Employee was cutting tops of utility tugger winch anchor bolts using a 6" grinder with a cut off wheel. Employees right hand was on the body and trigger of grinder and left hand was on the T - handle of the grinder. The grinder wheel snagged and kicked out of the employees left hand cutting the left middle finger.

Mostrando los 44Ee was using a hand held grinder when the cutting wheel bound up, causing the grinder to kick back, striking the ee on the chin. Sutures required to close the laceration. Ee released to full duty.
During operation of Hilti drill, ee's right hand came into contact w/ rotary section of the drill. Ee wore a glove on right hand, which became entangled in rotation of the drill. As a result, ee's hand was injured. Injury consisted of dislocation to middle and ring finger requiring stabilization, and cuts requiring sutures. Ee released to sedentary duty, TRJV couldn't accommodate.
Ee was spraying shotcrete. EE reported EE repositioned sprayer and was in process of setting up boom when EE slipped, bumped the joystick, causing boom to swing to left, pinching EE's right index finger between the light bracket on boom and the remote holder. Partial amputation of right index fingertip involving bone.
Employee was using a grinder when the wheel came apart and pieces struck the employee in the face and shoulder.
Ee was operating a bolter. A/C unit not operating so ee had it fixed. EE felt nauseous with headache and tingling fingers. Taken to surface and given oxygen. CO finger reading higher than normal. Ee returned to work after O2 given. Seen later that day at clinic, where EE had denied ongoing symptoms. Released to full duty. Subsequent visit resulted in ee off work pending testing.
Employee was using a jackleg drill and he pinched his right fingers between the jackleg drill handle and the head. He was seen at the ER and diagnosed with a fractured right ring finger. Surgery required to place pin in finger.
A driller assistant sustained a smashing injury to the right index finger while tripping out pipe. The employee was running the hoist plug and as he spun the plug off the pipe it fell when it came out of the pipe (not being supported when it released) pinching the employees finger between the plug and the NQ pipe.
After pulling the core tube, Driller sat down behind control panel and started his rotation when control panel shot up in the air. Quill hose broke and control panel came down on Driller and pinned his leg under the control panel.
Mechanic was working on a schwing concrete pump which had been sticking and not functioning properly. The mechanic noticed a piece of concrete in the discharge tube and reached into the tube to pull out the concrete. At that moment the pump completed a cycle with the mechanic's hand in its path. The pump edge amputated four fingers and part of the mechanic's left thumb.
The injured was using a portable grinder to cut a 7/8 inch hexagonal steel stock. The grinder blade caught in the cut while in use and kicked back striking the injured on the left forearm. The injured suffered a laceration on his arm which required sutures (staples) to close the wound.
Employee smashed hand between drill hammer and stake
Employee strained back pulling on jackleg steel.
Employee slammed Left knee into Grease Zerk on the Mucker causing small puncture wound to his knee.
Employee was holding overshot when drilling yanked a rag that was caught by the wireline causing the overshot to be yanked out of helpers hand and his hand got pinched on the shieve wheel.

Emp received soft tissue injury in back pulling on Jackleg.
The emplyee was using a gringer with a cut off wheel to cut a 2inch piece of stainless steel pipe, the pipe pinched the wheel and kicked the grinder back at him. The cutoff wheel hit him in the forearm causing a 3 1/2 inch laceration.
Employee was washing nozzle on spraymec with high pressure wand; tip came out of the nozzle and directly sprayed into the back of his left hand.
Trainee was driving a split-set into the rib when the driver broke and the Jackleg drill fell against his left knee.
Miner was standing over the remix truck watching it load. Cement came out of truck and splattered in eye.
EE STARTED TO DRIVE A SPLIT SET BOLT. HE TURNED ON JACK LEG DRILL. THE SPLIT SET BOLT JUMPED OFF DRIVER ON JACK LEG DRILL RESULTING IN SPLIT SET BOLT FALLING, STRIKING HIM ON THE LEFT SIDE OF HIS FACE. 12/2/05: AMENDED TO STATE THAT THIS WAS A MEDICAL PROCEDURE TO STITCH WOUND ON FACE - 1 STITCH.
Miner was pulling jackleg out of a drilled hole smashed left hand little finger between jackleg and the leg of the jackleg.
WHEN EMPLOYEE WAS OPERATING MUCKER IN HEADING, HE DROVE UNDER A VENT BAG WITH THE HOLE IN IT BLOWING DOWN. DUST FROM MATERIAL IN BUCKET BLEW INTO HIS LEFT EYE.
Mechanic was putting a valve back in a 3 yd lhd using an overhead crane. As he was pushing the valve into the hole, the valve slipped out and smashed left elbow between the loader and the valve.
MINER WAS IN THE PROCESS OF CHANGING STEEL ON A JACKLEG. PINCHED HIS RT. INDEX FINGER BETWEEN THE JACKLEG & THE LEG OF THE JACKLEG.
Miner was pulling jackleg out of the mud. He strained muscles of lower back and between shoulders.
While operating 2 yd loader, loader tipped over on its side and employee hit the back of his head on the rops cage.
While mining in the 3155 a rock fell out of the back and struck him on the lower left back and hip.
Employee bent over to pick up a bolt-the air blew his pony tail into the jackleg steel-employee's pony tail got caught and wrapped up tight.
EE WAS JACKLEGGING IN SHAFT ZONE 3990 HEADING, FINISHING BOLTING THE BACK. EE DIDN'T FINISH THE BOLT ALL THE WAY BECAUSE HE NEEDED A LONGER DRIVER. EE WAS BACK DRIVING ON BOLT WHEN EE FELT A LOT OF WEIGHT ON HIS LEFT SIDE OF HIS BODY. THIS WEIGHT WAS A ROCK THAT CAME OUT OF FACE IN THE HEADING, RUSULTING IN AN INJURY TO HIS LEFT LEG "KNEE" & LOWER BACK STRAIN.
STRAINED ARM MUSCLES AND TENDONS DURING EVERYDAY DRILLING PROCEDURES.
WHILE DRILLING 3/8" STEEL PLATE IN DRILL PRESS, VISE SLIPPED AND PINCHED FINGERS BETWEEN VISE HANDLE AND DRILL PRESS TABLE. RESULTED IN SMASHED FINGER REQUIRING STITCHES.
EMPLOYEE WAS MOVING PUMP BEARING UNIT WITH HAND TRUCK, PULLED HANDLE OF DOLLY (HAND TRUCK) TOO HARD RESULTING IN A LOSS OF CONTROL OF THE LOAD, HANDLE OF DOLLY FELL ON TOP OF EE'S FOOT, FRACTU RED METATARSAL.
WHILE COREDRILLING A ROTATION UNIT SEIZED UP. A SPECIAL NUT HAD TO BE REMOVED TO TAKE UNIT APARTWHILE REMOVING NUT WITH CHISEL & HAMMER THE NUT BROKE APART, SENDING PARTS OF THE COMPRESSION TO OL UNIT & STRIKING EE ON LEG. THE UNIT IS SPRINGLOADED TOGETHER.
EMPLOYEE WAS CLEANING BOULDERS OFF 1550 ROCK PICK GRIZZLY, THE LOADER BUCKET CAUGHT ON GRIZZLY RAIL, CAUSING EMPLOYEE TO GO FORWARD AND HIT HIS FOREHEAD ON HANDLE OF CANOPY.
WHILE INSTALLING 12' JEMAR BOLTS WITH MECH BOLTER. OPERATOR BROUGHT BOOM DOWN TO CHANGE STEEL, SET BOOM ON GROUND, DID NOT SEE IT WAS SITTING ON A ROCK, WHEN HELPER PROCEEDED TO CHANGE STEEL, BOOM SLIPPED OFF ROCK, HITTING SIDE OF RIGHT FOOT.
EE WAS INSTALLING IRON MAR BOLTS PUT BOLT IN DRIVER & LINED UP WITH BOLT HOLE. PARTNER PULLED THE BOOM DOWN & THE BOLT FELL OUT OF DRIVER BUT THE PLATE STAYED ON TOP OF THE SLIDE EE TRIED TO P USH BOLT THRU THE PLATE TO REPLACE THE BOLT IN THE DRIVER THE PLATE FELL OFF THE SLIDE HITTING EE IN FRONT OF HARD HAT THEN HIT THE UPPER LIP
EMPLOYEE WAS IN A BENT OVER POSITION ATTACHING APANEL OR WIRE SCREEN TO BOLTER WHEN LOOSE GROUNDFELL FROM A HOLE IN PREVIOUSLY INSTALLED WIRE SCREEN STRIKING HIM IN LOWER BACK AREA.
EE WAS INSTALLING A 12' JENMAR BOLT WITH A JACKLEG AFTER INITIALLY PUSHING THE BOLT IN AND WAITING A FEW SECONDS, HE CAME OFF THE BOLT TO GET A BETTER POSITION WITH THE MACHINE. THE JENMAR BOL T CAME DOWN OUT OF THE HOLE AND STRUCK EE ON TOP OF RIGHT HAND.
EMPLOYEE WAS BEATING SPRAYMET NOZZLE AGAINST THEBOOM OF THE SPRAYMET, TRYING TO UNPLUG NOZZLE; HE SAID HIS HAND SLIPPED AND SMASHED HIS RIGHT RING FINGER BETWEEN NOZZLE AND BOOM.
EMPLOYEE CAUGHT HIS GLOVE ON THE ROD, IT SPUN HIM AORUND HITTING HIS RIGHT LEG ON THE DRILL CAPSULE.
EMPLOYEE WAS HOLDING THE INSIDE OF NOZZLE ASSEMBLY WHILE ANOTHER EMPLOYEE WAS SPRAYING IT CLEAN EMPLOYEE WHO WAS SPRAYING SLIPPED CAUSING A SPRAY GUN TO GET TOO CLOSE TO EMPLOYEE. RIGHT HAND WHO WAS WEARING GLOVES CAUSING SMALL CUT ON RT LITTLE FINGER AND WATER INJECTION INTO FINGER.
