Employee was operating a mucker when the bucket caught the rib and bounced him into the overhead air conditioner unit. He struck his head. He completed the shift without pain and went home. He woke up the next day with a stiff neck and was given days off work
Employee was installing new doors in the explosives storage area underground. As the door was being moved into place with a boom crane, the door hung up and the employee attempted to get it loose. It swung towards him striking the back of his hand and causing a deep laceration that required sutures.
Welder was using an air arc torch to remove liners from a truck. He finished up & removed his welding PPE. He was walking over to office to use radio when he saw the ground clamp for air arc and decided to remove it so he wouldn't forget to before he moved the truck. The clamp was still hot from cutting job resulting in Burns to the right hand palm.
Employee was kneeling while using a hand held 9" grinder . The grinder jumped to the left and caught him in the leg causing an approximately 2" x 1/4" laceration on his left leg.
A Mechanic was helping to finish the tire job on a haul truck, he was using a torque wrench on the right front wheel and felt a sharp pain in his left lower abdomen or upper groin area. He checked himself and saw a "bulge" in the lower abdomen area. The mechanic was transported to hospital for treatment.
Underground Maintenance employee recieved foreign body in eye. The employee did not seek medical assistance and the object was not removed immediately. The employee saw the site medic the next day and the employee was sent to the hospital for removal as the skin of the eye had grown over the object.
Employee was installing angle iron in an overhead location. Piece had been positioned with clamps and was to be tack-welded in place prior to removing the clamps. Approx. 20 seconds after removing the clamps the tack welds failed allowing it to fall approx. 3 feet and strike the employee on the right hand causing a crushing laceration to the back of his hand.
Employee was engaged in laying the column bar. Column bar was suspended, saw chain hanging in the way, so he reached up to flip chain out of the way causing his light on belt to hit the hydraulic lever, swing the column bar sideways and catch his right arm between the column bar and the electrical box.
He was at the controls when his shoulder popped and became sore, limiting his motion. Surgery on 7/23/04.
WORKER ATTEMPTED TO REMOVE A SPLIT SET ROCKBOLT FROM THE ROCK BOLTER JUMBO CAROUSEL. THE FRONT OF THE ROCKBOLT POPPED OUT OF THE RETAINER AND STRUCK WORKER IN THE MOUTH.
MINER WAS PREPARING TO LIFT A METAL STRAP UP TO THE BACK WHILE ROCK BOLTING. A SMALL ROCK DISLODGE FROM THE BACK & STRUCK HIS RIGHT HAND. THE FORCE OF THE BLOW CAUSED HIS FINGER TO BE LACERATE D BY A SHARP EDGE OF THE STRAP.
WORKER WAS REPOSITIONING UTILITY TRUCK JACK HANDLES. HE WAS REACHING INTO CONTROLS FROM OUTSIDE. PUSHED ON HANDLES & FRONT JACKS RETRACTED CAUSING ENGINE COVER THAT WAS PLACED ON TOP OF CANOPY TO SLIDE FORWARD & FALL STRIKING WORKER'S HAND.
A BOOM TRUCK WAS LOWERING A FAN TO THE GROUND. THE FAN TOUCHED THE GROUND AND THE LIFTING SLINGSLIPPED AND DISENGAGED FROM THE FAN CAUSING IT TO FALL TO THE SIDE AND ROLL OVER WORKER'S FEET.
VACCUUMING WITH SHOP VAC, STRAINED BACK.
WHILE ADDING A ROD, THE ROD SLIPPED, CATCHING EMPLOYEE'S FINGER BETWEEN THE ROD AND THE TOP OF THE CHUCK.
WORKER WAS USING ELECTRIC HAND DRILL TO DRILL HOLES FOR LUBE FITTINGS AND WHILE IN AN AWKWARD POSITION, DRILL BIT STUCK CAUSING DRILL TO SPIN, PINCHING HAND/FINGER.
WORKER USED TORCHES ON GALVANIZED METAL MATERIAL& DID NOT WEAR RESPIRATORY PROTECTION.
WORKER WAS ADJUSTING BLADE CLEANER ON BAND SAW. THUMB CAME IN CONTACT WITH ROTATING SAW BLADE
WHILE DRIVING A TRACTOR IN A DRIFT GOING TO WORK AREA, RAN OVER ROCK AND "FELT BACK POP BETWEEN THE SHOULDER BLADES". SUBSEQUENTLY, ON DAY OFF BREAK, AT HOME OUT OF STATE, HE VISITED CHIROPRAC TOR. THIS REPORT IS LATE BECAUSE WE WERE UNAWARECHIROPRACTIC ADJUSTMENTS CONSTITUTE MEDICAL TREATMENT.
EE WAS WELDING/GRINDING & WEARING PROPER PPE- AFTER A TIME HE NOTICED DISCOMFORT IN EYE. F.B. WAS REMOVED BY DR-SMALL PIECE OF METAL FRAGMENT
WORKER WAS USING WELDING MACHINE TO HARD FACE MINING EQUIPMENT. A SPARK ENTERED EYE OVER THE TOPOF THE WELDING HELMET. FOREIGN BODY REMOVED & TREATMENT GIVEN BY MD FOR INFECTION.
TUBE CAME UNLOCKED IN HOLE AND HIT THE STUFFING BOX AND A NIPPLE HIT HIM IN STOMACH, CAUSING RUPTURE OF LIVER AND BECAUSE OF IMPACT CAUSES HIM CHEST PAIN.
EE WAS GETTING CORE TUBE OUT OF DRILL TODS & SLIPPED. CORE TUBE CAME OUT OF HIS HAND & WENT INTOHIS FOOT.
EMPLOYEE WAS STARTING TO USE STEAM CLEANER AFTER TURNING ON MACHINE WAS NOTICED SPRAY WAS NOT RIGHT AS HE TOOK HIS HAND OFF THE TRIGGER THE RELEASE DID NOT WORK & ST ARTED TO JUMP FROM HIS HAN DS AT WHICH THAT TIMETHE STREAM OF WATER HIT HISCALF & DCAUGED A PUNCTURE
HELPER PLACED PIPE WRENCH ON CORE BARREL TO BREAK ROD JOINT DRILLER ROTATED RODS BEOFRE THE HELPER'S HAND WAS CLEAR. THE HELPER'S FINGER WAS CAUGHT BETWEEN THE WRENCH AND DRILL.
LOWERING ROD INTO FOOT CLAMP. FINGER WAS TOO LOW ON ROD WHEN FOOT CLAMP CLOSED ON FINGER. SPLITTING LEFT INDEX FINGR.
EMPLOYEE WAS PUSHING AND LEVELING BACK FILL MATERIAL WITH TO D-4 TRACK DOZER WHEN HE WAS PINCHED BETWEEN THE SEAT OF THE DOZER AND THE BACK. THE EMPLOYEE HAD ALLOWED THE FILL MATERIAL TO BUILD UP TO A POINT THAT DID NOT ADEQUATE HEADROOM. AS HE STARTED UNDER THE BROW IN 30E DRIFT, HE WAS CAUGHT BETWEEN THE BACK AND THE SEAT CAUSING CONTUSIONS OF HIS CHEST AND ABDOMEN.
GRINDING ON METAL. GRINDER KICKED OUT AND HIT EMPLOYEE IN THE TEETH. 2 TEETH WERE KNOCKED OUT.
DISCHARGE HOSE CONNECTION CAME APART SWINING THE HOSE AND HITTING EMPLOYEE ON THE RIGHT WRIST.
TRIPPING BQ W/MACHINE, AS ROD SPUN OFF IT CAUGHT ON LAST THREAD AND WAS THROWN INTO HIS SIDE.
EMPLOYEE WAS HELPING HIS PARTNER ROCK BOLT HIS WORK AREA. AS HE WAS HELDING A MAT IN PLACE A SMALL ROCK VIBRATED LOOSE AND ROLLED DOWN THE MAT AND HIT HIM IN THE UPPER LIP. THE UPPER LIP WAS C UT REGUIRING TWO STITCHES MAKING INCIDENT REPORTABLE ONLY.
DRIVING JUMBO DRILL, HIT ROCK TIPPED UP HIT HIS NECK HEAD ON FALLING OBJECT STRUCTURE
EMPLOYEE WAS BOLTING WITH JACKLEG WHEN A 60-70 LB ROCK SLID DOWN THE DRILL STEEL STRIKING HIM ON THE LEFT SHOULDER AS HE FELL THE ROCK HIT HIS RIGHT LEG AND ANKLE FRACTURING THE RIGHT LEG.BOTH EMPLOYEES SPENT CONSIDERABLE TIME SCALING THE AREA BEFORE THEY STARTED BOLTING.
EMPLOYEE WAS HELPING BREAK RODS LOOSE FROM TEH DIAMOND DRILL WHEN A ROD FELL AND STRUCK HIM ON THE BACK OF THE HEAD. EMPLOYEE SUSTAINED A CONTUSION WHICH RESULTED IN THREE DAYS LOST TIME.
CORE BARREL RESTING ON FOOT, PARTNER BROUGHT ROTATION DOWN DOWN IT CAUGHT ON CORE BARRELL DRIVING IT THROUGH TOP OF BOOTS AND FOOT BIG TOE AND NEXT TOE WERE AMPUTATED

CUT RIGHT THUMB-9 STITCHES-IMPROPER PLACEMENT-IMPROPER SCALING WHILE DRILLING A BOLT THRU A BOLTED MAT A ROCK,FIST SIZE FELL FROM THE BACK STRIKING THE BACK SIDE OF THE RIGHT THUMB,WHILE HE WA S HANGING ON TO THE AIR HOSE.IT FELL ABOUT 7 FEET FROM BEHIND THE MAT HE WAS DRILLING IN.
EMPLOYEE LOST CONTROL OF GARDNER DENVER JACKLEG WHILE DRILLING. STEEL CAME LOOSE FROM MACHINE AND STRUCK EMPLOYEE IN FOREHEAD, ABOVE RIGHT EYE. EMPLOYEE SUSTAINED A LACERATION REQUIRING 6 STIT CHES.
