EE was attempting to nail two 2x4 boards together in a right angle. EE had a smaller 2x4 standing on end with the longer 2x4 on top. Was holding them together with left hand. Brought the automatic nailer up and then back down on the top of the board. EE missed the center. The nail grazed the edge of the board, deflected and penetrated EE's hand. A doctor removed it.

Mostrando los 8MSHA inspector had been in plant and had EE wearing noise and dust sample collecting equipment for the day. The hoses from these devices got tangled up and he was trying to straighten them out. He straightened his arm out and hand went into radial saw. Fatty tissue between thumb and 1st finger was cut, also his thumb. Wearing 2 devices caused the entanglement.
EE was welding a 12"X1"X3/4" piece of metal onto a mill gang cart. He was holding the piece in his R hand and welding with his L hand. He stated he heard it pop and then the welding wire bounced up and penetrated his welding glove and R thumb. He applied 1st aid and continued to work. On 4/3/11 it was swollen/sore, went to ER, did 1st aid. On 4/4/11 got worse so he was admitted.
The radial arm saw has 12' rollers on each side of it. Employee put a 10' 2"x4" on the rollers on the left side of the saw. With his left hand he grabbed the 2"x4" from the end closest to the running saw and tried to pull it into the saw. His hand slipped off and hit the blade on the running saw. He received multiple lacerations to two fingers on his left hand.
EMPLOYEE WAS USING CUTTING TORCHES TO CUT A 48" X 16" X 3/4" PIECE OF METAL OUT SO THAT IT COULD BE REPLACED. HE DID THIS FOR MOST OF THE DAY. AT THE END OF HIS SHIFT HIS EYES HURT & HE COULD NOT SEE WELL. HE WAS WEARING A FACE SHIELD WITH #12 LENS ALONG WITH TINTED SAFETY GLASSES.
EE had just finished drilling a 34" vertical hole with a track drill. He was removing the second circular drill stock when he lost control of it. As it fell he grabbed it with his right arm but the momentum of the drill stock falling caused his elbow to jerk.
EE JUST FINISHED DRILLING HOLES WITH A TRACK DRILL AND WAS CREASING THE HOLES TO SCORE BOTH SIDES. HE RAISED THE DRILL OUT AND GRABBED IT WITH HIS LEFT HAND. HIS GLOVES WERE LONGER THAN HIS FI NGERS SO WHEN HE GRABBED THE CREASER THE GLOVE WAS BETWEEN THE CREASER AND THE PALM OF HIS HAND. WHEN THE CREASER ROTATED, IT TIGHTENED THE GLOVE WITH HIS HAND IN IT.
EMPLOYEE WAS TAKING STONE OFF OF A SPLIT MACHINE AND PUTTING ON A PALLET. AFTER ONE PIECE WAS SPLIT HE REACHED OVER THE PIECE OF STONE AND INTO THE MACHINE TO GRAB BACK OF STONE INSTEAD OF FRO NT. AS HE DID SO HIS FINGER GOT PINCHED BETWEEN THE MACHINE AND THE STONE.
