Cutting with a torch, using a grinder, welder, and placing T-Bolts inside Kiln 7 cooler. Debris got into the maintenances workers eye. The maintenance worker noticed the eye irritation around 08:00 pm after being home for a few hours. Most of the work was performed overhead.

EE walks into the baghouse to see why the auger wasn't working. EE then walks back to the motor and sees the cover where the fan is looked like it had been beaten to death causing the fan not to run due to obstruction. EE was pulling on the belts to get the shaft to turn and that's when the motor kicked on causing EE's hand to get caught in between the pulley and the belt.

Employee reported employee had slag in left eye from working beside someone polishing boiler tube. Employee reports employee was wearing safety goggles. Employee presented to urgent care to have eye flushed. Employee now reports no discomfort to eye.

The EE was tramming the drill in the pit. The door was latched open. While moving, the door shut onto the EE's left hand. At the time, the injury consisted of minor lacerations. On 9/7/21, the EE's finger was swollen. We took EE to the doctor and EE had a minor fracture thus making the accident reportable.

EE was tightening the fasteners on the black plastic pipe (48"diameter) for the #1 kiln stack. EE had 7/8" wrench in left hand and a 1/2" impact with a 3/4" socket in right. When they pressed the trigger on the impact, the nut caught a burr on the bolt threads and the wrench spun out of hand and slapped EE across the knuckles.

Employee was using a hand held grinder on the seal flange on the # 4 feed end of the Kiln. A lot of material was airborne. Airborne material got into EE's shield and got past the safety glasses and into EE's eye.

Employee was performing maintenance on a dust pump. After replacing a butterfly valve and an auger bearing, the system was activated. The pump wouldn't discharge so the employee pressed the power switch again, the pump discharged sending fly ash out through the sliding hopper gate. Fly ash entered around the employee's safety glasses into eyes.

Employee was removing 3/4 threaded nuts from the screens rails. One got hung in the 3/4 Impact wrench socket. EE put left index finger into the 3/4 threaded nut, then accidently pushed the Impact wrench trigger. It rotated onto the tip of EE's finger

Employee was grinding on a flange on the #4 rotary kiln ring gear. Some dust got into EE's right eye. EE rubbed the eye probably imbedding the material from the dust. Later that day EE's eye became irritated.

Employee was using a 55ton hydraulic jack and a 6 inch I Beam to reline a rotary kiln hood. The jack slipped out of place, striking the employee to the face.

Employee was hooking up the welding trailer using the hydraulic lift from the tractor. Positioning the tong of the trailer and using the controls from the tractors hydraulic lift at the same time, the tong receiver on the trailer slipped off the ball hitch smashing EE's index finger.

Employee was cutting metal using an Oxyacetylene torch. It appears some rust was behind the metal employee was cutting, hot molten metal splashed back into employee's face. A small piece of molten metal worked its way behind employee's cutting goggles and attached to employee's eye.

The injured employee and another employee was erecting a 30 foot conveyor into place. The conveyor was suspended with chain and cable hoist. The conveyor needed to shift to the left a couple of inches. The injured employee had thumb in the area where IE believed it was hung up. The conveyor unexpectedly shifted forward pinching IE's thumb between the chute and conveyor.

Employee hooked the crane cable block into the lifting eye of the conveyor belt carriage. When the crane operator began cabling up, employee seen the cable block drifting toward a 55 gallon oil drum. He pushed on the block with his right hand. The outer cable caught his right thumb, pulling it into the cable block.

The EE was welding inside the loader bucket, using a LN25 wire welder. That Friday afternoon his eyes were sore, by Saturday evening his eyes became worse and began to swell.

EE was using a small jack hammer to remove the castable material from the #5 firing hood. A piece of castable material fell and hit the top of his left foot.

EE and a contractor EE where working from a bucket truck erecting an induced draft exhaust stack on the #7 kiln. On the decent the bucket truck boom moved suddenly giving EE a jolt.

EE was using a Purox oxygen & accetylene cutting torch to cut an 8" I-Beam which was at a 45 degree angle & secured with a chain fall on 1 end & welded to another I-Beam on the other. It must have been under pressure because when beam was cut the chain fall slid down & allowed beam to drop Appx 3" & during this our employee's wrist became squeezed betwen the chain fall & beam.

EE was in the maintenance shop tacking some pieces of metal together. He was wearing his safety glasses not his shield while doing this task. He positioned his metal and then looked away as he struck arc. At this time he believes the rusty material got into his eye.

EE was helping another employee change cooler plates inside the #6 kiln cooler system. EE was located in the lower compartment cutting a T-Bolt out using an Acetylene cutting torch. During this process he has his head slightly tilted, allowing a piece of hot molten metal to enter his left inner ear.

Employee was removing refractory (Castable) material from the #8 Kiln with an air jack hammer. He had his right hand positioned between the anchor that holds the refractory & the jack hammer handle. When he engaged the jack hammer his hand got caught in between the anchor and jack hammer handle.

EE was welding inside the welding shop on a piece of equipment. A hot piece of molten metal fell on top of his work boot. It laid on one corner of his boot, then burned through the fabric causing 2nd degree burns to his toe area before he was able to remove his boot.

Employee was using a hammer drill to place anchor holes in concrete. The drill bit became bound causing the hammer drill to twist in his right hand. He visited Hickory Orthopedic Center on 03-01-2007. Review of his roentgenogram confirmed a malrotated, oblique, distal diaphyseal of his third metacarpal.

The employee was looking into the #3 rotary kiln, when a foreign body material blew back & stuck to the employee's left eye (cornea area).

EE was shooting a ring out of the #6 Rotary kiln. A piece of metallic material ricochet back toward him penetrating into his left shoulder.

EMPLOYEE WAS CUTTING AN 8"I-BEAM WITH AN OXYGEN & ACETYELENE TORCH. WHEN HE HIT THE OXYGEN IT BLEW HOT METAL SLAG BACK INTO HIS FACE. HE DID HAVE HIS CUTTING GOGGLES ON, THE HOT METAL BURNED HIM. HE REMOVED SOME OF THE METAL & DEBRIS HIMSELF ALONG WITH THE SUPERVISOR & THEN WAS TREATED AT THE HOSPITAL. EE WAS RELEASED BACK TO WORK THE NEXT MORNING.

EMPLOYEE WAS GOING TO TURN THE HIGH PRESSURE WATER LINE OFF. HE LET GO OF THE NOZZLE TO TURN THE VALVE OFF, HOLDING THE WATER HOSE WITH THE OTHER HAND. WHEN HE LET GO OF THE NOZZLE IT WHIPPED BACK & STRUCK HIM IN HIS FOREHEAD & NOSE.

Ee was shooting a klinker out of the #2 rotary kiln, he was using an 8 gauge Winchester ring blaster. when he fired a shell into the rotary kiln, something ricocheted back at him, a piece of metallic material penetrated the right side of his neck. The foreign object measured 3mm x 2mm. His doctor is going to remove it when the swelling goes down.

EE WAS PUTTING GREASE INTO THE BEARINGS ON A TAIL PULLEY. AFTER GREASING THE BEARINGS HIS GREASE GUN GOT STUCK ON THE FITTING. HE PULLED ON THE GREASE GUN, IT CAME LOOSE STRIKING HIM IN HIS FR ONT TOOTH.

THE ACCIDENT HAPPENED ON #1 KILN, THE EE WAS HELPING OTHER KILN HELPERS BUST MOLTEN ROCK FROM THE DOOR OF THE COOLER SYSTEM. AS HE WAS POSITIONING THE JACK HAMMER TO THE MOLTEN ROCK HE TWISTED & APPARENTLY STRAINED HIS LOWER BACK. HE'S ON RESTRICTED DUTY.

EMPLOYEE WAS GRINDING ON #1 KILN, THE GRINDER KICKED BACK AND STRUCK HIM ON THE LEFT KNEE.
