Employee was cleaning sand buildup under the scalper and caught his shovel on a roller. The jerking caused a pinched nerve to his right shoulder. Two visits of physical therapy have been ordered. Restricted duty until 8/30/05.

EMPLOYEE WAS OPERATING A UNILOADER AROUND THE PLANT. THE HYDRAULIC HOSE BROKE AND SOME HYDRAULIC FLUID DRIPPED INTO HIS EYES AROUND HIS SAFETY GLASSES. THE CLINIC IRRIGATED HIS EYE.
EMPLOYEE WAS WELDING ON A CHUTE AND A PIECE OF SLAG BURNED HIS STOMACH.
