Miner was using pneumatic jackhammer (rivet buster) on a sidewall performing refractory demolition when the bit broke, causing the tool to slip and impact EE's left hand against the wall. Miner has laceration and fracture on left index finger.

Employee was operating a Brokk machine in the kiln when a capped brick came loose from overhead and struck EE in the right shoulder. Employee had a right shoulder strain from the impact.

The driller was tramming the drill off of the lowboy trailer, the driller had the door of the drill locked open in order to see the edge of the trailer. When the driller reached the point where it tipped forward onto the ramp, the driller reached out and grabbed the door frame, the jolt released the door lock lever, slamming the door onto the driller's hand.

Right hand pointer finger smashed

Associate performing demo work with a jackhammer upward, a large piece of cement broke off and came down and smashed finger, the pressure created a gash.

Using welding equipment on grizzly bars and working in an uncomfortable position for an extended period of time.

Operating back hoe on uneven ground. (This is under investigation due to previous underlying degenerative discs)

He was jackhammering a concrete block. He had one leg behind the other to get good leverage on the jackhammer. When he stopped and stepped back and stood up straight, his hip felt like it got stuck and felt a sharp pain in his right hip and could not walk.

Employee was loosening a bolt with an impact wrench and holding the nut with his fingers when the nut spun around and cut through his glove and slicing his right index finger.

RED IRRITATED RIGHT EYE

Ee was standing on the burner floor, air lancing a clinker pile build up, when hot material landed on his neck. The ee immediately reported the incident to his supervisor, but did not require medical attention at that time. The wound became infected, so medical treatment began on 7/20/2009. The injury became reportable on 9/30/2009 when received surgery.

Effected employee was working in the clinker cooler to assess damage/required repairs, when hot material fell from above on his head, neck, arms and back. EE was assisted out by a fellow workers and taken to the shower to rinse off. Ambulance transport to hospital ER per company policy. All LOTO procedures on equipment had been followed.

The employee was blowing concrete debris out of a hole with pressured air, when something went into the eye.

Employee was grinding on a flange when something flew into her eye.

Employee was walking through the mill area where co-workers were grinding on fiberglass gaskets. The wind picked up and blew a strand on fiberglass into the employee's eye.

Grinding metal when something got into the eye.

Pinched right index finger in between press arm and cap of pellet press.

Employee was walking into an area where three hand grinders were being used. A piece of grinding media flew between his safety glasses and his face and lodged in his eye, scratching it.

Build up from the preheater dropped onto the feed shelf, which caused hot kiln dust to spill from the feed end air seal. This spillage then fell 26 feet to the ground floor.

INJURED EE WAS REPLACING A HUB LINER IN THE NO.1RAW MILL SEPARATOR, USING A 1/2" ELECTRIC IMPACTWRENCH. WHEN THE WRENCH CORD BECAME HUNG UP THE EE PULLED UP ON THE CORD IN AN ATTEMPT TO FREE T HE CORD. THE CORD CAME FREE AND THE PLUG AT THE END OF THE CORD STRUCK THE EE IN THE TOOTH.

EMPLOYEE WAS GRINDING OFF WELD SLAG WHEN A PIECE ENTERED HIS EYE, EVEN THOUGH HE WAS WEARING A FACE SHIELD.

EMPLOYEE WAS CUUTTING "T" BOLT INT EH CLINKER COOLER WINDBOX 3HLE IN A KNEELING POSITION A HOT PIECE OF SLAG FELL INTO HIS BOOT AND BURNED HIS HEEL. HE DID NOT GO TO THE DR UNTL 1-27 WHEN HE FINALLY REALIZED THAT THE BURN WAS NOT HEALING PROPERLY.

EE WAYS HE HAD JUST FINISHED USING THE MORTAR MIXER & THREW TWO EMPTY SACKS IN TO CLEAN IT. HE HAD RAISED THE GUARD OVER THE PADDLES TO DO SO & THE SECOND SACK CAUGHT & JERKED HIS HAND INTO TH E PATH OF THE PADDLES & THE MIXING BARREL.

EMPLOYEE WAS ATTEMPTING TO LEVELTHE DM-45E DRILL FOR SETUP AT A HOLE & WAS ON A SLOPE WHICH CAUSED THE DRILL TO OVERTURN WHEN THE LEVELING JACKS TOUCHED THE GROUND
