Chipping rust with air chipper, rust/dust entered eye
While pushing roof bolt into hole the roof bolt stopped abruptly and the employee felt pain in abdomen. The employee was diagnosed with a hernia and worked till 5/13/2016 when employee went off for surgery.
Burning with torch. Torch came into contact with right hand.
Caught his left ring finger between the centralizer and the drill steel while bolting. Diagnosed with laceration and a simple fracture to the tip of his left ring finger. Received 4 sutures.
Employee was rock dusting when the rock dust hose came apart at a joint and struck EE in the face and blew rock dust in eyes. The employee was diagnosed with concussion symptoms on 11/25/2015 and taken off work.
EE was Rib Bolting. Steel was stuck and then broke, coming back and hitting EE on the head in the LT Temple area. EE was transported, via ambulance, to WHS-Greene. He received 6 sutures and a CT scan. The CT showed some possibility of a Skull Fracture and EE was Transported to Ruby Memorial.
REPOSITIONING A FULL BOX OF RESIN ON TOP OF CM AND NEEDED MORE LEVERAGE. EE STOOD ON HEAD OF BOLTER. EE REACHED FOR RESIN AND A PART OF HIS CLOTHING OR PIT BELT ENGAGED TIMBER JACK LEVER, DROPPING JACK AND PINCHING LT RING FINGER. EE REQUIRED 9 SUTURES TO CLOSE INJURY. THIS IS BEING SUBMITTED AS A MEDICAL TREATMENT.
EE was standing on the side of the bolter to put in a drill steel. The bolter moved or flipped toward the belt and pinned EE's knee between bolter and structure. EE used 1 day (1/20/15) for medical treatment. This is being reported as a NDL (Medical Treatment)incident due to EE going to physical therapy.
While roof bolting, ee was picking up gopher drill to move to another area. EE felt a strain/pain in his lower back.
EE was installing rib bolts. When placing wrench on head of bolt, a piece of draw slate fell behind ee, causing him to move foreword. This , in turn, made the wrench speed up and push ee's rt hand into rib between the bolt head. EE received 12 sutures between thumb and forefingers. EE's lost time started 9/11/14
Injured was standing in front of 94 shield. As shield came in, his Rt foot was caught between the pontoon and the panline. Injured was able to pull his foot out but suffered two broken toes and a laceration.
Employee was bolting on R side of miner and the starter was off too much to drill the hole straight. Employee used pushes to leverage bound up drill steel kicked out striking employee in R arm causing simple fracture.
Employee smashed finger between gopher drill and roof bolt.
Employee were setting a conveyor belt tail piece. A hydraulic jack was being lowered and he placed his hand on it. His right hand was on the jack and his right thumb caught between two stages of the jack lacerating it. This required five sutures to close. NO TIME WAS MISSED FROM WORK. THIS IS A MEDICAL TREATMENT INJURY.
EE was in the process of turning on a can duster to dust the beltline. The EE filled the duster and placed the lid back on the duster. After turning the air on the duster lid blew off striking the EE in the wrist, shoulder and head.
Putting in an 8' cable bolt & while holding the glue with 1 hand & the bolt with the other the end of the bolt struck the timber jack lever. His left hand was caught between the roof channel & the roof when the jack pushed the channel up. This lacerated his index & fourth fingers & fractured & lacerated his small finger. Began losing time next scheduled shift.
Drilling a 1/2" hole into stainless steel and the drill bound up causing his right hand to strike the box resulting in a one inch cut on his right index finger that required one suture to close. No Time was Lost.
Injured was assisting co-worker on center bolting. Employee did not realize that the steel was still in the roof. After removing driver steel the finisher steel unlodged itself from the roof falling and striking the employee in the right pinky finger knuckle.
Placed right hand on mast as it was descending and smashed right middle finger lacerating it.
Holding pusher steel while operator lowered drill head with fast feed, right foot was caught between steel and drill head causing a severe contusion. He began losing time on 2/24/12.
Installing a rib bolt and bent down to pick up something, when he went to raise up he inadvertently grabbed the rotation lever and his glove caught on the auger steel causing a compound fracture of the left middle finger and damage to two ligaments.
Clearing brush for borehole site and cut left knee with chainsaw.
Caught reinforcement while drilling causing EE's wrist to twist.
As the miner operator pulled the continuous miner back after sumping in the roof channel in the channel rack caught in the right rib. The channel flexed and sprung out of the rack and struck the injured in the back of the head and neck on his right side. The injured was standing at the rear left side of the full face continuous miner.
Drilling a hole for a center bolt and the connector broke. He reached up to get steels and the finishing steel fell out of the hole, striking him on the left index finger. The laceration took four sutures to close.
Injured was tightening a bolt on the axle of a 9 ton mantrip when he felt pain in his RT shoulder. Having an MRI the injured was found to have a rotator cuff tear and a ruptured bicep tendon of the RT arm. Injured work until surgery on 11/3/10. Injury is being reported as an occupational illness due to repeated trauma over 37 years of working in coal mines
EE was clearing site, cutting brush with a chain saw. Chain saw kicked back and cut EE's right leg above his boot.
EE was drilling a roof bolt, the roof was bad. While he was looking up, his right hand was resting on a guard over the pressure gauge. As he dropped the drill head with his left hand, the top of the jack caught his right hand between the guard and the head. EE fractured right 3rd finger with cuts and crushing injuries to his right 4th finger. No lost time due to this incident.
EE was conducting a repair to a vulcanized splice. EE was repairing the skive on the spice by using a low speed polishing sander. As the other EE held back the skive the EE started the buffing process when the grinder caught the belt and caused the sander to kick out of the skive hitting him in the forehead. EE received 4 stitches to his forehead.
Employee was assisting with threading a drill steel and got his hand wrapped up in the steel.
Injured was raising the drill head to put steels in drill chuck. Steels bent toward mast jack catching right middle finger. The finger was fractured and lacerated.
Victim was directing miner operator who was slabbing a new longwall face. The miner bits contacted the wood of a fold out bridge mat constructed of six inch timbers. Injured was thrown against the mine roof and struck by a timber. He sustained multiple contusions of his chest, trunk, legs, and two fractured ribs.
Cutting a broken beam when beam broke loose from side sheet (bolts were cut off) and pinched his right index finger on bottom deck stringer that was above the beam being cut. The laceration required five sutures. No time from work was lost.
Victum was positioning the Alpine miner shovel extension jack on the CM and it slipped. EE received a laceration on the left ring finger that required 5 sutures to close.
EE was in the process of placing miner cable and water hose on the Alpine miner head. The pump motor was running on the miner and he had the remote on his person. He was standing beside shovel and trammed the miner forward to put cable on the head, when the shovel extension caught him against the rib. EE received contusions to the spine, groin and abdomen.
Using a battery powered reciprocating saw to cut electrial conduit and, while holding it with both hands, his left hand slipped and his left small finger became caught between the saw blade chuck and the saw frame. The laceration required two sutures. No time was lost due to this incident.
After installing a roof bolt, victim was dropping the mast jack to remove the wrench. He had placed his left hand on the stabilizing cross member on the 2nd stage of the mast jack assembly. As the 3rd stage of the jack lowered, the bar on the 3rd stage caught the end of his middle finger and glove between 3rd stage and bar, severing tip of middle finger between 1st and 2nd joint.
He was a torch to burn off a shaft and the shaft fell striking him on the left side of his head. This resulted in a contusion to the head and laceration of the left ear. The laceration required five sutures. No time was missed from work.
Using a rock dust hose to apply rock dust in a high roof area, the dust hose was being held overhead and there was a quick surge of dust in it. The hose recoiled and he strained his right shoulder.
Using a two part steel to drill a hole & install a roof bolt. The lead steel stuck in the hole & he placed his right hand over the drill head to check for suction. As he did so, the steel fell out of the hole & struck his hand fracturing his right middle finger. THIS WAS NOT A LOST TIME INJURY.
Walking longwall face from the tail toward the head & was struck on the chin by a piece of coal that flew out from the shearer drum. The resulting laceration required two sutures to close. Injured did not miss any scheduled shifts due to the injury.
Putting drill pipe on the head, while shouldering up, drill pipe grabbed the rope and started spinning, wrapping him up with rope.
After drilling a hole while installing a roof bolt, injured was lowering auger and they sprung apart between the starter and extension auger, striking his left thumb. He suffered a laceration and dislocation that required surgery. He began losing time his next scheduled shift.
Installing 12 foot cable bolts in the tailgate set-up entry and the bolt bound up in roof screen. He was attempting to pull the bolt out when the bolt sprung back and struck him in the face and on the neck and shoulder. Injured received lacerations and abrasions. The cut on his forehead required six sutures to close. No work was missed due to this injury.
STANDING ON BARGE AS FLEET WAS BEING MANEUVERED WHEN HAUL CABLE JUMPED ACROSS BARGE STRIKING HIM IN THE BACK AND RIBS FRACTURING TWO RIBS.
RUNNING CABLE TOOLS, CAME OUT OF HOLE WITH TOOLS, MOVING TOOLS TO SIDE, RIG DID NOT LET TOOLS MOVE OVER. TOOLS CAME LOOSE ALL AT ONCE AND CAUGHT FINGER BETWEEN BOARD & TOOLS.
The weather was wet, pulling drill pipe back, hand slid down links tripped the line and his finger was smashed between links and elevators.
Using a water hose to clean debris from under slope belt. Belt was running. A mucker pump inby was running. He was lying down and spraying water when a piece of rock 3 inches x 3 inches came off mucker & struck him on the right side of his upper lip. The laceration required five sutures. No time was lost as a result of this injury.
As the shearer was mining from the tail to the head a piece of rock four to five inches in diameter fell from the roof, hit the tail drum, & was thrown. The rock struck injured in the nose & forehead, breaking his glasses & the bill of his hard hat & causing a 1-1/2 inch laceration on the bridge of his nose.
INSTALLING ROOF BOLTS AND AFTER FACEMAN PRESSURIZED THE JACK. INJURED STEPPED FORWARD WHEN A PIECE OF ROCK MEASURING TWO FEET X TWO FEET X -2INCHES THICK AND STRUCK HIM ON THE TOP OF THE HEAD.
