EE was retrieving a rock splitter tool from a rock when the rock separated, causing the tool to move EE's hand against the side of the man basket and when the rock separated, the man basket moved causing EE to hit right knee on the side of the man basket. EE declined medical attention on 6/14/16, EE called in from a scheduled day off on 6/21/16 requesting to see a doctor.

While grinding on a material screen plant a foreign object got into his right eye. He didn't notice it until 5:00 in the evening when showering.
