Mostrando los 17While drilling holes to anchor the section tail piece, the air line came loose from the drill and sprayed air and debris into individuals eyes, the foreign material in his eyes needed to be removed at the hospital and the Optometrist office.
While re-installing the relay bar pin to the pan line. A crib block was used to apply pressure to the relay bar, assisting in aligning the holes. The crib block flipped out and the shield rocked forward striking the individual's hand, causing a fracture to his right little finger.
While releasing the roof screen mesh from the top of the ATRS individual had right elbow pinched between mesh support plate and ATRS main frame. This caused a severe bruise to right elbow area, resulting in missing work to allow swelling to subside.
While installing the resin roof bolt into the mine roof by pushing the bolt in place with the chuck wrench, the wrench slipped off the head of the roof bolt and struck individual in the nose. This caused a small laceration that required 3 stitches to close.
While installing a roof bolt, the individual was rotating the chuck wrench aligning the end of the roof bolt and the chuck wrench. While rotating the chuck wrench, caught his left ring finger between the end of the bolt and chuck wrench. This tore fingernail loose. After examination it was determined that the fingernail needed to be removed.
Individual was reaching to remove a chain used to transport a longwall shield located within a shield haul vehicle, when the chain lift arm was activated to lower the shield pinching individual's right upper arm (humorous). The pinching of the lift arm fractured the individual's right upper arm.
After spraying some stoppings with Versa Foam polyurethane sealant, individual experienced shortness of breath and nausa. Taken to hospital and Doctor felt was having a reaction to the chemicals used in the foam. Doctor requested follow-up examination in two days for release to return to work.
While installing a roof bolt, a chuck wrench was used to help push the bolt into the hole. As push pressure was applied to the bolt and rotation started, the chuck wrench flipped free and struck individual in chin. This caused a 1/2 inch laceration to chin which needed stitches.
While helping install roof bolts, the injured individual was holding the wire mesh screen. The other operator raised the ATRS against the mine roof and the individual who was holding the wire mesh had his right hand pinched between the ATRS frame and the mine roof. This caused lacerations to all four fingers of his right hand and fractured the 3rd and 4th fingers.
While installing a roof bolt, the roof bolt plate became stuck against the drill mast. The individual moved the roof bolt plate to get it unstuck. As he moved the plate it popped free spinning and stricking the individual in the left ring finger. This caused a laceration to the back of the finger, which required stitches.
WHILE DRILLING HOLES IN ROOF TO BLAST FOR AN OVERCAST, INDIVIDUAL WAS USING A DOUBLE-BOOM LEE NORSE ROOF BOLTER. INDIVIDUAL MOVED BOOM TO REPOSITION AFTER LOWERING BOOM'S STAB JACK. THE CANOPY AND BOOM ROSE, PINCHING HAND AGAINST ROOF CAUSING A SMALL LACERATION BETWEEN 1ST & 2ND FINGERS THAT NEEDED STITCHES ON RIGHT HAND.
WHILE ROOF BOLTING WITH A STOPER ROOF DRILL, INDIVIDUAL HAD JUST COMPLETED INSTALLING THE ROOF BOLT. WHEN HE WENT TO LOWER HIS ARM IT WOULD NOT COME DOWN. INDIVIDUAL PUSHED AND FORCED ARM DOWN , FELT POP & PAIN. FOLLOWING AN MRI & DR'S APPOINTMENT ON 6/16/03 FOUND HAD SMALL TEAR TO ROTATOR CUFF IN RT SHOULDER. WILL TREAT W/THERAPY. HAS NOT MISSED ANY WORK, REPORT DUE TO POSITIVE DI
WHILE INSTALLING A RESIN GROUTED ROOF BOLT, THE INDIVIDUAL WAS RAISING THE DRILL BOX UP WITH A CHUCK WRENCH IN DRIL CHUCK. THIS WAS TO PUSH AND SPIN THE ROOF BOLT INTO HOLE. AS THE CHUCK WRENC H CONTACTED THE OF THE ROOF BOLT, THE JOINT MOVED AND PINNED INDIVIDUALS THUMB BETWEEN CHUCK WRENCH AND DRILL GUIDE BRACKET, CAUSING A 1/4" CUT ON RIGHT THUMB NEEDING 2 STITCHES TO CLOSE.
WHILE TRAVELING TO THE TAILGATE OF THE LONGWALL TO OBSERVE CONDITIONS, A SMALL BOUNCE OCCURRED, WHICH CAUSED A LUMP OF COAL TO COME OVER THE BACK OF THE PANLINE, STRIKING INDIVIDUAL IN RIGHT L OWER LEG CAUSING A FRACTURE TO RIGHT ANKLE.
WHILE ROOF BOLTING, DRILL STEEL BECAME STUCK IN HOLE. INDIVIDUAL PULLED ON DRILL STEEL WITH HAND. WHEN DRILL STEEL CAME FREE PINCHED RT RING FINGER TIP BETWEEN DRILL STEEL AND CHUCK WRENCH, CA USING A LACERATION TO TIP OF FINGER NEEDING STITCHES. RELEASED TO RETURN TO WORK 10-19-01. REPAIR SCHEDULED FOR 10-25-01. WAS OFF WORK UNTIL10-30-01 WHEN RETURNED.
WHILE PREPARING TO CONNECT HOSE TO ROCK DUST TANK OUTLET, THE 4" COVER CAP THAT WAS RESTRAINED BY A CHAIN WAS REMOVED. THE TANK STILL HAD PRESSURE IN IT AND THE CAP BLEW OFF WHEN THE CLAMPS WE RE REMOVED. THE RESTRAINED CAP FLEW AROUND AND HIT A GLANCING BLOW TO NOSE, CHEEK LT, AND UPPER LIP CAUSING SWELLING BRUISES AND ABRASIONS. THE CONTENTS OF THE TANK (ROCK DUST) BLEW INTO EYES
