Miner was blowing debris with air hose when a piece of debris fell in his right eye. Miner did not report injury until 8/4/11 and was taken by Safety to see an eye specialist. Doctor did not find anything wrong with eye. Said the red will go away in a few days. Miner returned to work and finished his day. Miner was wearing his safety glasses.

Miner was operating palletizer. Bagging chair is equipped with seat belt safety switch. Safety will not allow conveyor belt to operate unless miner has seat belt fastened. Miner removed seat belt, conveyor stopped, and miner stepped onto belt. Unknown to miner, female end of seat belt contained clay which allowed conveyor to restart. Miner fell on right hip.
Miner was operating automatic palletizer. Right foot was caught in pneumatic doors. Steel toes in safety shoes held doors open until pressure could be relieved and doors opened. Miner suffered abrasions on both sides of right ankle and possible hairline fracture.
Cold, dusty, windy weather conditions. Miner sustained trauma to right arm.
EMPLOYEE STATES HE WAS BAGGING AT THE ROLLER MILL BAGGING MACHINE, A BAG TURNED WRONG & HE HIT THE SWITCH TO STOP THE METAL CONVEYOR, BUT STEPPED DOWN ON IT BEFORE IT STOPPED. THE CONVEYOR PULLED HIS LEFT ANKLE UNDER THE STEEL BAR THAT TURNS THE BAGS OVER.
EE STATES, ON FRIDAY 4/12/02, HE WAS ON HIS KNEES WELDING A STAND FOR A SWAMP COOLER & FELT A STINGING FEELING IN HIS GROIN AREA.
