Getting onto a dozer ee neck popped causing instant tingling in extremities. On 8/19 saw doctor and was given a release to return to work with no restrictions. On 9/2 MRI showed cervical stenosis but uncertain to cause or if work related. EE has remained home due to the uncertainty, Reporting at this time, does not fit any category and may not be work related.
EE was working on palletizer 2. Part of the frame came down crushing his left index finger between the two pieces of iron.
Employee attempting to convert main plant packer saddles 100 lb. to 50 lb. bags. Placed both hands underneath the saddle, rather than on top. When he bent over, helmet activated switch causing the saddle to drop, his thumbs were caught between saddle and saddle mount. Received approx. 8 sutures to right thumb. Employee received Vicodin and Keflex and was released for full duty.
Employee stated he developed discomfort in right shoulder while operating scraper the week of April 18, 2005. Employee did not complete an incident report, do not know exact date or time of incident. Diagnosed May 25, 2005.
Injured was attempting to grind bentonite sample for testing, motor would not start, turned switch off & on rapidly - still wouldn't start. Reached hand underside of guard. Touched belt & motor started, pulling thumb into pinch point of belt & pulley.
Ee had put 1/2" pins in tablet machine. He started it without guards in place. He thought there was a loose part & reached his rt hand into operating machine to see if it was loose. The moving machine caught his hand. He was taken to local dr, & was sent to Billings MT. Drs removed his little finger, index finger at first knuckle, & broke his middle finger.
EE WAS REPLACING BAG CODER ON PALLETIZER AND PUT LITTLE FINGER OF RIGHT HAND ON FRAME OF CODER. A 100LB BAG PASSED UNDER CODER PUSHING CODER AND FRAME PINCHING IT BETWEEN FRAM AND CROSSBAR. FI NGER WAS RELEASED AFTER BAG PASSED THROUGH. TAKEN TO E.R. - FINGER WAS STITCHED AND BANDAGED AND EMPLOYEE RELEASED. EMPLOYEE DID NOT LOCK OUT.
EE WAS OPERATING D9 DOZER. WHILE BACKING UP, HE RAN OVER A WINDROW OR LARGE ROCK CAUSING THE REAR OF DOZER TO SLAM DOWN HARD ON ITS TRACKS CAUSING PAIN TO LOWER RIGHT SIDE OF BACK. EE RETURNED TO WORK NEXT DAY AND BEGAN SEEKING MEDICAL ATTENTION THE FOLLOWING WEEK.
WHILE ATTEMPTING TO UNPLUG CHUTE ABOVE HOLE PLUG PACKER AIR VALVE EE PLACED HIS HAND THROUGH AIR VALVE TO REACH PLUG. WHILE HIS HAND WAS INSIDE VALVE, IT CLOSED, PINCHING 3 FINGERS ON LEFT HAN D CAUSING INJURY.
