EE accidently hit start button on car puller. EE's hand was caught between car puller and rope, pinching crushing injury. EE was taken to ER for treatment. EE had surgery 4/19/2024 to repair broken fingers, damaged ligaments, and amputation of tip of finger.

Maintenance Leadman using a grinder with cutting wheel to cut piece of stainless steel for a chute. Grinder kicked back and cut right hand behind thumb. Taken to ER for glue and stitches. Guard was in place on grinder, loss of control of grinder caused injury.

Impact with Mini Excavator bucket while operating, X-ray negative, bruised shoulder.

The EE lifted the top portion of the freight elevator door with his left hand and used his foot to push down on the bottom portion. The door became un-wedged quickly and resulted in his left index finger getting caught in the door mechanism. He was given Loratab and released to work.

The employee claims to have awoken one morning with a sore arm that he attributed to using a jackhammer sometime in the previous 30 days but he was not sure of the exact time or date.

WHILE STANDING ON A SALT PILE MIXING CONCRETE, EE STRAINED HIS LEFT GROIN MUSCLE.

EE WAS GRINDING W/OUT PPE AND A PIECE OF STEEL GOT INTO HIS EYE.

EE WAS GRINDING ON A PIECE OF METAL WITH A GRINDER. HE WAS WEARING SAFETY GLASSES BUT NOT GOGGLES AND A SMALL PIECE OF METAL GOT IN HIS RIGHT EYE. HE ATTEMPTED TO WASH IT OUT WITH SOLUTION, BU T STILL COULD NOT GET IT REMOVED. DR. PUT DROPS IN HIS EYE AND GAVE HIM A PRESCRIPTION. HIS EYE WAS SCRATCHED, RED, AND IRRITATED.

EMPLOYEE WAS HELPING MAINTENANCE REPAIR BROKEN AXLE ON #2 EIMCO. HE PICKED UP THE AIR IMPACT WRENCH TO TIGHTEN SPINDLE FLANGE BOLTS. WHEN HE PULLED THE TRIGGER, HE NOTICED MATERIAL FLEW OFF SO CKET. SOME OF IT FLEW INTO HIS EYE, EVEN THOUGH HE WAS WEARING SAFETY GLASSES.
