EE was using the power washer to clean the concrete area where the water truck fills up. It is believed that EE was spraying off EE's work boots (an unsafe act) when the hot pressurized water entered the top of EE's boots and hurt EE's ankle.

Employee was changing segments on the loader bucket. Employee placed hand to catch a bolt, but the bolt caught the impact wrench, spun in place and cut the hand.
The miner was assisting a welder with fabrication activities when slag entered glove cuff and burned three fingers. The miner declined treatment at the time. The following Monday (7-17-2017), the miner complained of pain and swelling and was sent to an urgent care for treatment. Did not become a lost time accident until 7-18-2017.
The employee was running a track-mounted crushing machine. A rock the size of a cell-phone fell from the top of the feed hopper as the employee was looking up and the rock hit EE's mouth, cracking a tooth, and causing some swelling.
The miner was operating a crushing spread. The miner went to the top deck of the cone crusher to check if the cone was operating normally. The miner didn't follow proper procedure in this case. The miner looked into the cone when there was only a few rocks in it and when EE did, a rock bounced out and hit EE in the nose. The miner was seen at urgent care for a nose laceration.
EE was using D8T Cat. Dozer w/Semi-U dozer blade w/approx 3050 total hrs on machine. He failed to stop machine & reverse unit to climb back up slope he'd just dozed the material from. His failure to control machine resulted in pushing through safety barrier & causing machine to become upset from its normal operating position. Received cut to left hand.
