While replacing burned out lighting, 2 employees used the drill mast as a means of working from heights. The drill was used in the similitude of a fireman's ladder to raise and lower the employee to reach the lighting mentioned above. This resulted in the employee's foot being caught in between the drill mast and the pedestal resulting in a crushing injury of the left foot.

Mostrando los 25A driller's assistant was attempting to load an empty core tube into the rod string when the rod string fell and caught EE's hand between the rod string and the hole in the decking. The event resulted in a crushing of the right hand inflicting trauma to all 4 primary fingers excluding the thumb.
The injured miner was trying to dislodge a 10' drill rod that had difficulty unthreading. During the process the worker tried to use vibration to assist with the unthreading and began banging the rod on the rod roller. In the process the worker's finger was caught between the 10' rod and the roller which resulted in a smashing of the ring finger tip on the left hand.
Employees were installing split set bolts and the helper put the next bolt in the next hole and put it in to the wrong hole. It fell out of the hole striking the jackleg operator in the left lower leg. Employee had to get 11 stitches.
While Jack-legging Miner slightly bumped elbow. They noticed a few hours later pain and swelling.
Handing a band-saw to a co-working above them. Employee lost balance, pushing on trigger, lacerating finger.
The drill crew was in the process of anchoring the core drill, the binder was under pressure when it accidentally released striking the drill helper in the right side of the face.
On 19 August at approximately 11:00 AM an underground mechanic sustained an injury to abdomen when a suspended load fell and contacted EE. The employee was in process of lifting a Kubota tractor axle with a crane when the load fell and struck EE. The load fell approximately 18 inches and weighed 150 lbs.
While grinding foreign object was embedded in the left eye.

Employee was transferring oil from barrel to barrel with a pneumatic pump, pump quit working, employee reached over to adjust pump, pump started working catching finger.

While using bench grinder, employee's hand slipped and contacted bench grinder
While attempting to lift the Tetmite drill head, employee felt a "pop" in his back.
Employee was shotcreting a fan bulkhead, when a piece of steel plugged the nozzle causing pressure to build up in the hose which blew the nozzle off. When the nozzle came off the emloyee's wrist was bent back causing a minor sprain with some small abrasions.

The employee was drilling a hole between two aluminum tanks when the drill bit became caught on the aluminum. The energy was transferred into the drill handle causing his hand to be slammed into an edge of the aluminum tank. He sustained a laceration to the top right hand requiring six sutures. After treatment he was returned to full work duty.
While installing resin bolts with 4 foot sticking out to accommodate the installation of an overhead crane, the employee got his hand in a pinch point and received a 1.5 inch laceration between the thumb and first finger. The wound was closed with steri-strips and was diagnosed to have a possible infection.
The employee was holding a piece of pipe while cutting it on a bandsaw. The pipe snagged on the saw blade, drawing the employee's pinky finger into the blade.
Rock fell down raise, cutting air hose which caused a jackleg to fall over and strike employee's hand which was between jackleg and canopy post.
Using a 4" grinder with a wire wheel when the grinder kicked back and struck the thumb.

While pulling core tube out of hole - employee was assisting tube. Overshot let go of the tube causing tube to come down. Employee had his hand on top of chuck severing left index finger.
DRILLER MISSED HITTING THE CORE TUBE AND STRUCK INJURED'S HARD HAT KNOCKING HIM TO THE GROUND.
