a mechanic was separating a Sand grinder tank using an overhead hoist. As EE lifted the top part of the tank, the tank swung unexpectedly striking EE's left thumb between the two tanks. The employee received a thumb laceration requiring 12 sutures.

Mostrando los 12Miner was hoisting a piece of safety equipment onto the top of a slurry railcar. The item snagged the catwalk on the car. When it released it swung in and struck the miner just below the left knee causing a knot and bruising
The EE was operating bag machine when the EE reached up to position a fan. The blade on the fan struck EE's thumb causing a cut. Stitches were required in thumb.
Employee was using cordless grinder with cutoff disk to cut clamps from Sandgrinder hose at Sandgrinder manifold. When the Grinder kicked back from possibly contacting the wire inside the hose, the hand that was holding the side handle came off the handle and struck the blade. The cutoff disk cut through glove and into finger requiring 7 stitches in finger.
EE WAS OPERATING DOZER. EE SAID HE HAD PUSHED SOME WATER OUT OF A SMALL HOLE IN THE CLAY. WHEN HE WAS BACKING UP AND THEN GOING FORWARD, THE DOZER BUCKED WITH HIM, CAUSING A SHARP PAIN TO GO UP HIS BACK.
Employee was on land leveler tractor being pulled down hill by a dozer. tractor was not running so had no brakes and ran into back of dozer shaking up ee. complained of back pain.
Operator was attempting to get equipment-dozer unstuck...muddy conditions....Dozer dislodged and came down stockpile. Blade hit the bottom of stockpile causing abrupt stop. Operator was pitched forward into dozer cab striking head resulting in laceration to scalp.
EE WAS WELDING ON A DRILL PAN AND REMOVED HIS HELMET TO INSPECT THE WELD. HE BEGAN WELDING WHILE HOLDING THE HELMET IN HIS FREE HAND. HE RECEIVED FLASH BURNS TO BOTH EYES.
EMPLOYEE WAS CLEANING CALENDER AND TOP ROLL DROPPED UNEXPECTEDLY PULLING EMPLOYEE'S HAND BETWEEN ROLLS RESULTING IN CONTUSION TO THE RIGHT HAND.
MILLWRIGHT WAS WORKING ON A PIPE SWITCHING BETWEEN WELDING AND GRINDING. WHILE GRINDING, A PIECEOF METAL WENT INTO EYE. HE WAS WEARING FACE SHIELD.
EE ENTERED WOMEN'S BATHROOM AND AN UNKNOWN PARTICLE FLEW INTO HER LEFT EYE. THE PARTICLE IS SUSPECTED TO HAVE FLOWN INTO HER EYE FROM THE OVERHEAD HVAC DUCT. PARTICLE COULD NOT BE WASHED OUT(E MBEDDED) AND PRESCRIPTION EYEDROPS WERE GIVEN.
EMPLOYEE WAS USING A DRILL PRESS TO DRILL HOLES IN METAL PLATE. METAL PLATE WAS NOT COMPLETELY TIGHTENED DOWN WITH VICE. AS A RESULT, THE VICE SLIPPED AND SPUN CATCHING EMPLOYEE'S LEFT RING F INGER AND CAUSING A LACERATION THAT REQUIRED STITCHES.
