Employee was cutting a bolt out of the section head drive motor. He laid the cutting torch across his legs (he was sitting cross-legged). He reached up with his left hand and torch fell off his leg and landed on his hand. He was taken to ER and treated for minor burns and was released to return to work on 10/11/10.

Mostrando los 18Employee was in the process of bolting using a two piece steel. After he drilled his starter hole he started his two piece steel and had his index and middle fingers caught causing a laceration.
Member was bolting top in 3 right crosscut, while swinging drill boom out he placed his right hand on the canopy and the canopy struck the rib mashing his right index finger.
EE injured his left thigh when he came in contact with the head of a Joy 1415CM. EE was tramming the miner, and got too close. The head struck him on the leg.
Employee was installing a 42"resin bolt in the roof. As he was pushing the bolt up the bolt bent & sprung out of the pot & the 8 x 8 plate cut his left forearm.
EE WAS MOVING BOLTS TO THE SIDE TO GAIN ACCESS TO THE OIL FILL LID. THE BOLTER OPERATOR WAS TRAMMING THE BOLTER MACHINE AND DID NOT SEE EE. THE MACHINE RAN OVER HIS FOOT (LEFT).
EE WAS REMOVING A WHEEL UNIT MOUNTING BLOCK FROM A SHUTTLE CAR. AS HE WAS CUTTING THE WELD FROM THE FRAME, THE FLAME FLASHED THROUGH FROM THE TORCH HE WAS USING & EXTENDED ABOUT 2'.THE FLAME CAUSED BURNS ON HIS FACE.
EE WAS BOLTING ROOF TOP IN #1 HEADING & HIS PUSHER BECAME FOULED IN THE DRILL CHUCK. HE PUT THE STEEL BACK TOGETHER & SWUNG THE POT TOWARD HIMSELF TO TRY TO BREAK IT LOOSE. THE COUPLING BETWEE N THE FINISHER & PUSHER BROKE IN TWO ALLOWING THE PUSHER TO SWING OVER & HIT HIM IN THE FACE.
EE WAS BOLTING IN #5 ENTRY INBY SPAD #7828 ON THE OFFSIDE OF BOLTER WHEN A PIECE OF DRAW ROCK 7' LONG, 3 1/2' WIDE, 4 1/2" THICK, FLIPPED BACK UNDER ATRS, LANDING ON RIGHT LEG, BREAKING LOWER FIBULA & TIBIA & 3 BONES IN RIGHT FOOT.
SUCTION WAS STOPPED UP ON ROOF BOLTER. OPERATOR HAD HIS HAND ON TOP OF THE POT. HE INSTRUCTED CO-OPERATOR TO RAISE BOOM. WHEN THE BOOM WAS RAISED HIS HAND WAS CAUGHT BETWEEN THE ROOF AND POT. THIS RESULTED IN THE MIDDLE FINGER ON THE LEFT HAND BEING AMPUTATED.
OPERATOR WAS ATTEMPTED TO DRILL STARTER HOLE. WHEN HE ENGAGED ROTATION HIS GLOVE GOT CAUGHT BETWEEN POT AND DRILL STEEL. THIS RESULTED IN TORN LIGAMENTS IN HIS RIGHT THUMB. EE WORKED UP TO VAC ATION 12/21/01 AND DID NOT RETURN TO WORK AFTER VAC 1/2/02 DUE TO INJURY.
EE WAS BOLTING TOP WHEN STEEL CURVED OUT HITTING HIS LEFT INDEX FINGER RESULTING IN FRACTURED FINGER.
EMPLOYEE WAS PUTTING UP FIRST ROW OF BOLTS IN 2 LEFT WHEN IN BY BIB ROLLED OFF HITTING HIM IN BACK. RIB WAS 8 FT LONG BY 31" WIDE BY 11" THICK.
EE WAS TAKING BOLTER CABLE DOWN & A PIECE OF ROCK ABOUT 4"X40"X30"FELL FROM BETWEEN BOLTSW & STRUCK EE ON THE HEAD, BACK & LEFT FOOT. HE WAS WEARING METATARSAL BOOTS & THE ROOF BOLTS WERE 3FT FROM RIB WITH 4 FT CENTERS
EE WAS BOLTING TOP & RIB ROLLED OFF HITTING HIM ON THE LEFT LEG & ANKLE.
EE WAS WALKING BACK TO GET A BOLT, WHEN A ROCK FELL (40" X 30" X 5" THICK) HITTING HIM ON THE THIGH AND LOWER LEG.
AS EMPLOYEE WAS RETRIEVING HIS FINISHER STEELE, HE PUT THE PUSHER IN THE TRAY AND THE STARTER FELL OUT, HITTING THE POT AND STRIKING HIM IN THE MOUTH. THIS ACCIDENT WAS NOT REPORTED TO THE SAFETY DEPT. UNTIL SEPT. 6TH, 2001.
EE WAS BOLTING TOP. HE APPLIED PRESSURE ON THE DRILL POT & THE DRILL STEEL POPPED OUT & STRUCK HIS HAND CAUSING A SLIGHT SPRAIN TO THE LEFT HAND.
