Worker was welding in shop. They removed their welding hood after completing a weld and the slag popped and sent material into their eye. The material had to be removed by medical professionals and they were prescribed antibiotic and steroidal eye drops.

2 employees were working on a palletizer, but only one of them had it locked out. When the locked out employee was done, they asked the other employee if they were clear, and heard "yes". The other employee was still working, and when the machine was started, their right hand was caught, amputating the index finger, and degloving the middle finger.

employee was sealing a product bag with the a heat seal when their right finger tip made contact with the heated surface, burning their right finger tip.

EE was using the SteelWeld Shear to cut metal pieces to make a machine guard. When EE activated the foot pedal on the machine, EE's hand was in position under the clamp to hold the metal. EE's right middle finger was caught between the hydraulic piston that holds the metal being cut on the SteelWeld Shear work table.

The bulldozer then slid off of the rock, and shift forward suddenly, hitting the scraper. This caused excessive force and resulted in a whiplash type of injury to both operators.

Employee in 631E scraper was hauling crude from mining pit to crude pile in rotation with other scrapers, which were being pushed with a D-9N dozer. Accident occurred when employee had turned in seat to see where dozer was, when D-9N dozer made contact with the 631E scraper, causing neck strain injury to employee.

EE was changing out the cutting blade on the Shipping film applicator. When making final adjustments, EE noticed some film was stuck on the edge of the blade. EE tried to pull the film off the blade when his left index finger came in contact with the blade.

Workers were repairing a broken water line. They were saw cutting a water main. The employee was pouring water from the bucket to irrigate the saw blade, when the saw got stuck and kicked back at his face.

The employee stated that he was welding in an awkward position for a long period of time.

Employee was placing and welding patches on 601 cyclone and strained his back. Employee did not report this injury to his Supervisor till 05/18/06. He is claiming the strain was from an old injury sustained in 1986.

EE was using a jack hammer to break some concrete when his left wrist became sore. The doctor placed EE on restricted work activities.

EE was packing 50 lb bags at parking station a bag blew up, causing dirt to get in his right eye. EE was wearing safety glasses at the time of the accident.

Celite feels this is not a valid injury, employee not working, Supervisor stated injury occurred at home. Claim denied by L&I. New claim using injury date 11-30-04 accepted 1-28-05 by L&I. Acc rpt filled out 1-18-05 stated he pulled pallet from stack set on machinery for supersack bags felt pulling back of leg, pulled another pallet from stack felt stinging lower back.

Injured and another employee were removing an air line from a compressor. The air line was under pressure when the fittings were released. The pressurized hose struck him in the left hand. His work was temporarily modified to restrict use of the left hand.

WHEN HE TURNED BACK AROUND THE PRODUCT WAS FALLING RAPIDLY IN BAG. EE GRABBED BAG TO CENTER ON PALLET BUT BAG WAS TOO HEAVY TO MOVE, STRAINING HIS LEFT SHOULDER.

EE WAS BURNED ON THE LEFT WRIST FROM SLAG CREATED BY THE USE OF A CUTTING TORCH TO BURN OFF A BEARING AT #3 SECONDARY BLOWER. THE EVENT OCCURED ON 3/9/04 AND BECAME REPORTABLE 3/15/04

EE WAS PACKING 50 LB BAGS. HE WAS CLOSE TO EMPTYING THE BIN WHEN A BAG BLEW UP ON #3 PACKER. HE WAS PATTING THE BAG AT THE TIME. WHEN IT BLEW UP IT THREW PRODUCT INTO HIS LEFT EYE. EE WAS WEARING SAFETY GLASSES AT THE TIME. HE WAS TAKEN TO E.R. WHERE THEY IRRIGATED EYE AND RELEASED HIM.

EE WAS OPERATING SCRAPER, STRIPPING BASALT IN WEST PIT, OTHER EE WAS OPERATING PUSH CAT. AS EE IN SCRAPER WAS DROPPING BOWL, OTHER EE BUMPED INTO HIM HARD FROM BEHIND, CAUSING SCRAPER TO GO OVER A LARGE PIECE OF BASALT, JARRING EE A SECOND TIME, STRAINING HIS NECK AND BRUISING HIS RIBS AND HIPS.

EE WAS PREPARING TO BEND A PIECE OF 1/4" FLAT STOCK METAL IN THE PRESS BRAKE. HIS RIGHT FOOT CONTACTED THE START SWITCH/DEADMAN. THE PRESS LOWERED CONTACING HIS LEFT HAND.

EMPLOYEE WAS WALKING DOWN A FLIGHT OF STAIRS. A TRAP DOOR DESIGNED TO COVER THE STAIRWELL JARRED LOOSE. THE DOOR UNWOUND ON A WINCH AND STRUCK HIM ON THE TOP OF THE HEAD.

EE WAS REMOVING BLOWER ROTOR WHICH WEIGHS ABOUT 800 LB. THE ROTOR WAS SUSPENDED ON AN OVERHEAD HOIST. EE PUSHED THE ROTOR AWAY FROM PIPE & HOUSING. HIS LEFT FINGER WAS CAUGHT BETWEEN.

EE WAS PACKING 50# BAGS OF PRODUCT. HE DROPPED A BAG ONTO THE CONVEYOR. THE BAG WAS OFF CENTER. HE TRIED TO STRAIGHTEN THE BAG WITH HIS FOOT. HIS FOOT AND LEG WERE CAUGHT IN THE BAG PRESS. HE PULLED THE EMERGENCY STOP CORD TO THE CONVEYOR.

EMPLOYEE HAD WELDED A GUARD IN PLACE ON 09/01/99. ON 09/02/99 AT ABOUT 3:00AM, EMPLOYEE AWOKE WITH A BURNING SENSATION IN HIS RIGHT EYE.

EMPLOYEE WAS CLEARING A BAG JAM AT PALLETIZER. SLIPPED AND HER LEFT LEG WAS CAUGHT IN AN OPENING CREATED BY A MISSING BAG ROLLER. SHE REPORTED THE ACCIDENT ON 3-4-99.

EE WAS USING A WIRE FEEDER OUTFITTED WITH ./075 HARDWIRE. HIS RIHGHT ARM SLIPPED AND HE CONTACTED HIS LEFT WRIST WITH THE WIRE.

EE TOOK OVER BAG PACKING DUTIES FROM ANOTHER EE AS HE STEPPED IN FRONT OF A 50LB BAG BEING FILLED ON THE SPOUT TO GET TO THE CONTROLS, A SEAM BURST ON THE BAG SPRAYING HIM WITH POWDER PRODUC T. THE STREAM KNOCKED HIS SAFETY GLASSES & RESPIRATOR OFF GETTING PRODUCT INTO HIS EYES.

EMPLOYEE WAS OPERATING SCRAPER ON THE EDGE OF AN OVERSURDENN PILE, DUMPING MATERIAL. HE HAD DUMPED ROCK MATERIAL THERE PREVIOUSLY. HIS BOWL WAS TOO HIGH AND HE WAS TRAVELING TOO FAST TO CONTR OL THIS MACHINE. A TIRE SLIPPED OVER THE EDGE STEERED DOWN THE PPILE TO CORRECT BUT HIS BOWL KEPT LIDING SIDEWAYS AND WOULD NOT STRAIGHTEN OUT. THE MACHINE SLOWLY ROLLED AT THE BOTTOM STRAI

EMPLOYEE WS SPLATTERED WITH FINE MATERIAL PRODUCT WHEN 50LBS. BAG HE WAS LOADING POPPED. THE MATERIAL BLEW UP BEHIND HIS SAFETY GLASSES AND INTO HIS EYES. HE FLUSHED HIS EYES OUT AND WENT TO T HE HOSPITAL AND TO HIS DOCTOR.HE RECIEVED MINER SCRATHES ON HIS EYES AND WAS ABLE TO RETURN TO WORK ON HIS NEXT SCHEDULED DAY.

EE WAS OPERATING TEREX SCRAPER IN MINE (SECTION 7), WHEN THE D10 PUSH CAT STOPPED. THE TWO PIECES OF EQUIPMENT. THIS SCRAPER PULLED AHEAD ABOUT 20 FT THEN HE STOPPED ALSO. THE CAT OPERATOR THE N PROCEEDED AHEAD TO RE-ENGAGE WITH THE SCRAPER AND FINISH THE STRIP. THE CAT OPERATOR MISJUDGED SPEED, DISTANCE AND BLADE HEIGHT, REAR ENDING THE SCRAPER.

EE WAS CLEARING BRUSH WHILE A COWORKER WAS CUTTING WITH A CHAINSAW. THE CHAINSAW BECAME STUCK IN A CUT. WHEN COWORKER PULLED THE CHAINSAW BACK, HE STRUCK EE, CUTTING EE'S RIGHT LITTLE FINGER.

EE WAS USING A VACUUM SYSTEM TO CLEAN UNDER AND STRUCTURE. WHEN HE STOOD UP FROM A CROUCHED POSITION HE FELT A SHARP PAIN IN HIS BACK. THIS CASE BECAME REPORTABLE ON 6-13-97, WHEN THE DR RESTR ICTED EE FROM WORK ACTIVITIES.

EE WAS RE-INSTALLING A CONTER WEIGHT ON A LIFT TRUCK. THE WEIGHT WASS SUPPORTED BY AN OVERHEAD HOIST. OPERATED BY ANOTHER EE. EE WAS GUICING THE COUNTER WEIGHT INBTO POSITION WHEN HIS LEFT LIT TLE FINGER WAS CAUGHT BETWEEN THE WEIGHT & TRUCK

E AND TWO OTHE REPLOYEES WERE TESTING A NW OPERATION WATER AND STEAM WERE PLACED IN THE HAVEC TANK. TO AGITATE THE WATER, AIR WW INTRODUCED INTO THE TANK. THIS CAUSED THE WATER TO EXPAND AND BOIL OVER, CONTACTING EE.

DISCHARGE END OF KILN WAS PLUGGED. EMP WAS CLEANING IT OUT WHEN KILN FLUSHED, COVERING LEFT FOOTWITH HOT PRODUCT, BURNING FOOT.

HE PLACED AN EMPTY BAG ONTO A PACKER, HIT THE START (FILL) BUTTON AND MOVED TO HIS LEFT. THE BAG RUPTURED CAUSING DUST PARTICLES TO ENTER HIS RIGHT EYE. THIS CASE BECAME REPORTABLE ON 4-22-96 WHEN THE DOCTOR PRESCRIBED MEDICINE.

THIS ACCIDENT OCCURRED DURING PUSH LOADING SCRAPERS.INJURED WAS OPERATING THE SCRAPER HE CAME INTO THE WORKING CUT AND LOST TRACTION ON LOOSE MATERIAL,BRINGING THE VEHICLE TO A STOP.DUE TO DUS TY CONDITIONS THE PUSH CAT OPERATOR COULD NOT TELL THAT THE SCRAPER HAD STOPPED MOVING.THE PUSH CAT STRUCK THE SCRAPER HARDER THAN USUAL CAUSING INJURED TO JOLT FORWARD WHICH JOLTED HIS NECK.

EE WAS FILLING BULK BAGS WITH PRODUCT. THE BULK BAG HOLDING ARMS STUCK IN THE OPEN POSITION, EE STEPPED ONTO THE ROLLER BED OF THE FILLING EQUIPMENT TO ACTIVATE THE HOLDING ARM RELAY, LOST HIS FOOTING AND FELL BACK HITTING HIS LOWER BACK.

INJURED VACUUMING MATERIAL USING A 10' ALUMINUM WAND TO REACH OVERHEAD WHEN HE FELT PAIN IN HI SUPPER BACK. THIS CASE BECAME REPORTABLE WHEN HE MISSED WORK ON 8-25-94.

EE WAS BENDING METAL WITH THE POWER BRAKE. THE MACHINE HAD NOT BEEN SET UP PROPERTLY BENDING THE METAL PIECE PAST 90 DEGREE SMASHING EE LEFT THUMB BETWEEN THE METAL AND THE MACHINE FRAME.

INJURED WAS BLOWING DOWN A BIN ARCH WITH AN AIR LANCE WHEN SHE FELT PAIN IN HER UPPER BACK.
