Employee reported eye irritation. Stated they had washed it out with saline solution, but the following day was still experiencing discomfort. They requested to be seen at the clinic. Employee was seen by clinic. Instructed to return to work the next day. Prescribed antibiotics. The employee's medical visit concluded after the end of work shift on 9/15.

A five-employee crew was stringing a new conveyor belt using a 350 excavator, forklift, and pry bars. As the belt was being pulled into place, it lifted out of the troughing rolls, swinging toward three employees standing on the catwalk. The miner was caught between the belt and the handrail, causing a strain to neck, jaw and shoulder.

Hot slag from welding flew into eye of employee.

While chipping at a boulder in the feeder of Plant 2 with an air hammer, the employee felt a twinge in his low back.

CONTRACTOR'S EE WAS REPAIRING A HAUL TRUCK TIRE.HE INCORRECTLY PLACED HYDRAULIC RAM TO REMOVE METAL TIRE BEAD. WHEN HE PRESSURIZED THE RAM IT POPPED OFF MOUNTING & STRUCK HIM ON THE FOREHEAD. INJURED WAS AN EE OF A CONTRACTED COMPANY WHICH DOES NOT HAVE AN MSHA ID#.

EMPLOYEE WAS GRINDING SOME METAL AND SOMETHING SETTLES INTO HIS EYE. CO-WORKER REMOVED FOREIGN BODY. 2 DAYS LATER EYE WAS IRRITATED SO EMPLOYEE WENT TO CLINIC. EMPLOYEE GIVEN PRESCRIPTION MEDI CINE.

THE EXHAUST AIR ON AN AIR TOOL BLEW DUST INTO THE EE S EYE. THIS IS REPORTABLE BECAUSE THE MED CLINC REFERRED THEEE TO AN EYE SPECILIST.
