Employee was drilling a hole with jackleg drill when the steel became stuck. Employee pulled drill down, steel fell out of hole striking the back of his right hand.
Employee was drilling a hole in the bottom to pin down a bolt tail. He attempted to pull jackleg drill out of hole when he experienced pain in his lower left back.
Employee was operating continuous miner all day. Near the end of shift he experienced pain in shoulder, limited movement in shoulder.

He reached through the opening of the canopy, directly behind the operators seat, to push the brattice to the side. While doing this, his left hand was above the canopy and his left index finger was squeezed between the roof and the canopy.
EE was operating a Joy 21 SC Shuttle Car behind a continuous miner. A lump of coal approx 2" X 2" X 2" was thrown from the boom of the miner and struck him on the left eye.
The roof bolters were installing bolts in a low area of the mining section. The EE was on his knees beginning to install a bolt, when the jack hammer and leg struck him on the left side of his forehead.
Employee was cutting stainless steel and incurred flash burn to eyes from errant plasma cutter arc.
EE was backing a lee norse 265 cont miner out of a cut the miner climbed onto a large stone. When the machine came off the stone, the operators compartment hit the bottom throwing his left shoulder against the roof canopy.
EE WAS TRAMMING THE MINER FROM ENTRY #2 TO ENTRY#3 IN THE EAST SECTION. HE WAS VISUALLY CHECKINGTHE CLEARANCE AT THE BACK LEFT OF THE MINER WHILE TRAMMING & DID NOT NOTICE THE NARROW AREA BETW EEN THE RIGHT RIB & THE MINER. HIS RIGHT LEG WASPINNED BETWEEN THE MINER FRAME & THE RIGHT RIB.
WHILE ROOT BOLTING THE JACK BUCKED AND BOUNCED LANDING ON TOP OF LEFT FOOT, PUNCTURING THE TOP OF THE FOOT AND BREAKING A BONE.
EMPLOYEE WAS DRILLING A HOLE TO INSTALL A ROOF BOLT. HE WAS USING A JACK HAMMER AND JACK LEG TO DRILL THE HOLE. THE BOTTOM OF THE JACK LEG KICKED BACK AND STRUCK HIM ON THE ANKLE.
EE WAS DRILLING A ROCK FACE, USING A JACK HAMMER. WHILE DRILLING THIS ROCK, IT FELL TOWARDS HIM & HIT HIM ON THE RIGHT FOOT.
EE WAS ROOF BOLTING IN APPROX 42 INCH OF COAL AND APPARENTLY STRAINED HIS LOWER BACK.
EE WAS STARTING TO DRILL A ROOF BOLT HOLE AND HIS DRILL STRUCK A ROOF JACK, WHICH WAS POSITIONED NEAR BY. THE ROOF JACK BECAME DISLODGED AND FELL STRIKING THE EE ON THE BACK OF THE NECK.
THE EMPLOYEE HAD JUST FINISHED BOLTING A PLACE & HE LEANED THE JACK JAMMER HE WAS USING IN AN UPRIGHT POSITION AGAINST THE RIB. WHEN HE TURNEDTO MOVE A ROOF JACK THE HAMMER FELL STRIKING HIM ON THE BACK OF HIS LOWER LEG BRUISING HIS ACHILLES TENDON.
THE EMPLOYEE HAD JUST FINISH DRILLING A BOLT HOLE. AS HE WAS LOWERING THE JACK HAMMER, THE DRILLSTEEL REMAINED IN THE HOLE. SUDDENLY THE DRILL STEEL FELL FROM THE HOLE STRIKING THE EMPLOYEES F INGER BETWEEN IT AND THE JACK HAMMER.
EMPLOYEE WAS RIDING A GOLF CART TO THE OUTSIDE AND HIS CART BECAME STUCK ON A HILL.HE ATTEMPTED TO PUSH THE CART AND STRAINED HIS LOWER BACK.
COAL BONDED TO THE IMMEDIATE ROOF.
HE WAS IN #5 HEADING, BOLTING. A PIECE OF ROCK DISLODGED FROM TOP AND STRUCK HIS LEFT HAND. ROCK STRUCK HIS HAND IN AREA OF KNUCKLE CLOSEST TO THUMB AND NEXT KNUCKLE.
MINER WAS RUNNING THE BOLTER AND A LOCKNUT HOLDING THE HAIRLINE TO THE HAMMER CAME LOOSE AND HIT HIM UNDER RIGHT EYE. HE WAS TAKNE OUTSIDE, THE WOUNDW AS WASHED AND THE BLEEDING STOPPED AND H E SHOWED NO SIGSN OF SHOCK OR ANY OTHER PROBLEMS. HE WAS ABLE TO GO BACK TO WORK, HOWEVER, THE COMPANY ELECTED TO TAKE HIM TO THE EMERGENCY ROOM FOR FURTHER EXAMINATION. X-RAYS WEE NEGATIVE
