The 103 Dragline at Ramp 53 contacted the cable handler #768 and was struck by the left rear house of the dragline while the dragline was in full swing. As a result, the cable handler was tipped onto its side. The cable handler operator was transported to the Dr. Office for further medical evaluation. On 10/13/25 the MRI confirmed that the employee would need surgery.

Injured employee was working on the longwall face, pulling in shields. As the shearer passed near EE's location the tailgate side drum picked up a large piece of coal, the piece of coal hit the pan and then struck employee on shoulder and foot. Employee has not been able to return to regular job duties.

Employee was drilling a 5/8" holes in 3"x3" angle iron with a Wilton drill press. The piece pulled up out of the vice and grabbed EE's welding glove on EE's left hand. The drill twisted EE's hand and arm into the bit until it pulled off the glove and the bit broke. This accident resulted in a hairline distal fracture of the left ulna.

The roof bolting machine was moving to another entry, there was approximately 15 feet of cable slack behind the machine. The cable reel retracted the slack and the machine began to tram forward, the miner was struck by the cable hanging from the mine roof when the cable was pulled down from the hangers. After medical evaluation, the miner has been placed on restricted duty.

Employee was using a duke to raise shields, the duke slipped off of safety chain and hit employee's right ankle against pan sigma. At time of injury no medical attention was sought. Recent medical evaluation (10-17-2016) revealed a bone fracture from the incident.

Employee was working in shield #36 on the longwall face. A support (Duke) had been placed from the pan to the shield canopy, the Duke bled off and struck employee on the head. Employee momentarily lost consciousness, diagnosed with a mild concussion.

Employee was filling a rock dust hopper on a roof bolting machine. As employee was filling the bopper, the machine trammed on uneven mine floor and pinched employee's foot between machine frame and mine floor. Employee was been placed on restricted duty with a fractured bone in foot.

The employee had right hand on the tugger rope just above a snatch block pulley. The employee wanted to put slack in the tugger roper but hit the in button on the controller by mistake pulling right pinky finger into the snatch block pulley resulting in a fracture to right pinky finger.

Employee was cutting bolts off of a Sandvik scoop, he was using a Saw-all to perform the task. As he was cutting the bolts, a piece of metal fell behind his safety glasses and lodged in his eye. He received a eye prescription for the the injury after the piece of metal was removed.

Employee was tightening trusses that had been installed on a previous shift. while he was pushing the truss gun onto the cable he caught his head between the roof and the truss cable and the operator of the gun control activated the hydraulic function tightening the cable.

Contract employee was using a cutting torch on the Main West four head roller when a hot piece of slag went into the employee's mouth. Employee was was restricted to light duty for one shift.

Employee was on the longwall face where the shearer was cleaning up the tailgate area. As the shearer was leaving tailgate area towards the headgate a 4" line was struck by shearer and the 4" line pinched employee against a shield.

Contractor was roof bolting, his top drill steel became stuck in the roof mesh, while bringing steel down, the wrench came out of the chuck, he reached to put wrench back in place and the drill steel struck his fingers. He was placed on restricted duty.

Employee was removing the muffler from a F250 pickup. The employee removed the brackets from the section next to the muffler and was in the process of removing the remaining brackets on the tail pipe section. When the muffler came loose, the bracket pinched the employee's left middle finger against the truck frame.

Employee was changing a hose on a longwall shield, shield was locked out. An adjacent shield moved causing some motion to the shield being worked on. Employee's foot was pinched and fractured a toe. He has been placed on restrictive duty.

After drilling first drill steel he put in second one. When bringing the drill box down along with the steels, the top one fell out of the bottom steel and the room. The drill steel hit the right eye.

Chipping rock floor with jack hammer, moved hammer and rested it on left foot, hit trigger and pushed hammer into left big toe.

While welding some hot slag fell onto some water on the mine floor. The water caused the slag to pop back up behind employee's welding helmet and safety glasses. The employee received a prescription for his eye.

Employee was spot bolting, as he was inserting the resin he felt pain in his back.

While bolting ribs, employee twisted his back as he was installing a truss bolt. Medical attention was not sought until July 19, 2010 and at that time he was placed on restricted duty.

Employee was in process of repairing long wall shield, he accidentally turned on a valve supplying pressurized emulsion to a hose that he was going to hook up to the shield. The pressurized hose struck employee in the head and also put emulsion into his eye. He received prescription medication for his eye.

Injury was walking between two roof bolters. One of which was being operated by other employee. The operator moved the machine to drill a new hole pinning other employee between the two machines. He was treated at the hospital ER for multiple bruises, contusions (pelvis, groin, lower abdomen and thighs) and released.

While installing a new spring board on a continuous miner using a roof jack to assist in the installation of the spring board, the jack handle slipped out and hit the injured miner in the right upper lip. The injury required stitches.

The employee was cleaning "Tek-Seal" material from the hopper of the Micon pump and his glove got caught in the auger.

Crushed between head of miner and rib.

Employee was reaching for some vent tube straps on the left side of the Fletcher roof bolter. He placed his right hand on the left boom retraction point; the bolter operator was retracting the boom and the employee's hand was caught between the retraction point and the frame of the bolter. All x-rays were negative but the employee was placed on restricted duty on 6/8/06.

EE was installing belt hangers with a Fletcher roof bolter. He had started drilling a hole & was ready to add a 3' extension steel. While pushing the drill steel back into the roof his left hand was on the steel. When the steel bottomed out it bent & caught his left index finger between the drill steel & the frame of the drill mast. Sutures were required to close the laceration.

At the time of the incident Employee was observing a co-worker hoisting a brake accumulator out of a CAT 988 wheel loader. As the accumulator was being lifted out of the loader it suddenly rotated on the end of the hoist cable and struck employee's right middle finger, pinching it between the accumulator and a handrail.

WHEN CHANGING A TRACK ON THE DOZER, THE CHAIN HOOK SLIPPED OFF THE BOGIE. THE EMPLOYEE'S ARM WAS CAUGHT BETWEEN THE BOGIE AND THE TRACK. THIS RESULTED IN A SIGNIFICANT CONTUSION TO THE EMPLOYE E'S RIGHT FOREARM. THIS INJURY RESULTED IN RESTRICTED DUTY FOR THE EMPLOYEE. HOWEVER, EE WORKED 4 ON/4 OFF SHIFTS & 4 SHIFT CANCELLED. HE RETURNED NEXT SHIFT AT FULL CAPACITY.

THE EE WAS POLISHING A SHAFT ON THE LATHE. WHEN THE EMERY CLOTH WAS PULLED UNDER THE SHAFT, THE EE'S JERSEY GLOVE WAS ALSO PULLED INTO THE ROTATING SHAFT. THIS RESULTED IN THE EE'S RIGHT MIDDL E FINGER(JUST ABOVE THE MIDDLE KNUCKLE) TO BE PULLED COMPLETELY OFF HIS HAND.

THE EE WAS REMOVING THE FRONT TRANSMISSION/DIFFERENTIAL ASSEMBLY FROM A SCRAPER. THE ASSEMBLY WAS SUPPORTED BY AN OVERHEAD CRANE. WHEN THE LIFTING EYES ON THE ASSEMBLY FAILED, A PRY BAR BEING USED TO SEPERATE THE ASSEMBLY FROM THE HOUSING STRUCK THE EE'S RIGHT HAND. THIS RESULTED IN A FRACTURE TO THE EE'S RIGHT HAND.

EE WAS PERFORMING MAINTENANCE DUTIES ON A D-11 DOZER USING A CUTTING TORCH. AS HE WAS CUTTING ON A TRACK PAD A PIECE OF METAL FLEW UP & STRUCK HIM ON THE FOREHEAD RESULTING IN A LACERATION THE LACERAION REQUIRED 5 SUTURES. THE D-11 DOZERWAS DONW FOR REPAIRS & WAS NOT BEING OPERATED

EE ARRIVED EARLY FOR WORK AND WAS CURLING HER HAIR IN THE LOCKER ROOM. SHE DROPPED THE CURLING IRON AND IT FELL ON HER EYEBALL AND BURNED IT. IT WAS HER RT EYE. SHE WAS SEEN BY THE ON-SITE PA AND THEN SENT TO THE ER. SHE THEN WENT TO AN OPTOMETRIST.

WHILE GRINDING OVERHEAD WITH PPE (SAFETY GLASSES WITH SIDE SHIELDS PLUS FACE SHEILD). TRASH GOT IN EYE.

EMP WAS SPRAYED WITH DIESEL FUEL WHEN A CONNECTION NOZZLE POPPED OFF FROM FUEL TANK CONNECTION. SOME OF FUEL SPRAY ENTERED EMPS EYES. THIS RESULTED IN A CHEMICAL BURN IN EYES WHICH REQUIRED TR EATMENT WITH PRESCRIPTION MEDICATION.

WHILE ARC GOUGING IN A TUB COMPARTMENT, DEBRIS FROM THE ARCING BLEW INSIDE EMPLOYEE'S WELDING HOOD AND BEHIND HIS SAFETY GLASSES.

WHILE WELDING IN TUB COMPARTMENT #22, THE AIR HOSE CAME LOOSE FROM FITTING ON AIR CHIPPER. THE HOSE BLEW TRASH AND DUST IN EE'S RIGHT EYE. EE WAS WEARING HIS SAFETY GLASSES AT THE TIME OF INCI DENT.

EMPLOYEE WAS OPERATING A HAUL TRUCK AND WAS IN THE PROCESS OF BEING LOADED BY A BE SHOVEL. WHILE BEING LOADED, A LARGE ROCK FELL FROM THE SHOVEL BUCKET AND HIT THE BACK OF THE HAUL TRUCK BED J ARRING EMPLOYEE IN THE CAB. AT THE TIME OF THE EVENT, IT WAS DETERMINED THE RESULTING INJURY WASA BACK STRAIN. FURTHER EVALUATIONHAS DETERMINED EMPLOYEE HAD A HERNIATED DISC. RESTRICTED 5-2-9

THIS EMPLOYEE REPORTED "HAVIG SOMETHING IN HIS EYE". A FOREIGN BODY WAS FOUND IN CORNEA. THE FOREIGN BODY WAS REMOVED AT THE HOPSITAL.

EMPLOYEE WAS USING AN ALLEN SOCKET ON AN IMPACT WRENCH TO INSTALL A PLUG. THE PIN HOLDING THE ALLEN HEAD TO THE SOCKET BASE FLEW OUT OF THE SOCKET BASE AND STRUCK THE EMPLOYEE'S LEFT HAND. THIS RESULTED IN A PUNCTURE REQUIRING SUTURES.

EMPLOYEE WAS CLEANING THE PAGE 757 DRAGLINE HOUSE WITH AN AIRHOSE. AFTER THE END OF THE SHIFT, HE FELT MATERIAL IN HIS LEFT EYE WHICH WAS CAUSING AN IRRITATION. RESULTING INJURY WAS CORNEAL FOREIGN BODY.

95149ED WAS AIR-ARCING ON GEAR SEGMENT OF A DRAGLINE WHEN HE RECEIVED A FLASH BURN TO HIS RIGHT EYE.

WHILE REPAIRING A CATWALK ON A HAUL TRUCK, EE REACHED TO KNOCK OFF A PIECE OF LOOSE MATERIAL. AT THE SAME TIME, A SECOND EMPLOYEE WHO WAS WELDING ON THE CATWALK RAISED HIS TORCH BURNING HIM ON THE RIGHT WRIST.

EMPLOYEE WAS INSTALLING A SUBSTATION GROUND PAD, UTILIZING CADWELD. WHEN HE IGNITED THE CADWELD, A PIECE OF SLAG FROM THE FLASH BLEW INTO HIS LEFT EYE RESULTING IN AN ABRASION TO THE EYELID WI TH SECONDARY INFECTION.

EE WAS CHIPPING SLAG AWAY FROM WELDS WHEN A PIECE OF SLAG FLEW INTO HIS LEFT EYE. CORNEAL FOREIGN BODY WITH PRESCRIPTION MEDICINE.

WHILE EE WAS TAKING OFF COVERALLS AT THE END OF HIS SHIFT, HE REPORTED THAT HE FELT LIKE HE HAD GOTTEN SOME DIRT IN HIS RIGHT EYE. THE EYE CONTINUED TO BOTHER HIM, SO HE WENT TO THE HOSPTIAL E MERGENCY ROOM FOR REMOVAL OF CORNEAL FOREIGN BODY.

HH WAS WORKING IN RING GEAR AREA OF DRAGLINE CLEARING. A SECOND EMPLOYEE WAS PERFORMING GRINDING ACTIVITY APPROX. 30 FEET AWAY. A PIECE OF SLAG BLEW INTO CALDWELL'S RIGHT EYE RESULTING IN CORN EAL FOREIGN BODY.

EMPL WAS UP IN AB OF 844 MARION 82000 DRAGLINE. WHEN CAB HEATER CAME ON EMPL FELT SOMETHING IN EYE.

EE WAS GRINDING A CRACK ON AN EXHAUST MANIFOLD MOUNTED ON AN ARBOR AND DRIVEN WITH AN AIR HANDHELD DIE GRINDER. THE DISC CAME APART STRIKING THE EE IN THE RIGHT EYE AND NOSE.
