Employee was removing a motor and reducer. Had motor rigged up with motor puller and chain and was helping lower the motor to the ground when the shaft slid out of the broken section of the auger jerking the employee forward. Declined initial treatment. Went to the clinic on April 10, 2026 due to shoulder pain and was diagnosed with a right shoulder strain and deltoid tendinitis.

While using a chain hoist to open the door on a hammermill the door moved more than expected after passing center balance point causing the employees hand to get pinched between the door and the clevis attached to the hoist
Person was injured during routine cleaning of pin mixer. Machine was locked out, operator did not wait until it came to a complete stop before opening door. EE used EE's foot to get through buildup in front of the door and a pin attached to the spindle struck EE's foot. Broke some bones, caused a large laceration. One toe had to be surgically removed.
Worker was cleaning out a plug air locking device under bag house unit and air lock engaged catching his hand while removing blockage amputating two ends of his right hand fingers.
EE was blowing off the dryer and felt the heat by his hand so reflexively pulled his arm back bumping his right elbow. He did not report it until the next day and declined to see a doctor. IT was still bothering him on 7/9 so we sent him to a doctor. He missed work on 7/9 and returned to work on 7/10.
