Employee was starting to drill for their inside bolt. A piece of rock fell from the outside bolt to where they started the inside bolt. The rock hit the canopy and slid off landing on the back side of their right foot. This caused a sprain to their foot.
Employee was sitting on a bucket at the end of the boom loading a shuttle car with a miner. A rock fell off the boom and landed on both of EE's feet. This caused a laceration on EE's left foot that required 4 stitches and a bruise on EE's right foot.
Employee was starting to drill the outside hole. When the steel touched the roof, the bit broke and the up pressure caused the drill steel to bend in 2 places. EE hit the rotation causing the steel to spin around and strike EE's right thumb causing a fracture.
Contract employee was in the bolter training program under the supervision of a foreman. When the employee was trying to put the put the bolt in the pot, EE hit the up pressure instead of the down. The bottom of EE's middle finger got caught between the bolt and the pot. A skin graft was required to repair the finger.
Employee had just installed the first row of bolts in a head hole and was drilling the test hole. A piece of draw rock fell and struck the back of left arm causing a laceration that required stitches.
EE was in the process of installing a roof bolt on a corner. EE could not get the bolt started in the hole because of gob. The bolt was positioned on the right side of EE's body. EE switched sides and as the bolt went in the hole, the plate caught EE's cheek and caused a laceration which caused stitches.
Employee was in the process of installing a curtain board. Employee was between the canopy and board standing up. When they turned around their knee hit the joystick which caused the arm of the bolter to move up and catch their pinky between the boom and canopy post causing a laceration.
Employee was bolting and a rock fell from the top, down the pin steel, and hit employee on the wrist causing injury.
Employee was in the process of installing EE's inside bolt. A piece of rock flew from the opposite side drill boom and hit employee on the left check causing a fracture.
Employee was cleaning out the pre-dump on the bolter. While employee had hand in the pre-dump employee's partner closed it causing a laceration on employee's thumb.
Employee was loading a S/C with the miner. Employee had seen a rock come down the boom. Employee thought it was loaded on the S/C. After the S/C had left employee looked back at the face to continue cutting. The rock that was on the boom fell off and struck employee on left ankle. Employee continued to work until 11/18/2021.
Another employee was drilling with Jackleg drill. Drill got in a bind and kicked back hitting injured employee in head causing IE to fall backwards. Laceration on head behind left ear - 7 stitches.
Pinner steel got stuck in top, it fell out hitting employee on right middle finger.
Had a bent bolt close to top, tried to straighten it with rotation, the bolt came around and hit employee on left wrist.
Employee got their glove caught between steel and wrench while bolting. This caused their hand to be twisted.
Employee was tramming roof bolter from deck. Employee's hand slipped off tram lever. Employee's arm was caught between deck and rib.
Employee was putting up a roof bolt when EE mashed EE's right pinkie (little) finger between a roof bolt and pot of the roof bolter. On 08/11/2017 EE worked 5.5 hours then left to go to the doctor for a follow-up on EE's finger, at which time the doctor took EE off work due to a minimal distal fracture on the tip of EE's right little finger. EE began missing work as of 08/12/2017.
Employee stated was pushing finisher into hole that had just been drilled. It hung up and employee felt something pop in lower back. Employee did not miss any work until 04/10/2017.
Employee was spot bolting in return. Had finger over top of steel, pushing up to top. Caught finger between finisher steel.
Employee started drill steel when a small piece came off the steel cutting employee on the left arm just above the kevlar sleeve. Required 3 stitches.
Employee was pulling a rock duster hose when employee slipped and fell striking hand on a rock on bottom of mine floor. This resulted in a small laceration to left hand requiring 2 stitches.
Employee was installing a roof bolt when the plate wire caught glove causing injury to right arm. Employee did not miss work until 07/14/2016 when doctor removed employee from work for evaluation.

Using a hydraulic ram pressing a bearing in a Joy Miner frame, the ram slipped hitting employee in the right knee
MSHA has requested the filing of this Form 7000-1 in order to terminate a citation. EE informed mine personnel that he had back spasm on 4-24-15. EE informed mine personnel of previous back issues, and MRI shown a bulging disk. Excel Disagrees with citation believes no work-related accident occurred, and is completing form 7000-1 under protest.
He was putting up a roof bolt. The bolt bent. He accidentally hit the rotation. The bent bolt hit him on his left wrist resulting in a fracture.
He was in the process of placing bolt into pot when he bumped the rotation. A burr on the bolt caught his glove resulting in two facture fingers to left hand.
While in the process of installing permanent roof support, the plate was not uniform with the bolt pattern. When employee placed his left hand to straighten the plate he unintentionally applied up pressure and mashed the tip of his left index finger between the plate and mine roof.

Employee was in the process of installing the outside roof bolt in the last row, he had just put the two pieces of drill steel together and was in the process of starting to drill while applying up pressure when the collar broke resulting in the bottom piece striking him on the left side of his face.
Skill saw kicked back while cutting steaks for concrete forms cutting right thumb of employee requiring surgery on 8/10/12.
While in the process of removing the two piece drill steel from mine roof, the top piece separated from the bottom piece. Bottom piece fell striking injured employee on his pinky finger resulting in a amputation to the tip of finger.
On 10/3/2011 employee was installing permanent roof support, while straightening the bolt to place in the church of the pot, employee felt pain in right shoulder and arm. Employee continued to work, but the pain worsened at which time he was seen by his family doctor on 10/24/2011 and removed from work.
Employee was bolting in #5 entry on 7 section. Drilling hole when steel broke striking employee on safety glasses causing a small cut under right eye. The cut require two sutures.
EE was drilling a hole. His glove was caught between the two pieces of finisher steel twisting right wrist. He continued to work until 5/18/11 when doctor took him off. No broken bones. The doctor told him to rest, and not use hand as treatment.
While in the process of removing two pieces of drill steel from mine roof, top section of steel became lodged and separated from bottom section of steel. Employee placed his left hand on bottom, top section fell striking employee's left thumb resulting in a laceration.

While in process of using a auger drill, employee was drilling a hole for anchor pins, drill got hung in hole, causing drill's handle to spin, striking employee's left wrist, resulting in a scaphoid fracture.
Injured employee was installing permanent roof support when he cut his left index finger on a roof bolt plate.

The drill steel was hung in the hole, as Injury tried to remove it, his hand was caught between the roof and the drill pot cutting his left index finger. The cut required four sutures to close.
Employee was positioned near a coal rib, while the operator of a dual boom roof bolter was moving the machine. The roof bolting machine moved suddenly to the right resulting in contact with the injured employee. Injured employee is currently waiting to see a specialist.
