On 4/7 while tightening a lug nut the impact wrench twisted workers right arm and slipped out of their left hand. Worker didn't report injury until 4/8. Declined medical evaluation at that time. Worker continued full duty. 4/18 the worker took themselves to a specialist without company knowledge. The injury became reportable on 4/21 when they reported to work with restrictions.

Mostrando los 12The injured Driller Assistant was guiding a piece of 5-7/8" casing being lifted by the wireline to install when EE's left hand was caught between the casing EE was handling and the drill table. The middle left finger received smashing laceration and broken finger tip.
Worker was involved in a collision while operating a grader on 9/22/2022, but denied injuries and did not wish to seek medical evaluation at that time. This event became reportable on 10/12/22, when worker requested medical evaluation for neck and back pain which resulted in restricted duty assignment.

Employee attempted to hold wear iron bolt in place with finger while loosening nut with impact gun. The bolt slipped down while spinning and cut his left index finger. Report required due to medical treatment consisting of five sutures.
Employee was unloading a hammer drill bit from the pick up when it slipped pinching his Rt index finger causing a laceration requiring medical treatment.

Employee was removing a nut from a 992 loader bucket wear plate using an air impact wrench and socket. The socket came off of the wrench striking the employee's left index finger tip causing approximately a quarter inch laceration.
EMPLOYEE PINCHED THUMB IN DOOR OF BLAST HOLE DRILL CAUSING LACERATION REQUIRING SUTURES TO CLOSE WOUND. EMPLOYEE RETURNED TO NEXT SCHEDULED SHIFT ON RESTRICTED DUTY.
