Miner was bolting the right rib installing a 6-ft dog when the glue set sooner that expected, causing the jackleg to spin out of EE's hands. The jackleg struck EE in the jaw, causing EE to fall and strike the back of EE's head. Miner lost consciousness briefly.

While using a hand grinder to cut a hinge, tha hand grinder kicked out striking the EE on the left knee causing a laceration. the laceration required sutures
While drilling an I-drift round, the injured miner's drill steel broke causing them to fall forward. As miner fell, miner reached out to catch themself on the rib and miner's hand contacted the edge of a bolt mat causing a laceration on the left palm/hand. The miner was taken to the doctor where miner received sutures.
The Miner was drilling a hole in the back to install roof bolts, as EE let down EE's machine, EE tripped on the oiler causing the machine to strike the miner on the right cheek. The Miner was taken to the local hospital and was treated with sutures for a laceration
Nipper was knocking a drill bit off of a jackleg steel. A chip of rock broke off and flew approximately 20' and hit the miner in the eye.
While bolting employee got some grit in left eye. Eye was flushed out at doctor's office and eye drops were prescribed.
While finishing drilling a small rock burst ejected a small amount of rock from the back striking the miner on right knee causing a laceration.

#3 Hoist House. Hoist man was using a personal hand blender. EE did not unplug tool before cleaning. The blender was activated and cut EE's right thumb and forefinger. Emergency room sutures.
Miner and EE's partner were breaking boulders on the 4900 #3 grizzly. A rock chip penetrated the tissue of EE's upper right thigh. ER visit and sutures.
Miner was bolting the drift on 5500 level. The bolt and plate fell out of hole in back. The plate came in contact with Miner's left forearm causing a laceration. Sutures were needed.
2400-081 Mucking out ore heading hit dag or rock while digging out the pile causing an abrupt halt bouncing miners body in mucker. Dismounting mucker noticed left abdominal pain, possible hernia.

4900-210 Move wooden post with slusher, end of post caught rib came back hitting miner in lower right leg. Went to clinic - released to full duty on March 26th. Miner went back in on April 4th - doctor put on lost time till April 12th - elevate leg and have MRI done
Changing bit on pneumatic chipper, hit trigger on shaft divider and shot out bit hitting miners upper lip.
4900-195 Incline, drilling with jackleg for roof bolting, machine came back striking miner with handle in right cheek, fracture right sinus

Pulling machine out of ratty hole, felt something pull in left shoulder. EE continued to work till Feb 16, 2015. Went to clinic and was put on restricted duty - doing physical therapy.
This accident was originally reported as a minor inj. Employee was diagnosed with sprained wrist on 6/17/14. He was drilling with a jackleg when the steel hung up and the machine rotated suddenly which twisted and sprained his right wrist.
The employee was bolting the back with a jackleg drill. As he was backing out of the hole the steel puller bounced up and released the steel which caused it to fall striking the employee between the thumb and forefinger. This caused a laceration which required sutures. The employee was disciplined for using defective equipment.
An employee was trying to reposition an electric motor that was hanging vertically from an overhead hoist. He placed a block under the motor and slowly lowered the hoist so the motor would tip. The motor rotated unexpectedly and moved laterally which caused his finger to become pinched between the block and a protruding housing.
Accident occurred on 6/7/13 but was reported as minor. EE continued to experience symptoms and was treated on 7/17/13. EE was task training on the termite drill. This work requires repetitive twisting motion with both arms. He noticed symptoms of numbness and pain later that evening after shift. He did not feel any pain during his shift. EE was not used to this activity.
The employee was drilling in the back with a 4' steel for the purpose of installing ground support. The steel broke near the chuck which caused the machine to extend unexpectedly and start to fall. The employee quickly retreated but his left arm made contact with a split bolt which was standing next to the rib. This caused a laceration which required sutures.
An employee was working on a Welden pump that was pumping water from a concrete pour. There was a leak in the discharge hose which sprayed water in his face when the pump cycled. The water sprayed under his safety glasses and caused significant irritation to his eyes. He was prescribed medication to treat the irritation.
Employee had just finished drilling a hole in the face and as he pulled the steel out of the hole he stepped on a rock and lost his balance. This caused him to lose control of the machine which pivoted forward on the leg and caught his hand in a pinch point. This resulted in a fractured bone in the employee's hand.
Employee had just finished drilling a lifter hole with a jackleg drill and was pulling the drill out of the hole when he felt a pain in his back. This injury was initially reported as a minor injury but persistent symptoms caused the employee to seek medical care on 9/26/12. This was diagnosed as a strain and the employee received a cortisone injection.
Employee was sitting on motor while EE was mucking with 12b mucker and got sand in his right eye.
Employee was lifting the step of the track mucker to put it into the mucker bucket. Employee was assisting the lift with his leg and his right little finger was caught between the step and the mucker bucket. Right little finger was fractured just below the first joint.
The employee was pulling out of a jackleg hole when he inadvertently opened the chuck tender which released the steel unexpectedly. This caused the machine to fall forward and his hand was pinched between the nose of the machine and the rock. His glove was caught in the chuck tender so he couldn't pull his hand away.
The employee was engaged in the slushing phase of his mining cycle. He walked by a compressed air ventilation device ie:(whiz bang) and a particle was blown into his eye. He reported that he was wearing safety glasses which is required by our safety policy.
The employee was drilling the face with a jackleg drill. As he pulled the drill out of a back hole he felt a pain in his shoulder. This injury occurred during the course of his normal work and was not the result of any occurrence out of the ordinary.
EE was bolting with a jackleg drill and as he attempted to insert the split bolt driver that was in the chuck into the bolt he lost his footing and the machine continued to move forward and rotated back on the swivel to the locked back position. At this moment his left middle finger was in the pinch point of the machine.
The victim had just installed a dywidag bolt with a jackleg. As he pulled the driver off of the bolt he lost his balance and the machine pivoted forward against the leg. The victim's hand was in this pinch point and the result was a laceration on the pinky finger and possible hairline fracture.
Running bazooka diamond drill, cycle drill steel cuff of glove caught in slide mechanism crushing tip of right thumb resulting in amputation of thumb tip.
Miner was preparing to bolt the roof when he stumbled and inadvertently twisted the leg feed on the drill. Machine rose up suddenly and struck the miner in the mouth. Miner's front bridge was broken and had 2 sutures on the inside of his upper lip. There were 4 teeth broken out of the bridge.
Drill steel was hung up in hole, pulling on machine came out quick and smashed "R" ring finger between rib and jack leg which cut tip of "R" ring finger.
As EE was bolting his brow round in 40-298W, a slab peeled off the crown of the brow. The slab hit the end of the steel and pushed the machine back towards the inside cap. This caused the EE to get his fingers pinched between the machine and the cap.
EE was drilling brow round 4th hole in row 4, had difficulty collaring hole. EE woke up on deck. Supervisor conducting accident investigation assumes EE lost control of jack leg drill and drill steel clipped him on the right side of his head.
