Ee was picking up a porta band saw. EE grabbed the trigger with EE's right hand and then reached to the blade with left hand. EE didn't realize the safety was not engaged. While picking it up, EE depressed the trigger and activated the saw. The saw blade caught EE's glove, pulling EE's hand into the blade. Ee sustained a cut to EE's left index finger requiring stitches to close.

Mostrando los 15Employee was cutting metal on the metal rack, after the metal was cut through the cut end came up and contacted the employee's lower lip. The lower lip was bruised, and two lower front teeth were chipped.
Tire technician was blowing debris, mud, dirt and rocks off rim base for truck 415 with compressed air and got a piece of metal in right eye. According to technician, all required PPE was worn at the time and flushing of eye was ineffective. No further treatment has been recommended since removal of metal sliver and employee was back to work.
While pressing an apron feeder track pin into place with a hydraulic track press. The employee chose to add a small 3-pound sledgehammer at the end of the pin to steady it. Once pressure was applied, the sledgehammer was ejected from its location and struck the employee in the chin fracturing one tooth.
While operating a D10T Dozer to build a new access road, the employee tracked over a topsail berm upon reaching the bottom of an embankment. As the dozer crested over the top of the berm, the tracks of the dozer contacted the ground with enough force that the seatbelt restrained operator sustained a lower back strain.
The employee was holding a bolt head from the bottom of the cutting edge so another employee could remove the nut from the bolt with the air wrench. The employee lost control of the head bolt causing it to spin lacerating EE's right index finger.
The dozer operator backed over a large rock. Employee was jarred in the cab when the dozer cleared the rock.
Pad Operator 2 was operating the backhoe pulling pipe off of the leach pad and started to notice tightness in his right upper part of back. Operator sought treatment at chiropractor.
Operator was changing loader edges and was removing the edge bolts. One of the edge bolts fell through the grove and EE held it back up with his left hand and when he used the air wrench the bolt caught his glove and hand and lacerated his thumb pad the operator required stitches.
Operator was tramming a track drill on a snow covered bench when he hit a loader tire rut approximately 8" deep. When the drill came out of the rut the front tracks went up into the air and slammed to the ground jarring the operator while he sat in his seat.
Employee was using an air impact wrench to loosen a bolt on a cutting edge. Bolt came loose sooner than expected. Torque of impact wrench made wrench rotate up striking the person on the lip. Laceration to lip required stitches.
Operator was moving drill parallel to a cutbank when the material on the crest gaveway. Drill tipped over on it's side and fell into trench. Operator was ejected out of the drill through the window and came to rest when he hit the drill mast. Operator received two broken ribs.
Employee was grinding on a piece of channel iron, with safety glasses and a welders hood on. Piece of metal flew into his left eye. Metal was removed by flushing and a prescription antibiotic was applied to prevent infection.
EE WAS IN THE WELDING BAY WHEN HIS EYE BAGAN TO HURT. EYE BEGAN WATERING AND WAS SENSITIVE TO LIGHT. OPTOMETRIST REMOVE SLIVER OF METAL EMBEDDED IN EYE.
EE WAS REPOSTIONING A LARGE HEATER BY PUSHING ON FAN GUARD. FAN GUARD BROKE ALLOWING FINGER TOSLIP THRU GUARD. FAN BLADE AMPUTATED DISTAL END OF RIGHT LITTLE FINGER
