Employee was loading a shield into a shield trailer when the push rams activated smashing both feet

IE Grabbed a rotating drill steel causing their glove to get wrapped around drill steel, jamming their fingers into their wrist

IE went to the tailgate side of shear, opened the cabinet door to run the tailgate cowl off the valve bank While operating the cowl, the cabinet door closed striking their right forearm. IE was diagnosed with a hairline fracture. They returned to work their next shift

While bolting in 3 entry on 406 bolter the bolter helper was setting the ATRS when the left side bolter operator boomed in and smashed the helper's finger between the frame over the levers and the bolter boom tray. Left middle finger is the injury.

IE was bolting on the right hand station. IE was bolting in the roof on a corner at an angle. IE had the drill box all the way up. On the way back down IE had the left foot below the drill box. Once IE got the drill box toward the bottom IE smashed the foot. IE has a broken left, big toe. IE is in a boot and on crutches. At this point IE is seeking orthopedic care.

Injured Employee (I.E.) was tramming a 12CM12 continuous miner (company #317) into the face and positioned on the right side. I.E. was found pinned between the continuous miner tail and the rib. Current known injuries: 7 broken ribs (right) 4 broken ribs (left) and a collapsed lung. I.E. is stable.

IE caught left hand (second and third finger) in fan area of roof bolt machine.

IE was bolting in face. As IE was drilling IE's drill bit would not make a hold due to angle and was just spinning off of desired mark in a downward direction. As IE did IE's drill wrench came loose out of drill chuck and due to angle of drill steel caused the wrench to be propelled at IE's station, striking IE in the face on lower jaw.

IW was roof bolting. IW was holding the bolt with the plate above hand and just above the stationary portion of the jaw assembly. As the IW fed the bolt into the hole, the bolt caught on a ledge in the hole causing the bolt to bend smashing the IW's right little finger between the bolt and stationary portion of the jaw assembly.

IE attempted to catch the drill steel with right hand and the drill steel struck 4th digit finger. IE informed supervisor. Once IE arrived home after shift, IE removed first aid gauze and sought medical treatment since laceration would not stop bleeding. The emergency room diagnosed IE with a laceration and fracture to the tip of IE's finger.

IE was installing monorail hangers for the pump monorail. IE drilled the hole & installed the bolt. The drill box was raised to extend the second stage. IE had placed hand at the top of the first stage of drill mast frame. As IE lowered the drill box IE's fingers were caught between the second & first stages. IE broke one finger and dislocated another requiring surgery on 8/4.

IE was installing an 8' bolt, and the bolt began to bend. IE spun the bolt and as IE was rotating the bolt--the short extension snapped at the weld and a piece hit the injured above the right eye. Received 21 stitches in the forehead

While placing a mat on the ATRS, the IE's belt accidentally activated the "jaws" lever. EE had right hand on the jaw assembly which closed on EE's finger.

Employee (contractor ID A3234) was starting to drill first hole and got finger caught in the jaws. Employee was holding onto drill steel and lowered left hand/arm and inadvertently contacted the lever to close the jaws. Employee was taken to the ER for medical treatment. The employee received 10 sutures to close the wound.

EE was connecting an extension steel to the starter steel for drilling a bolt hole. He was feeding his steel up with the feed action of the drill box and at the same time the off side bolter was lowering the drill head off his bolt resulting in pinching and lacerating the flesh between his thumb and index finger. He was transported and received five sutures.

IE was bolting and he had finished drilling his first steel, and was inserting his second long steel. He was holding onto the steel, and over-applied feed pressure causing the steel to bend and pinch his left middle finger between the steel and the drill mast. First aid was applied, he was transported outside, inspected by an EMT then transported to the hospital.

IE was dusting in 5MN 2 Entry. His airstream helmet fogged so he placed the rock dust hose between his legs and raised his visor to clear the lens. The dust line surged, nozzle rose and sprayed him in the face with rock dust. Eyes were flushed at the mine, during transport, and at ER. Diagnosed as corneal abrasions to the right eye and given prescription antibiotics.

Injured contract employee (Stanhurst-Contractor ID A3234) was bolting in the tailgate. After approximately an hour of bolting, the chuck collar beneath the wrench on the bolter separated and one bolt struck the I.E in the right side of the mouth. This resulted in a bruise and laceration to the lip, a cracked tooth and 1 chipped tooth.

IE was welding/cutting when a small piece of metal entered his eye. He noticed irritation but had not pain, tearing or vision effects. IE was wearing a welding shield. The piece of metal was removed however a rust ring had begun to form. It was cleaned up and antibiotics were administered.

Employee was setting a bolt hanger when he began to spin the bolt the return of resin coming out of the hole flung off the plate and went around his safety glasses and into his left eye. It was believed this was filed on 4/4/13. It does not appear on the DRS so it is being submitted now. We have a printed copy on file from 4/4/14.

Employee states he was trying to unclog the hopper on a duster. As he reached inside the auger housing, a co-worker activated the hydraulic control, causing IE's middle finger to get caught by the auger. EE sustained a fracture and finger-tip avulsion of the middle finger.

Employee was feeding the steel to the roof. When the steel made contact with the roof the steel bent. Employee grabbed the steel so it wouldn't fall out while letting it down. When employee went to push the feed again he pushed it the wrong way, the steel swung around hitting employee in the wrist.

Employee was learning how to roof bolt. He was installing the second bolt of the row, Pushing the bolt to the roof. The hole was not straight and the roof began to bend. At this time the yellow extension wrench snapped and struck employee below right eye.

Employee was rockdusting when a surge in the hose caused rockdust to get into his eye. Injured employee was seen in the ER and diagnosed with a corneal abrasion and prescribed ointment to treat his eye.

EE was cleaning up tools, He was packing a 8" Grinder he placed on a load bed the cord on the grinder wrapped around his tool belt pulling the grinder down. It fell and landed on his left hand pinky finger. Partial amputation.

The employee states he was installing roof bolts and fingers got caught between steele and mast. His ring finger and middle finger were fractured.

EE lying on side using die grinder. Debris entered corner of eye.

While grinding, debris entered corner of eye behind safety glasses.

Employee states while installing a roof bolt, the drill steel bent, pinching his left middle finger between the drill steel and the jaws.

While feeling to verify that eyelets were lined up the injured employee had his fingertip pinched off when another employee lowered the inner link. The right index finger was pinched off between the back of the nail and the first joint.

The TRS had lost pressure due to a leak. The employee was trying to find which hose was leaking by moving the hoses around. He told his partner to bump the lever, his hand was on the hose that was leaking (unknowingly), the hyd. fluid went through his glove and made a small puncture hole in his hand allowing hyd. fluid to enter his hand.

Employee and two co-workers were in the process of lowering a water line pipe to the ground. EE was using a drill steel to release the pipe from the chain when an s-hook failed, letting the pipe fall. The drill steel was knocked out of employee's hand and it whipped around and struck employee on the forehead.

Employee stated he was standing under LW shield #3 removing rock props when the shield operator bumped out the shield flipper. The flipper struck him on the left shoulder. He was examined for the bruised and strained shoulder; he did not miss any scheduled time from work. He had surgery on Friday June 19, 2009 which is the first missed work day.

Employee states as he reached out to steady a fellow employee, he placed his right hand on the lower portion of the roof bolting machine for support. Simultaneously his middle finger was caught between the lower carriage assembly and internal carriage slide of the bolter, crushing the tip of his right middle finger.

Employee was inside the drum of the CC Centrifuge #2 installing some bolts when the unit was inadvertantly energized. Employee received cuts on his left hand and left knee which required stitches.

Employee states he and a co-worker were aligning and setting a winch roller assembly on a concrete pad. Before setting the assembly down, EE reached under the assembly with a prybar to clean out a chunk of coal, when the hyd. release for the porta power holding the assembly up was hit releasing the assembly and it came down and struck EE's right hand.

Employee stated a piece of metal entered his left eye as he was grinding handrail. Employee states he was wearing saftey glasses. Medical treatment.

While using a cutting torch, a piece of slag came over his glasses and got in his eye. A small piece was removed and he returned to work.

Employee states he was being instructed on operating the levers on the roof bolter, when a wrench fell and hit the top of ee's left knee cap.

Employee states a bolter was plugging up and dust and rock blew out, getting debris behind his glasses and embedding in his left eye, which had to be removed.

During the raising process the workdeck winches continued to run and chains broke causing the workdeck to yo-yo. During the yo-yo process employee fell approx. 25 to 30 feet to shaft bottom. Two other employees were injured on the workdeck but did not fall off.

Operating roof bolter, attempted to pull bent drill steel out of roof, acidentally hit rotation lever, which caused drill steel to spin out of his hand, coming back and striking him on right wrist.

EE states he was transfering roof bolt plates from the back of th bolting machine when he slipped and his feet went between the drill boom platform and step located on the roof bolting machine. His left foot was caught by the forward moving cat track assembly resulting in four fractured bones in his left foot.

EMPLOYEE WAS STRUCK ON THE EAR BY A PIECE OF COAL. WOUND REQUIRED 3 STITCHES.

THE INJURED PLACED HIS LEFT HAND ON THE DRILL MAST, REACHED UP WITH HIS RIGHT HAND, AND GRASPED THE DRILL STEEL TO REMOVE IT FROM THE DRILL HOLE. DURING THIS PROCESS HIS LEFT MIDDLE FINGER WAS EITHER STRUCK BY OR CAUGHT BETWEEN AN UNKNOWN OBJECT AND/OR OBJECTS CAUSING CRUSHING INJURIES TO THE DISTAL PHALANGE OF THE LEFT MIDDLE FINGER.

WHILE ROOF BOLTING, EE INJURED HIS WRIST DURING THE PROCESS OF ADDING AND CONNECTING AN EXTENTION DRILL STEEL (STRUCK BY DRILL WRENCH).

EE STATES HE WAS OPERATING THE CM WHEN A PIECE OF CAP ROCK FELL JUST INBY THE LAST ROW OF BOLTS. THE CAP ROCK STRUCK THE END OF THE VENTILATION TUBE, CAUSING THE TUBE TO FALL & STRIKE HIM ON THE HEAD & NECK.

EMPLOYEE STATES WHILE HE WAS BEING TRAINED ON THE ROOF BOLTER, HE CAUGHT HIS RIGHT MIDDLE FINGER IN THE JAWS. HE THEN PULLED THE WRONG LEVER, TIGHTENING IT AGAINST HIS FINGER. HE FRACTURED THE BONE AT THE TIP OF HIS FINGER AND THE LACERATION REQUIRED EIGHT STITCHES. HE HAS BEEN PLACED ON LIGHT DUTY FOR APPROXIMATELY ONE WEEK.

WHILE A SHIELD WAS BEING RAISED, A ROCK FLEW OUT, HITTING AND CUTTING THE LEFT WRIST. AFTER THE WOUND WAS TREATED, EE WAS ON LIGHT DUTY 4 DAYS.
