EE was hooking a wire cable around a block of stone at the mill. The wire cable was hooked to an overhead crane. The block of stone was sitting on a 2" rock skid. EE was using right foot to push the wire cable under the rock. The crane operator thought heard EE was done and started up with the wire cable to raise the rock. EE's right foot was caught between the wire cable & rock.

Got right hand caught in the belt of the pitching machine. They had to cut the belt to get hand out.

As A bundle of scrap stone was being set down to be dumped, employee had gotten their right thumb between two pieces of stone.

EE WAS OPERATING A PUMP. HIS HAND SLIPPED AND CUT HIS LEFT MIDDLE FINGER.

EMPLOYEE WAS DRILLING UP BED OF CUT WHEN HE SLIPPED & DROPPED THE DRILL THAT HE WAS USING ON HIS LEFT FOOT CAUSING PAIN TO THE LEFT FORO, SPECIFICALLY THE GREAT TOE & JOINT [EMPLOYEE WAS WEARI NG STEEL TOED BOOTS AT THE TIME OF THE ACCIDENT.

EMPLOYEE WAS HOLDING METAL TRACK, SWING AROUND TOO QUICKLY & HIT RUBBER TIRE ON BELT SAW MACHINE WHICH RESULTED IN EMPLOYEE'S HAND W/ THE TRACK BOUNCING OFF OF TIRE & HITTING THE SIDE OF THE M ACHINE & THE FORCE OF THE CONTACT MASHED THE TIP OF THE EMPLOYEE'S FINGER ON HIS RIGHT HAND. A TOTAL OF THREE STITCHES WAS REQUIRED TO CLOSE WOUND.

EE WAS DOING HIS JOB & THE WIRE SAWYERS WERE PULLING WIRE, WIRE BROKE STRIKING EE ON RIGHT SIDE OF NECK & KNOCKED EE DOWN & BRUISED RIGHT HIP & HURT HIS NECK.
