The employee was welding a wear plate inside the bed of a haul truck. After completing a weld, the employee chipped off slag and was visually inspecting the weld when a hot piece of slag rebounded and struck the employee in the eye, resulting in an eye injury that required medical evaluation

Ee was picking up a porta band saw. EE grabbed the trigger with EE's right hand and then reached to the blade with left hand. EE didn't realize the safety was not engaged. While picking it up, EE depressed the trigger and activated the saw. The saw blade caught EE's glove, pulling EE's hand into the blade. Ee sustained a cut to EE's left index finger requiring stitches to close.

Employee was cutting metal on the metal rack, after the metal was cut through the cut end came up and contacted the employee's lower lip. The lower lip was bruised, and two lower front teeth were chipped.

Tire technician was blowing debris, mud, dirt and rocks off rim base for truck 415 with compressed air and got a piece of metal in right eye. According to technician, all required PPE was worn at the time and flushing of eye was ineffective. No further treatment has been recommended since removal of metal sliver and employee was back to work.

While pressing an apron feeder track pin into place with a hydraulic track press. The employee chose to add a small 3-pound sledgehammer at the end of the pin to steady it. Once pressure was applied, the sledgehammer was ejected from its location and struck the employee in the chin fracturing one tooth.

While operating a D10T Dozer to build a new access road, the employee tracked over a topsail berm upon reaching the bottom of an embankment. As the dozer crested over the top of the berm, the tracks of the dozer contacted the ground with enough force that the seatbelt restrained operator sustained a lower back strain.

The employee was holding a bolt head from the bottom of the cutting edge so another employee could remove the nut from the bolt with the air wrench. The employee lost control of the head bolt causing it to spin lacerating EE's right index finger.

The dozer operator backed over a large rock. Employee was jarred in the cab when the dozer cleared the rock.

Pad Operator 2 was operating the backhoe pulling pipe off of the leach pad and started to notice tightness in his right upper part of back. Operator sought treatment at chiropractor.

Operator was changing loader edges and was removing the edge bolts. One of the edge bolts fell through the grove and EE held it back up with his left hand and when he used the air wrench the bolt caught his glove and hand and lacerated his thumb pad the operator required stitches.

Operator was tramming a track drill on a snow covered bench when he hit a loader tire rut approximately 8" deep. When the drill came out of the rut the front tracks went up into the air and slammed to the ground jarring the operator while he sat in his seat.

Employee was using an air impact wrench to loosen a bolt on a cutting edge. Bolt came loose sooner than expected. Torque of impact wrench made wrench rotate up striking the person on the lip. Laceration to lip required stitches.

Operator was moving drill parallel to a cutbank when the material on the crest gaveway. Drill tipped over on it's side and fell into trench. Operator was ejected out of the drill through the window and came to rest when he hit the drill mast. Operator received two broken ribs.

Employee was grinding on a piece of channel iron, with safety glasses and a welders hood on. Piece of metal flew into his left eye. Metal was removed by flushing and a prescription antibiotic was applied to prevent infection.

EE WAS IN THE WELDING BAY WHEN HIS EYE BAGAN TO HURT. EYE BEGAN WATERING AND WAS SENSITIVE TO LIGHT. OPTOMETRIST REMOVE SLIVER OF METAL EMBEDDED IN EYE.

EE WAS REPOSTIONING A LARGE HEATER BY PUSHING ON FAN GUARD. FAN GUARD BROKE ALLOWING FINGER TOSLIP THRU GUARD. FAN BLADE AMPUTATED DISTAL END OF RIGHT LITTLE FINGER

EE WAS OPERATING D-9 DOZER PUSHING MATERIAL TO ALOADER DOZER RAN OVER A LARGE ROCK CAUSING INJURY TO EE'S NECK WHEN IT CAME DOWN ON OTHER SIDE OF ROCK. INJURY OCCURED WITH NO TREATMENT NECESSA RY. INJURY PROGRESSED TO THE POINT WHERE SURGERYWAS REQUIRED TO REPAIR A RUPTTURED DISC IN HIS NECK

EMPLOYEE STATES WHILE PREPARING TO PULL RODS, HE GRABBED THE WRONG LEVER AND WAS STRUCK IN THE LEFT SIDE BY THE OVERSHOT. SUSTAINED FRACTURE OF TWO RIBS.

EMPLOYEE WAS USING A METAL BANDING MACHINE TO PUT A METAL STRAP ON A HOSE. THE STRAP SLIPPED AND CUT HIS MIDDLE FINGER REQUIRIGN STITCHES.

EMPLOYEE WAS ATTEMPTING TO REMOVE A CUUTTING EDGE FROM A CAT 992 LAODER USING AN AIR WRENCH. WHEN THE BOLT STARTED SPINNING. HE GRABBED IT AND CT HIS HAND, REQUIRED STITCHES.

EE WAS LAYING ON HIS BACK GRINDING UNDERNEATH CAST RUBBER TIRE DOZER MODEL 834. A FORIEGN BODY ENTERED HIS LEFT EYE ENDERNEATH HIS PERSONAL PORTECTIVE EQUIPMENT. ON 1/7/97 EE WENT TO DOCTOR TO HAVE THE FOREIGN BODY REMOVED.

EE WAS ASSISTING CHANGING CUTTING EDGES USING AN AIR WRENCH. EE WAS HOLDING A BOLT WHEN IT STARTED TO SPIN, IT CUT HIS HAND WHICH REQUIRED STITCHES.

TAKING FRONT TIRE OFF OF HAUL TRUCK. LOOSENED LAST BOLT WHEN TIRE KICKED OUT PINCHING HAND BETWEEN IMPACT GUN AND THE HAND.

EMPLOYEE WAAS DOZING ORE ON A HEAP LEACH PAD WHE HIS EYE STARTED BOTHERING HIM. FOREIGN BODY FOUND IN LEFT EYE.

EMPL WAS BY THE OUTRIGGER HOLDING A LOCKNUT WHILE ANOTHER EMPL ADJUSTED THE PRESSURE & A CONTRACTOR OPERATED THE OUTRIGGER UP & DOWN. THE CONTRACTOR LOWERED THE OUTRIGGER ALL THE WAY DOWN & IT CAUGHT EMPLS FOOT BETWEEN THE OUTRIGGER & GROUNDCAUSING BRUISING TO THE TOES.

EMPLOYEE WAS WELDING OUT DOORS ON A WINDY DAY WHEN A SMALL SPARK BLEW INTO HIS RIGHT EAR. EMPLOYEE WENT TO THE DOCTOR ON MAY 12 AND FOUND OUT HIS RIGHT EAR WAS INFECTED.

EMPLOYEE WAS AIR ARCING IN SCREEN DECK HE RAISED HIS SHIELD TO REPOSITION HIMSELF THE AIR ARC CONTACTED METAL CAUSING A FLASH WHICH BURNED HIS EYES

EMPLOYEE WAS STARTING TO COLLAR A DRILL HOLE. WHEN HE TURNED ON THE AIR DUST AND DIRT WAS BLOWN UP TOWARDS HIS FACE AND UNDER HIS SAFETY GLASSES. HIS LEFT EYE WAS IRRITATED AND REQUIRED PRESCR IPTION MEDICATION.

LOWER BACK PAIN

PULLING HX TUBE WITH CORE TUBE WHEN OVERSHOT RELEASED ATTEMPTED TO CATCH IT IT LATCHING HEAD CAUGHT L INDEX FINGER AND R THUMB, SEVERING L INDEX FINGER AND CRUSHING R THUMB

EMPLOYEE WAS ASSISTING IN CHANGING THE CUTTING EDGES ON A CATEPILLAR 992 LOADER,EMPLOYEE WAS HOLDING THE HEAD OF A BOLT WITH HIS RIGHT KNEE AND USING AN AIR WRENCH TO LOSEN THE NUT.EMPLOYEE EX ERTED PRESSURE ON NUT CAUSING BOLT TO SLIP OUT OF PLACE AND SPIN AGAINST EMPLOYEES KNEE CUTTING THE SKIN REQUIRING STITCHES.

EMPLOYEE WAS AIR ARCING INSIDE A CRUSHER LINER CHUTE REFLECTION ON SHINY PART OF CHUTE REFLECTED IN REAR OF HOOD LATER IN THE EVENING HIS EYES STARTED TO HURT

CRUSHING AND SPLITTING SAMPLES. ROCK PARTICLE EMBEDDED IN RIGHT EYE.

SETTING A 55 GALLON DRUM IN BACK OF VEHICLE USING AN AUTOCRANE. BARREL SLIPPED OUT OF STRAP AND FELL APPROXIMATELY 12 INCHES. CAUSED A FRACTURE OF THE FIBULA AND TIBULA ON THE LEFT LEG.

WELDING UNDERCARRIAGE OF DRILL, HOT SLAG DEPOSIT DROPPED OUT AND LANDED INSIDE WELDING LEATHERS CAUSING SECOND DEGREE BURN APPROX.2 CM FRONM THE CHEST.

INJURED PARTY WAS VENTING STEAM FROM BOILER WHEN A PIPE TWISTED & SCALDED HIM WITH STEAM & HOT WATER

CLAMP BROKE AND3THE HOSE CAME LOOSE, HITTING THE DRILLE IN THE SHOULDER AND BACK OF NECK, KNOCKING DRILLER APPROX 15' FROM DRILLER

EMPLOYEE WAS CU7TING STEEL SIDING WITHA SKIL SAW WHEN A METAL PARTICLE FLEW INTO HIS EYE. SAFETY GLASSES WERE BEING WORN.

EMPLOYEE WAS OPERATING A CAT 245 SHOVEL DIGGING ROCK FROM THE PIT. HE WAS PRYING A ROCK LOOSE WITH THE SHOVEL BUCKET WHEN THE ROCK CAME LOOSE THE SHOVEL JOLTED FORWARD STRAINING THE OPERATORS BACK.

WELDING ON THE WOBBLER BARS AT THE CRUSHER, TOUCHED STINGER TO STEEL BEFORE FLIPPING THE VISOR DOWN ON THE WELDING HOOD. FLASH BURN ON LEFT EYE.

RECEIVED FLASHBURNS TO EYES FROM WELDING MASK WITHOUT GASKET INSTALLED IN LENS HOLDER OF MASKA DN FROM DIRECT ARC CAUSED WHEN CHANGING WELDING CABLE.
