Injured Employee was torqueing a CAT 795 haul truck with a RAD 34 torque gun. Injured employee received a dislocation, laceration and break to EE's right thumb when it became caught between the extension of the RAD 34 and wheel motor.
In the process of moving the Rad 15 impact gun from one nut to the other, the injured employee received a fracture and laceration to EE's left ring finger when EE's finger became lodged between the Rad 15 impact gun and the wheel.
A technician was welding in close proximity to other welders inside a rev. frame. The light reflected off the walls behind and came in behind EE's welding hood causing EE to receive flash burn to EE's eyes. EE was given a prescription of eye drops.
The employee was using a handheld grinder with a cutting wheel to cut off a section of handrail, when the tool kicked back, striking EE in the lower leg, which resulted in a laceration that required stitches.

The tugger cables were being used to pull the front sloped house roof panel back into position when it became stuck. A millwright's left middle finger was pinched between the rope window edge and the channel frame of the panel when the panel shifted. The result was a laceration requiring stitches.
Employee was lifting a sheet of metal with the forklift to place on sawhorses. Sawhorses were not level and when employee tried to level the sheet of metal on them the metal fell and struck the employee on the back of the ankle. This resulted in a fractured left ankle.
Employee was using a hydraulic torque wrench (high torque) that slipped off the nut. The employee reached to catch the wrench at which time his left ring finger was pinched between the wrench and drag line hoist case
The technician's shoulder was jarred when the dozer he was operating rolled onto its side. He returned to work continuing his normal duties until he underwent a procedure and returned on June 6, 2014 with restricted duty.
The technician was assisting in moving a 2800 P&H shovel when the machine lost propel capabilities. The machine then rolled in reverse striking the pickup driven by the technician causing the driver to be pinned in the cab.
Using a rad 30 gun to tighten wheel bolts on unit 239. While positioning the gun on first bolt the trigger hit the wheel starting the gun pinching finger between gun and wheel causing a crush injury with a laceration to left index finger.

Welder's helper and welder working on haul truck. The helper was standing near the welder when the stinger of the welding machine made contact with the back of the helper's hand. The welding wire pierced through the glove and punctured the back of the helper's hand. The injury resulted in infection.
A maintenance planner's left hand was lacerated when he was struck by a piece of metal as he watched a dragline bucket lip shroud being installed using a hydraulic hammer.
Individual was using an impact wrench when the nut broke loose, pinching his finger between the wrench and the transmission. Stitches were required for the laceration.

Two employees were welding and cutting on the apron of 6147-4341 Scraper. The welder warned the other that he was going to start grinding. As the employee turned away from the grinder, a piece of foreign material blew in behind his glasses.

EE operating electric drill-bit became stuck-drill was wrenched from EE, causing fracture to back of right hand.
At the start of the shift the employee started up an abandoned ramp near a shovel dig face. Part way up he stopped and began to retreat. The dozer rolled over once and landed on it's tracks at the base of the face. The employee suffered trauma to his head and neck.
At the time of the incident the technician was using a grinder to cut a bracket that supported the steering suction tube on a CAT 793 truck. As the technician was cutting the bracket a metal fragment struck and became lodged in his right eye.
HT257 pre spotted on the left side of the shovel when the dig face came down striking the back of the truck. The front of the truck came off the ground and went back down to the ground jarring the operator. The operator had pain in the lower back and legs.
Loosening nuts with a rad gun, got his hand caught between the action arm and the rim of the wheel. Right index finger open fracture.
While running 316 scraper, the operator was turning off the main haul road into Thunder Cloud and hit a pothole causing the seat to hit hard while seat belted resulting in a back injury.

The employee was using an oxyacetylene torch to cut dragline cables. He shut the torch off and rubbed his glove across the tip to remove some slag. The torch ignited and blew a hole throught the palm of his glove. The cotton liner ignited and caused a 2nd degree burn to his left hand.
The new employee was standing behind the trainer while she was washing a haul truck. He asked her about an overhead fixture and as both looked up, the pedestal-mounted water cannon slipped from her grip and flipped over, spraying the new employee directly in the face. His eye was lacerated and treated with antibiotic.
Haul truck was struck by a shovel dipper door jostling the truck driver. The truck driver had pre-existing degenerative disc problems which were aggravated by this incident. She sought chiropractic treatment.
Employee was lifting a jackhammer out of a crusher and he had his left hand on the bit. The trigger was inadvertently contacted while lifting it out, and the jackhammer was activated momentarily. The bit cut through his leather glove and cut the palm of his left hand about 3/4", and sutures were required to close the laceration.
Employee complained of back pain at the end of his shift, although there was no single event that caused any pain. A doctor examined him and concluded that a multitude of factors have contributed to this condition over time including weight, physical condition and operating heavy equipment. He was placed on bed rest and prescribed physical therapy.
EE TURNED ON FAN MOTOR FOR HEATER; THE HEATER OUTLET BLEW OUT A PUFF OF DUST & DIRT. EE REMOVED SAFETY GLASSES AND RUBBED HIS EYE. WHEN HE DID THIS HE RUBBED A PIECE OF DIRT INTO HIS RIGHT EYE.
WHILE ADJUSTING THE BELT TENSION ON A MOTOR RUNNING A BELT DRIVEN MACHINE BORING HEAD, THE MOTOR WAS TURNED ON BY THE OTHER EE WITHOUT CHECKING TO SEE IF THE INJURED EE HAD HIS HANDS AWAY FROM THE BELT. WHEN THE MOTOR TURNED ON IT GRABBED THE EE'S HEAVY WELDING GLOVES AND PINCHED IT BETWEEN THE BELT AND THE SHEAVE. RESULTING IN A SMALL FRACTURE TO THE LEFT INDEX FINGER.
Employee was taking down a welding hooch after welding and grinding operations. He went home at the end of shift and woke the next day with his left eye hurting. He visited the hospital where a metal particle and rust ring were ground from his left eye. He was also given antibiotic drops for his eye.
WHILE CHANGING A TIRE ON UNIT 184 THE RAD GUN POPPED OFF THE LUG NUTS STRIKING EMPLOYEE IN LEFT SHOULDER CAUSING A SPRAINED LEFT SHOULDER.
WE WERE USING AN I-BEAM & TROLLEY TO MOVE GEAR RACKS IN & OUT OF TUB. WE WERE PLACING A GEAR RACK INTO PLACE. THE EE WAS PULLING THE RACK, HE LOOKED DOWN TO PLACE HIS FEET AND THE TROLLEY RAN OVER HIS FINGERS.
EMPLOYEE WAS GRINDING ON THE SWING RACK OF A SHOVEL DURING WINDY CONDITIONS. THE NEXT DAY HE REPORTED THAT HE HAD SOMETHING IN HIS EYE. HE WAS TAKEN TO A DOCTOR, WHO REMOVED AN EMBEDDED METAL PARTICLE FROM HIS EYE.
HIT A ROCK WITH SCRAPER, SCRAPER STOPPED IMMEDIATELY CAUSING STRAIN TO OPERATOR'S BACK.
EMPLOYEE OPERATED 651 CAT SCRAPER, NEXT DAY HIS BACK WAS SORE.
TRANSFERING WATER IN FIXED PIPE SYSTEM - OPENED VALVE AND PUMP HOUSING FAILED. THE CASTING HIT HIM IN THE LEG CUTTING IT SUPERFICALLY. HOWEVER, AN INFECTION FLARED THREE DAYS LATER REQUIRING A NTIBIOTICS AND REST.
