Worker was welding in shop. They removed their welding hood after completing a weld and the slag popped and sent material into their eye. The material had to be removed by medical professionals and they were prescribed antibiotic and steroidal eye drops.

2 employees were working on a palletizer, but only one of them had it locked out. When the locked out employee was done, they asked the other employee if they were clear, and heard "yes". The other employee was still working, and when the machine was started, their right hand was caught, amputating the index finger, and degloving the middle finger.

EE was using the SteelWeld Shear to cut metal pieces to make a machine guard. When EE activated the foot pedal on the machine, EE's hand was in position under the clamp to hold the metal. EE's right middle finger was caught between the hydraulic piston that holds the metal being cut on the SteelWeld Shear work table.

He was using a sheet of paper held over the valve seat while the valve plunger was open. When the valve was closed to test the seat, it caught his left index finger causing a laceration and fracture at the tip of the finger.

Employee in 631E scraper was hauling crude from mining pit to crude pile in rotation with other scrapers, which were being pushed with a D-9N dozer. Accident occurred when employee had turned in seat to see where dozer was, when D-9N dozer made contact with the 631E scraper, causing neck strain injury to employee.

EE was changing out the cutting blade on the Shipping film applicator. When making final adjustments, EE noticed some film was stuck on the edge of the blade. EE tried to pull the film off the blade when his left index finger came in contact with the blade.

Workers were repairing a broken water line. They were saw cutting a water main. The employee was pouring water from the bucket to irrigate the saw blade, when the saw got stuck and kicked back at his face.

The employee stated that he was welding in an awkward position for a long period of time.

EE was using a jack hammer to break some concrete when his left wrist became sore. The doctor placed EE on restricted work activities.

Celite feels this is not a valid injury, employee not working, Supervisor stated injury occurred at home. Claim denied by L&I. New claim using injury date 11-30-04 accepted 1-28-05 by L&I. Acc rpt filled out 1-18-05 stated he pulled pallet from stack set on machinery for supersack bags felt pulling back of leg, pulled another pallet from stack felt stinging lower back.

Injured and another employee were removing an air line from a compressor. The air line was under pressure when the fittings were released. The pressurized hose struck him in the left hand. His work was temporarily modified to restrict use of the left hand.

WHEN HE TURNED BACK AROUND THE PRODUCT WAS FALLING RAPIDLY IN BAG. EE GRABBED BAG TO CENTER ON PALLET BUT BAG WAS TOO HEAVY TO MOVE, STRAINING HIS LEFT SHOULDER.

EE WAS BURNED ON THE LEFT WRIST FROM SLAG CREATED BY THE USE OF A CUTTING TORCH TO BURN OFF A BEARING AT #3 SECONDARY BLOWER. THE EVENT OCCURED ON 3/9/04 AND BECAME REPORTABLE 3/15/04

EE WAS PACKING 50 LB BAGS. HE WAS CLOSE TO EMPTYING THE BIN WHEN A BAG BLEW UP ON #3 PACKER. HE WAS PATTING THE BAG AT THE TIME. WHEN IT BLEW UP IT THREW PRODUCT INTO HIS LEFT EYE. EE WAS WEARING SAFETY GLASSES AT THE TIME. HE WAS TAKEN TO E.R. WHERE THEY IRRIGATED EYE AND RELEASED HIM.

EE WAS OPERATING SCRAPER, STRIPPING BASALT IN WEST PIT, OTHER EE WAS OPERATING PUSH CAT. AS EE IN SCRAPER WAS DROPPING BOWL, OTHER EE BUMPED INTO HIM HARD FROM BEHIND, CAUSING SCRAPER TO GO OVER A LARGE PIECE OF BASALT, JARRING EE A SECOND TIME, STRAINING HIS NECK AND BRUISING HIS RIBS AND HIPS.

EE WAS PREPARING TO BEND A PIECE OF 1/4" FLAT STOCK METAL IN THE PRESS BRAKE. HIS RIGHT FOOT CONTACTED THE START SWITCH/DEADMAN. THE PRESS LOWERED CONTACING HIS LEFT HAND.

EMPLOYEE WAS WALKING DOWN A FLIGHT OF STAIRS. A TRAP DOOR DESIGNED TO COVER THE STAIRWELL JARRED LOOSE. THE DOOR UNWOUND ON A WINCH AND STRUCK HIM ON THE TOP OF THE HEAD.

EE WAS REMOVING BLOWER ROTOR WHICH WEIGHS ABOUT 800 LB. THE ROTOR WAS SUSPENDED ON AN OVERHEAD HOIST. EE PUSHED THE ROTOR AWAY FROM PIPE & HOUSING. HIS LEFT FINGER WAS CAUGHT BETWEEN.

EE WAS PACKING 50# BAGS OF PRODUCT. HE DROPPED A BAG ONTO THE CONVEYOR. THE BAG WAS OFF CENTER. HE TRIED TO STRAIGHTEN THE BAG WITH HIS FOOT. HIS FOOT AND LEG WERE CAUGHT IN THE BAG PRESS. HE PULLED THE EMERGENCY STOP CORD TO THE CONVEYOR.
