The employee was unplugging an overflow pipe in a tank with an air lance. As he was working the lace up and down the pipe he felt a sharp pain in his lower back. Did not request to see Company Doctor until 5/6/13.
After EE completed grinding a weld on a handrail with a 4" grinder, he raised his face shield and a small debris particle came off the faceshield, went between his eyebrow and glasses and got is his left eye. He missed one day of work for a doctor's exam to check his eye. He was treated and he was prescribed eye drops. There is no permanent damage to the left eye.
THE EE WAS ATTEMPTING TO ADJUST A SLIDE DAMPER ON A FUME COLLECTION STACK AND THE HOT FUMES BELCHED OUT OF THE STACK AND HIT HIMIN THE FACE
THE INJURED WAS USING A PEDESTAL GRINDER WHEN HE SNEEZED CAUSING HIS HAND TO MOVE. AT THAT POINT HIS LITTLE FINGER ON HIS RIGHT HAND CAME IN CONTACT WITH THE GRINDING WHEEL.
THE BELT IN THE BUCKET ELEVATOR HAD BROKE AND WAS BEING HOISTED OUT THROUGH THE TOP USING THE TRUCK CRANE. IT WAS NEARING THE TOP WHEN THE BELT (WITH BUCKETS ATTACHED) CAME FREE FROM THE CRANE . AT THE SAME INSTANT, THE VICTIM LOOKED OVER THE TOP. THE "BOUNCE" FROM THE CRANE HOOK STRUCK THE VICTIMS FOREHEAD.
THE INJURED WAS3BENT OVER CHECKING A LEAKING SEAL ON THE QUENCH PUMP WHEN THE HANDLE OF AN OVERHEAD VALVE VIBRATED OFF AND HIT HIM ON THE BACK OF HIS HEAD.
