR AND HAD SMALL METAL PORTICLE REMOVED FROM EYE. EE STATES THAT HE WAS WEARING SAFETY GLASSES AND FACE SHIELD DURING WORK ACTIVTIES.
EMPLOYEE WAS WELDING AND NOTED IRRITATION IN EYE. THE NEXT DAY EMPLOYEE OBSERVED OBJECT IN EYE. DOCTOR FOUND AND REMOVED SMALL METAL PARTICLE FROM EYE.
THE CREW WAS PREPARING TO PUT CORE CASING IN A PREVIOUSLY DRILLED HOLE. JUST AS THE FIRST JOINT OF CASING WAS BEING HOISTED INTO PLACE THE INJURED WORKER PUT HIS HAND ON THE CLAMP HOLDING THE PIPE HIS MIDDLE R FINGER BECAME STUCK BETWEEN THE CLAMP AND LIFTING HOOK. THE TIP OF THE FINGER WAS PINCHED AND SEPARATED FROM THE HAND.
THE EE WAS HANDLING THE ROD SLING WHILE THE ROD WAS BEING WINCHED UP, THE HOOK ON THE WINCH LINE DISENGAGED FROM THE ROD SLING, THE RODD DROPPED ON EE'S RIGHT BOOT.
HE WAS WASHING RODS INTO HOLE AND SMASHED RIGHT FOREFINGER AND THUMB BETWEEN THE WATER SWIVEL AND THE HEAD ON AN LF-70 CORE DRILL.
