Mostrando los 16Ee was using air lance. Rt eye felt irritated after task was completed. Began eye irrigation with supervisor. Small particle from his upper eyelid was removed. The EE was then taken to a medical provider. Both eyes were examined and found an abrasion on the right eye. EE was prescribed an antibiotic and was release to full duty with no restrictions the same day.
Mechanic suffered a laceration to his Left thumb, on the dorsal side, while drilling a hole in plumbers strap to secure the air line seal air on R1 pump discharge bearing. The drill caught and the metal curled toward him and cut his Left thumb between the 2 joints.
EE Was reaching for vacuum hose when his left arm was caught in hose resulting in swelling and abrasions.
Employee had face shield & prescription safety glasses on while using grinder. Dust & particles became airborne & irritated employee's eye. First aid was given & employee was taken to the doctor. Released to full duty the same day.
EE was using an air lance 2 dislodge remaining mat'l with air. He jabbed at the blockage 2x & hot mat'l came out door burning ee ear. He then came 2 ctrl room 4 assistance. At no time did we believe that there was any hot mat'l remaining to drop above the tipping valve door as it was completely cleaned out during 1st step of job. Additional details available, no room 2 input.
Hydraulic wrench slipped and hit the right lateral side of his calf and caused pain to the right medial side of his knee. EE was examined and returned to full duty, however, in a follow up exam and MRI shown there was a torn ACL, MCL and mincus. He was put on modified duty on August 2, 2010.
Employee was using a personnel blow off station to blow dust off of himself and clothing. Dust got into his right eye. Eye flushed immediately. Taken to the doctor's office, diagnosed with minor "lime" burns. He was given a prescription for antibiotic eye drops.

HE WAS GIVE ANTI BIOTICS AND ANESTHETIC. EE COMPLETED HIS SHIFT. HE WENT TO THE DR ON NOV 26. FIRST AID USING EYE WASH WAS ATTEMPTED BEFORE SENDING HIM TOT HE DR. EE WAS ON CALL IRRITATION NOT BAD TILL AM OF
COWORKER WAS LIFTING LID/GATE OF ROLLOFF BOX W/ FORKLIFT FORKS. EE ENTERED ROLLOFF AREA TO BEGIN WORK PRIOR TO LID/GATE BEING PROPERLY SECURED. LID SLIPPED FROM FORKS AND FELL, HITTING EE IN K NEE CAP. PROPER PROCEDURE WAS NOT FOLLOWED BY EE.
EMPLOYEE HEARD AN AIR LEAK- WHILE INSPECTING FORTHE LOCATION, FOREIGN MATERIAL STRUCK THE RIGHT EYE. WENT TO THE OPTOMETRIST ON 9/10/01, HE WAS PRESCRIBED BACITRACIN.
EES WERE PREPARING TO DUMP 1 TON OF BALLS FROM BALL HOPPER INTO MILL. HOPPER WAS FIRST SET TO FAR WEST AND WHEN IT WAS RAISED TO CENTER OVER DOOR THE HOPPER SWUNG EAST PINNING RIGHT FOOT BETWE EN HOPPER FRAME AND EXPANSION RING OF MILL.
EE WAS USING A DRILL PRESS TO DRILL HOLES IN "L"BRACKET MADE FROM ANGLE IRON.DRILL BIT CAUGHT IN MATERIAL, SWUNG AROUND, CLIMBING DRILL BIT, STRUCK RIGHT FOREARM. LACERATION REQUIRING SUTURES.
2 OPERATORS PROCESSING DRUMMED MATERIAL INTO DISPERSION TANK WITH HIGH SPEED MIXER. MIXER MOVED TOO FAR. OPERATORS PUSHED AGAINST HEAD OF MIXER TO MOVE IT BACK TO NORMAL OPERATING POSITION. OPERATOR THEN NOTICED DISCOMFORT. OPERATOR SAW DR. ON SEPT. 6, 2001. CONDITION AGGRIVATED. WENTTO REGULAR DR. ON 10/03/01- ON 10/09/01 TOLD NOTTO WORK.
EE WAS CUTTING IRON INSIDE OF DRAG THRU INSPECTION DOOR WAS PUTTING TORCH IN THE HOLE TO CUT & ACCIDENTLLY STUCK TORCH HEAD IN CUFF OF GLOVE
EMPLOYEE OPENED AIR VALVE AND A HOSE THAT WAS UNCONNECTED WHIPPED UP HITTING HIM ON THE LEFT SIDE OF HIS CHIN. LACERATION, REQUIRING SUTURES.
