EE states they were grinding a handrail by VSI crusher when a piece of metal went into eye. EE stated they were wearing proper PPE for the task. EE sought medical treatment the following night where the foreign body was removed from left eye. EE did not report the initial incident or subsequent medical treatment to the employer.

Mostrando los 12EE was tightening hammer bolts. EE was trying to get torque gun out of the crusher and dropped it on EE's pinky finger. EE went to local urgent care and a fracture on EE's small finger was identified.
Employee was operating a jack leg installing bolts into the face of the high wall. The employee was installing the bolt at a downward angle. The socket slipped off the head of the bolt causing the jack leg to rotate forward and mash the operators hand between the machines handle and the highwall face.
EE was replacing a cut edge on a Komatsu WA-500 loader using a 3/4" impact to remove the old bolts. One didn't come out so EE used a hand to grab the bottom of the bolt & use the impact to finish removal. When the bolt spun the sharp edge cut EE's finger.
Centrifugal pump moved approx. 5 inches where 2 Mideast Workers were working. They were welding box tubes on the side of pontoons. IE was on platform and other EE was on the boat welding. IE was lowering box tubes down to EE with a rope and EE would weld them . EE heard a loud bang.
Operator in trackhoe using hydraulic hammer, secondary rock breaking. Heard a loud crack from above, dust flew and rock hit the cab. Lacerations on head, hands and left knee each requiring stitches. Admitted overnight for observation only.
I was fusing 12" HDPE pipe. While working the hydraulics, I got my left hand in the wrong spot pinching it between a block of wood and clamp on the fusing machine. Broke my index finger on left hand.
EE was walking from the plant to the shop to get on a piece of equipment.When he walked by our #4 screen,Something bounced from the screen and got in his eye.EE reported the incident to management at the end of his shift.he did have a scratch on his lens.He was released to work immediately with NO restrictions by dr's.
Fusing 12" black poly pipe for a water discharge line. Employee was helping stage pipe in the fuser. As a finished section was pulled over and off of the first pipe support stand the pipe's end struck the employee on top of the right wrist. This caused a laceration to the employee's wrist. The employee was taken to an urgent care where he received 13 stitches.
Employee was cleaning out the #7 bin on the secondary plant. The inspection door had been removed and a water hose was being used to spray out material. Employee's left hand was resting on the mounting frame of the door, when material broke free striking his hand.
WORKER WAS REMOVING SCREEN BOLTS FROM SHAKER WITH A AIR WRENCH WHEN A BOLT THAT WAS WORE DOWN REAL SHARP SPUN ACROSS HIS FINGER AND CUT IT OPEN.HAD TO HAVE FINGER STITCHED UP, 4 STITCHES.
EE WAS GREASING LOADER. AIR GREASER PISTON JAMMED AND EE TRIED TO MANUALLY PUSH IT UP, WHEN IT BECAME FREE IT SMASHED HIS MIDDLE FINGER ON BOTHHANDS.
