Mostrando los 17The operator of the base plant was standing on the guarded platform guiding the backhoe operator to place the rock breaker on the rock jam. The plant operator signaled the backhoe operator to swing to the left and it would not stop. The rock breaker struck the plant operator on the left cheek. EE fell down 4 stairs and injured.
Flashburn on eyes due to welding in truck bed
Employee was using a grinder to buff a shaft. While he was running the grinder one of the strings from his hooded sweatshirt fell out of his coat and got wound up in the shaft of the grinder behind buffing wheel. This caused the grinder to get sucked up to his face striking him in the mouth. When it hit him it chipped a corner off of his front tooth
While doing a slump test the employee was packing down mud with a tamping device & caught his finger between the cone and the tamper. It caused a small open wound on one finger, and partial removal of a fingernail. The tamper was hand-held and the employee misjudged the distance between his finger and the item he held in his other hand.
Employee was in the process of building a guard, and was welding at the time, when another employee with a lit cutting torch pulled on the torch to get more slack in the hose. In the process he ran the lit torch across the arm of the employee that was welding. This caused a second degree burn on the employee's upper left forearm about two inches in diameter.
EE RECEIVED WELDER ARC BURN TO THE EYES WHILE HELPING AN EE WELD CHUTES. EE WAS USING P.P.E. BUT MAY HAVE SUSTAINED THE BURN WHILE WORKING IN THE AREA.
CUTTING AND GRINDING STEEL THROUGOUT THE DAY, WHEN THE EE GOT HOME AFTER HIS SHIFT HAD SOMETHING IN EYE BOTHERING HIM AND CALLED SUPERVISOR.
DRILLING NEAR EDGE WHEN BANK FAILED, PARTIALLY TURNING DRILL ON SIDE.
NO DIRECT CAUSE OR INJURY. NECK STARTED GETTING STIFF AND AT SHIFTENED TIME HAD PAINS, IN LEFT SIDE OF HEAD, NEAR EAR. WAS TAKEN TO CLINIC AND DIAGNOSED WITH BRACHIAL NURITIS OR RADICULITS NOS PULLED MUSCLE OR PINCHED NERVE.
EMPLOYEE WAS CLEANING UP BUILD UP FROM UNDER JAW FEEDER. AS EMPLOYEE EXITED AREA ON HANDS & KNEES, A ROCK FELL FROM JAW FEEDER ABOVE CUTTING LEFT RING FINGER. (PINCHED BETWEEN ROCK & GROUND.)
EE WAS REMOVING A CUTTING EDGE FROM A SKID-STEER WITH AN IMPACT GUN. WHEN NUT LOOSENED HE WAS HOLDING BOLT IN WITH HIS FINGER. THE BOLT DROPPED & SPUN CUTTING HIS LEFT INDEX FINGER (9 SUTURES)
OPERATING DOZER - REPETITIVE MOTION. EE NEEDS THERAPY 3 TIMES PER WEEK FOR 2 WEEKS & RECHECK.
EE AND ONE OTHER EE WERE FUSING POLY PIPE AT THE TIME THE INCIDENT OCCURRED. THE PIPE WAS IN A BIND AND WAS LYING IN THE MACHINE. THEY WERE GOING TO GET ANOTHER "BITE" ON THE PIPE WITH THE EXC AVATOR WHEN THE PIPE CAME OUT OF THE FUSION MACHINE AND STRUCK EE ACROSS THE LEGS.
HOISTING CRUSHER PARTS WITH HYDRAULIC BOOM & CHAIN, EE WAS HOLDING ON TO CHAIN WHEN PICKING EYE BROKE OFF OF PART STRIKING EE'S HAND & WRIST.
2 EMPLOYEES WERE CHANGING BUCKET ON CAT BACKHOE 426B. A THIRD EMPLOYEE APPROACHED WORK SCENE AND IN OPERATION. THE SECOND EMPLOYEE WAS SEARCHING FOR PLUGS IN THE CAB AND ACCIDENTLY, ENGAGED THE BOOM SWING LEVER. THE 3RD EMPLOYEE MOVED BACKWARD AS THE BOOM CAUSE FORWARD PUSHED HIM ALSO STRIPPED OVER A CUTTING EDGE LYING ON THE GROUND EMPLOYEE FELL ON HIS HIP.
EMPLOYEE COMPLAINED OF PAIN IN RIGHT ELBOW.
